La población costarricense con discapacidad es, desde el punto de vista odontológico, de las meno... more La población costarricense con discapacidad es, desde el punto de vista odontológico, de las menos estudiadas. Esta investigación exploratoria observacional descriptiva de corte transversal, tuvo por objetivo determinar la viabilidad de uso el Sistema ICDAS(International Caries Detection and Assessment System) en niños y niñas de 5 y 12 años procedentes del Centro de Enseñanza Especial "Fernando Centeno Güell" de San José, Costa Rica, así como conocer la prevalencia de caries dental y el tiempo que se requiere para examinar todos los dientes y superficies dentales de los sujetos de estudio con el Sistema ICDAS. Se localizan las lesiones cariosas y se registran la presencia de restauraciones y sellantes. Para ello, un dentista calibrado en el Sistema ICDAS (Kappa Inter e intra examinador 0.8 y 0.86, respectivamente) reclutó a los participantes de los Departamentos de Retardo Mental (RM) (n=19), Audición y Lenguaje o Discapacidad Auditiva (DA) (n=17) y Deficientes Visuales o Discapacidad Visual (DV) (n=6) del mencionado Centro. El criterio para la viabilidad se definió como la ausencia de uso de restricción f ísica activa, y la variable tiempo como el lapso requerido para completar el examen clínico de todos los dientes presentes. Resultados: En el grupo total de participantes, la viabilidad fue de 79% (n=33): 88,3% para DA, 73,7% para RM y 33,3% para DV. En relación con los códigos de lesiones ICDAS, el 2 fue el prevalente (49%), seguido por el 1 (18%), 3 (16%), 5 (10%), 6 (4%) y 4 (3%). La prevalencia de caries para DV (n=3) fue de 62%, para RM (n=14) 42.8%, para DA (n=15) 37,6%. El tiempo medio necesario para completar la inspección visual fue de 9 minutos; 8,4 y 10,2 minutos para los infantes de 5 y 12 años de edad, respectivamente. Conclusiones: Se corrobora la viabilidad de uso del Sistema ICDAS en los infantes con discapacidad. Para estudiar la prevalencia y el patrón de caries en esta población se requieren otros análisis en personas con discapacidad, y mayor número de participantes.
N(6)-methyladenosine (m(6)A) is the most prevalent internal (non-cap) modification present in the... more N(6)-methyladenosine (m(6)A) is the most prevalent internal (non-cap) modification present in the messenger RNA of all higher eukaryotes. Although essential to cell viability and development, the exact role of m(6)A modification remains to be determined. The recent discovery of two m(6)A demethylases in mammalian cells highlighted the importance of m(6)A in basic biological functions and disease. Here we show that m(6)A is selectively recognized by the human YTH domain family 2 (YTHDF2) 'reader' protein to regulate mRNA degradation. We identified over 3,000 cellular RNA targets of YTHDF2, most of which are mRNAs, but which also include non-coding RNAs, with a conserved core motif of G(m(6)A)C. We further establish the role of YTHDF2 in RNA metabolism, showing that binding of YTHDF2 results in the localization of bound mRNA from the translatable pool to mRNA decay sites, such as processing bodies. The carboxy-terminal domain of YTHDF2 selectively binds to m(6)A-containing mRNA, whereas the amino-terminal domain is responsible for the localization of the YTHDF2-mRNA complex to cellular RNA decay sites. Our results indicate that the dynamic m(6)A modification is recognized by selectively binding proteins to affect the translation status and lifetime of mRNA.
Over the last decade, capstone design courses have progressively required more and more sophistic... more Over the last decade, capstone design courses have progressively required more and more sophisticated as well as specialized design and manufacturing processes as the entire product realization process has been incorporated into these courses. At the same time, the general population of engineering students has less hands-on shop experience than their predecessors, required manufacturing-oriented coursework in curriculum has decreased, and the complexity of tools used for detailing designs and handling materials have increased dramatically. To remedy this situation, the authors have implemented a learner-centered approach for creating and maintaining an internet archive of just-in-time videos that support the capstone design experience. The approach we have developed is responsive to competency gaps identified by the Society of Manufacturing Engineers and is aligned with five pillars of on-line learning advocated by the Sloan Foundation (learning effectiveness, accessibility, student satisfaction, faculty satisfaction, and cost effectiveness). The paper includes a methodology for video development that elevates skills of three distinct audiences within the capstone design program (undergraduate student users, undergraduate student authors, and graduate student mentors/faculty/professional staff).
