  "You're not a goddamn looney now, boy. You're a fisherman". This line potrays exactly how we view mental illness. We view it as a label, which noone can escape from it. Once you are labelled as menatlly ill, you can't escape it. Maybe that is why even thought many of the patients were by their own will there, they didn't leave. Because even them felt that they belonged there.
  Moreover this movies potrays exactly how 'Total' institutions work. Stripping people of their identities. There new identity now is how the institution views them. For example, mentally ill in the asylums, criminals in prisons.
