Patient Classification
Systems - refers to
measurement systems
in nursing that reflect
actual patient care
needs for staffing
purpose.
PATIENT ACUITY TOOL
PURPOSES:
● Categorize patients according their needs and the time and skill
needed to satisfy each category needs.
● Determine workload and nursing care requirements.
● Patient Classification/Acuity System (PCAS) is defined as “the
methods and processes of determining, validating and monitoring
individual patient/family care requirements over time in order to assist
in such determinations as: unit staffing, patient assignments, case mix
analysis, budget planning and defense, per patient cost of nursing
services, variable billing and the maintenance of quality assurance
standards” .
PATIENT ACUITY TOOL
Each patient is scored
on a 1-to-4 scale based on the
clinical patient characteristics
and the care involved
(workload)
● 1 - stable patient
● 2 - moderate-risk patient
● 3 - complex patient
● 4 - high-risk patient
PATIENT ACUITY TOOL
Each nurse scores his or her patients, based on acuity,
for the upcoming shift and relays this information to the charge
nurse, who then assigns patients before the shift change. The
goals of the tool are to:
● CARE PROVIDER.
● EDUCATOR
● MANAGER
● ADVOCATE

TRIAGE SYSTEM
Examples of triage acuity rating systems are listed below.
TRIAGE SYSTEM
SIMPE TRIAGE AND RAPID TREATMENT (START TRIAGE)
- most widely used triage system for mass casualty incidents.
BLACK – 0: Deceased
RED – 1: Immediate (severe injuries but high potential for survival
with treatment)
YELLOW – II: Urgent/Delayed (serious injuries but not
immediately life-threatening)
GREEN – III: Minimal (Walking wounded minor injuries)
TRIAGE SYSTEM
EMERGENCY SEVERITY
INDEX (ESI)
• Five-level emergency
department (ED) triage
algorithm.
• Assigns patients into five
levels from 1 (most urgent) to
5 (least urgent) on the basis
of acuity and resource need.
EMERGENCY SEVERITY INDEX (ESI)

TRIAGE SYSTEM
OTHER TRIAGE SYSTEMS (There are well-validated and reliable five-level
triage systems that have become the standard in other countries. Three of those systems
are described in Table 2-2.)

during intubation

INTRODUCTION
I. High Acuity Nursing
-focused on delivering intensive and
vigilant care to patients who are
acutely unwell, yet their condition
doesn't necessitate the intensive care
unit (ICU) level of attention.
- refers to the delivery of specialized care
to patients who are at a critical
juncture in their medical condition.
