The review discusses the role of pharmaceutical and drug design methodology in the development of... more The review discusses the role of pharmaceutical and drug design methodology in the development of the high-tech segment of the development and production of original drugs for personalized and precision medicine (PPM). Topical issues of using predictively personalized tools for precision medicine in pharmaceuticals are highlighted. New opportunities and prospects for increasing the effectiveness of drugs in various fields of medicine created on the basis of pharmaceutical design are shown. Discussed are original approaches to standardization and reproducibility of the effects of new drugs at the preclinical stage using microfluidic bioconstructs organ-on-a-chip systems. The relevance of improving university curricula for training specialists in the field of molecular drug design (drug design), harmonized with dynamically developing biotechnologies in pharmaceuticals, as well as the needs of domestic healthcare in original medicines for PPM, is shown.

Introduction: Implantation of unfractionated bone marrow cells (BMCs) into mouse hearts post-myoc... more Introduction: Implantation of unfractionated bone marrow cells (BMCs) into mouse hearts post-myocardial infarction (MI) can prevent a decline in cardiac function. However, clinical trials of BMC therapy have been less successful. While most mouse experiments use young healthy BMC donors, MI patients undergoing autologous cell therapy (i.e., receiving their own BMCs) are older and are post-MI. We reported previously that BMCs from old mice, or from mice that are themselves post-MI, are impaired in this therapeutic capacity; and that inflammation caused by donor MI mediates impairment of BMC therapeutic potential. As an indication of ongoing inflammation, we reported reduced levels of B cells in post-MI BMCs. Hypothesis: Because B cells have been reported to be a therapeutic subpopulation of BMCs, we asked whether B cell changes in bone marrow may account for the reduced therapeutic potential of BMCs from post-MI and/or aged donors. Рассмотрены ключевые проблемы перехода национальной системы здравоохранения на новую платформу пе... more Рассмотрены ключевые проблемы перехода национальной системы здравоохранения на новую платформу персонализированной медицины и, в частности, педиатрии. Во второй части, публикуемой в настоящем номере, анализируются основные механизмы построения новой модели. Представлены основные предпосылки (финансово-экономические, научные фундаментальные и научные прикладные), актуализировавшие в конце прошлого века ее появление и строительство в структуре глобальных трендов развития рынков здравоохранения. Показано, что одним из основных стимулов развития персонализированной медицины является заметный рост индивидуального и общественного спроса на превентивно-профилактические средства и соответствующий инструментарий. Все большую значимость приобретают разработки программ по управлению собственным здоровьем, разрабатываются и внедряются адаптированные к новой платформе бизнес-модели развития (в том числе государственного и частного партнерства). Рассмотрены доказательства того, что прогрессивный сценарий развития модели персонализированной медицины обеспечит значительное снижение расходов на медицинское обеспечение. Большой раздел статьи посвящен развитию положения о том, что понимание и осознание современным государством, врачебной аудиторией и индивидуумом-гражданином значимости охраны и управления собственным здоровьем,-важнейший аспект образовательной и просветительской деятельности врача и медицинской сестры. Детально рассмотрены перспективы персонализированной педиатрии как глобального инструмента реструктуризации всей системы здравоохранения.
Predictive preventive personalized medicine Liver cancer is the fifth most common form of cancer ... more Predictive preventive personalized medicine Liver cancer is the fifth most common form of cancer worldwide [1], with an incidence rate almost equals the mortality rate and ranks 3 rd among causes of cancer related death [2]. The coexistence of two life threatening conditions, cancer and liver cirrhosis makes the staging challenging. However, there are some staging systems, e.g. the Barcelona staging system for Hepatocellular carcinoma (HCC) [3], that suggest treatment options and management. Whereas diagnosis in early stages gives hope for a curative outcome, the treatment regime for around 80 % [2] of the patients classified as severe stages only gears towards palliation [4]. An intra-arterial radiation approach, radioembolisation (RE) is ubiquitously applied as one of palliative approaches. Although, in general RE shows promising results in intermediate and advanced stage HCC [5], individual treatment outcomes are currently unpredictable. Corresponding stratification criteria are still unclear. We hypothesised that individual radioresistance/radiosensitivity may play a crucial role in treatment response towards RE strongly influencing individual outcomes. Further, HCC represents a highly heterogeneous group of patients which requires patient stratification according to clear criteria for treatment algorithms to be applied individually. Multilevel diagnostic approach (MLDA) is considered helpful to setup optimal predictive and prognostic biomarker panel for individualised application of radioembolisation. Besides comprehensive medical imaging, our MLDA includes non-invasive multi-omics and sub-cellular imaging. Individual patient profiles are expected to give a clue to targeting shifted molecular pathways, individual RE susceptibility, treatment response. Hence, a dysregulation of the detoxification pathway (SOD2/Catalase) might indicate possible adverse effects of RE, and highly increased systemic activities of matrix metalloproteinases indicate an enhanced tumour aggressiveness and provide insights into molecular mechanisms/targets. Consequently, an optimal setup of predictive and prognostic biomarker panels may lead to the changed treatment paradigm from untargeted "treat and wait" to the cost-effective predictive, preventive and personalised approach, improving the life quality and life expectancy of HCC patients. Abs against myelin basic protein/MBP endowing with proteolytic activity (Ab-proteases with functi... more Abs against myelin basic protein/MBP endowing with proteolytic activity (Ab-proteases with functionality) are of great value to monitor demyelination to illustrate the evolution of multiple sclerosis (MS). Anti-MBP autoAbs from MS patients and mice with EAE exhibited specific proteolytic cleavage of MBP which, in turn, markedly differed between: (i) MS patients and healthy controls; (ii) different clinical MS courses; (iii) EDSS scales of demyelination to correlate with the disability of MS patients to predict the transformation prior to changes of the clinical course. Ab-mediated proteolysis of MBP was shown to be sequence-specific whilst demonstrating five sites of preferential proteolysis to be located within the immunodominant regions of MBP and to fall inside into 5 sequences fixed. Some of the latter (with the highest encephalitogenic properties) were proved to act as a specific inducer of EAE and to be attacked by the MBP-targeted Ab-proteases in MS patients with the most sev...
