Depression is an illness that's so destructive and heavy to live with yet to the outside world it appears as nothing and is hard to get a understanding of from anyone who hasn't experienced it or so so doesn't live with it (All of which is symbolised in the opening sequence with the pedestrians looking through the car window at the main character).