Objetivos: El objetivo del trabajo es la descripción de las herramientas y algoritmos utilizados ... more Objetivos: El objetivo del trabajo es la descripción de las herramientas y algoritmos utilizados para la construcción de un prototipo mobile, con la capacidad de realizar un reconocimiento óptico de caracteres (OCR) de la imagen del empaque del medicamento obtenida mediante el uso de la cámara del dispositivo o su código de barras y obtener información farmacéutica. Metodología: Implementación de un prototipo para la plataforma IOS utilizando el lenguaje de programación Objective C y C. Resultados: Se implementó un prototipo capaz de reconocer texto en una imagen que incluye algoritmos de procesamiento de imágenes, algoritmos de procesamiento de texto y el estudio de técnicas algorítmicas para la manipulación y búsqueda de información en la base de datos. Discusión: Las técnicas utilizadas debieron contemplar aquellas limitaciones propias de la plataforma: tiempo de procesamiento y capacidad de almacenamiento. Se decidió trabajar con aquellos algoritmos y estructuras que presentaron el mejor rendimiento en ambos aspectos, en relación con los resultados obtenidos.
Es necesario para la administración de un hospital lograr un sistema que refleje en forma confiab... more Es necesario para la administración de un hospital lograr un sistema que refleje en forma confiable y continua el estado de la ubicación de los pacientes internados, así como de la disponibilidad de las camas libres (censo hospitalario). En nuestro hospital existía un error del 14% en el sistema en reflejar el censo hospitalario. En noviembre del 2004 se unificaron los sistemas existentes que informaban dicho censo, el mismo pasó a mantenerse desde la Historia Clínica Electrónica. Se realizaron dos estudios transversales antes y después de las mejoras para evaluar la eficacia de las mismas. Se observó que el error global disminuyo al 5% y que la sensibilidad en detectar el estado de las camas aumento del 86% al 95% (p <0.001). La efectividad del sistema mejoró disminuyendo las complicaciones sobre los pacientes y la organización.

Medical advances have experienced progress due to the emergence of Nanomedicine. As data grows exponentially, novel biomedical approaches and tools, such as Grid Technologies, are needed to manage this kind of data. The objective is to describe how Nanomedicine, Bioinformatics and Grid Computing are represented in the medical bibliographic databases Medline. A search of text words such as Nanomedicine, Bioinformatics and Grid Computing was performed in the bibliographic reference database MEDLINE. On each category results were analyzed by number and characteristics of obtained citations, MeSH terms included, Types of publications and periodicals, Language, Country of origin and Progress in number of citations per year. There is an accurate representation in MEDLINE of Nanomedicine and Bioinformatics, in contrast the lack of adequate MeSH terms that describe Grid Computing makes it difficult to specify the extent of its domain, there is a need of proper representation of Grid-Computing in bibliographic databases.

Games with the purpose of improving an individual's knowledge, skills, or attitudes in the "real" world are called "Serious Games". This paper describes the analysis and actions taken to redesign the Contingency Plan Procedure for the Electronic Health Record System of Hospital Italiano de Buenos Aires. After conducting a thorough analysis of the data gathered at post-contingency meetings, weaknesses were identified in the procedure; thus, strategic actions were recommended to redesign the Contingency Plan to secure an effective communications channel, as well as a formal structure for functions that may support the decision-making process. The main actions were: 1) to incorporate the IT Contingencies Committee (Plan management); 2) to incorporate the Coordinator (general supervision of the procedure); and 3) to redefine the role of the Clinical Informatics Resident, who will be responsible for managing communication between the technical team and Electronic Health Record users.
