ÖZET Amaç: Omurganın üç boyutlu deformitesi olarak tanımlanan skolyoz, hastalarda hem fiziksel, hem de sosyo-psikolojik etkilere neden olmaktadır. Cerrahi tedavinin amacı hem skolyozun neden olduğu hastalıkların ve nörolojik kötüleşmenin önüne geçmek, hem de sosyo-psikolojik etkileri hafifleterek hastaların yaşam kalitesini artırmaktır. Hastanemizde cerrahi tedavi uygulanmış skolyoz hastalarının postoperatif yaşam kaliteleri ile bu yaşam kalitesini etkileyen faktörleri değerlendirmeyi amaçladık. Yöntem: Hastanemizde 2010-2012 yılları arasında cerrahi tedavi uygulanan 50 skolyoz hastası değerlendirildi. Yaşam kalitesi değerlendirmesinde, ameliyatın üzerinden en az 6 ay geçtikten sonra uygulanacak şekilde Skolyoz Araştırma Cemiyetinin Sağlık İlişkili Yaşam Kalitesi-22 (SRS (Scoliosis Research Society)-22) formu kullanıldı. Anket formu ile ağrı, imaj, fonksiyon, ruh sağlığı ve tedaviden tatmin olmak üzere 5 alt başflık halinde tedavi memnuniyeti değerlendirildi. Dosyalar retrospektif olarak tarandı. Eşlik eden hastalıklar, yapılan ameliyat, Cobb açısı, skolyoz tipi, ameliyat süresi, düzeltme uygulanan vertebra sayısı, preoperatif ve postoperatif hemoglobin düzeyleri, intraoperatif ve postoperatif kanama ve transfüzyon miktarları ile komplikasyonlar, ekstübasyon süresi ve yoğun bakımda kalış süresi kaydedildi. Bu parametrelerin SRS-22 skorları üzerindeki etkileri değerlendirildi. Bulgular: Konjenital tipte skolyoz ve ek hastalığı olanlarda SRS-22 fonksiyon alt grup skorlarının daha düşük olduğu, diğer alt grup skorlarının ise cerrahi tedavinin yaşam kalitesini artırdığını gösterecek şekilde yüksek olduğu bulundu. Diğer skolyoz tiplerinde ve ek hastalığı olmayanlarda ise SRS-22 skorları, cerrahi tedavinin yaşam kalitesini arttırdığını gösterir şekilde yüksek bulundu. Kadın hastalarda yoğun bakımda kalış süresi daha kısa bulundu, ancak bu farkın SRS-22 skorlarını etkilemediği gözlendi. Skolyoz tipi ve eşlik eden hastalıklar dışında, intraoperatif ve postoperatif döneme ait incelenen verilerin SRS-22 skorları üzerindeki etkisi gösterilemedi. Sonuç: Çalışmamızın sonucunda, cerrahi tedavinin, skolyozun tipi ve eşlik eden hastalıkların varlığına bağlı olarak fonksiyonel iyileşme sağlamakta sınırlı kalabileceği, buna rağmen SRS-22 genel skorları değerlendirildiğinde yaşam kalitesinde kesin bir iyileşme sağladığı gösterilmiştir. Bununla birlikte, yoğun bakımda kalış süresinin uzunluğundan bağımsız olarak yaşam kalitesinde iyileşme sağlandığı görülmüştür. ANAHTAR KELİMELER: Skolyoz, Yaşam kalitesi, SRS-22 SUMMARY Objective: Scoliosis, the three dimensional deformity of the vertebral colon, causes physical and social-psycological effects. Surgical repair aims to prevent the worsening of the diseases caused by scoliosis itself and neurologic outcome, and also to improve the quality of life by reducing the impact of social-psycological status. Our aim was to evaluate the quality of life in these patients after surgical treatment and the impact of different factors on this postoperative life quality. Method: Fifty patients, undergone scoliosis surgery between 2010-2012, were assessed for the study. SRS (Scoliosis Research Society)-22 questionnaire was used to determine the postoperative quality of life and was given to the patients at least 6 months after surgery. Five subgroups entitled pain, image, function, mental health and satisfaction, were assessed. The records of these patients were retrospectively reviewed for co-morbidities, the type of surgery, Cobb angle, the type of deformity, the durations of surgical procedures, the corrected vertebral segments, intraoperative and postoperative hemoglobin levels, the amount of bleeding, requirements for transfusion, the time needed for extubation and the duration of ICU stay. These parameters were assessed for their effects on SRS-22 scores. Results: The SRS-22 functional subgroup scores were lower in patients with congenital scoliosis and the patients with co-morbidities, whereas the other scores were high. In the other types of scoliosis and in patients without co-morbidities, all the scores were high, revealing that the surgical treatment improved the quality of life. In female gender, the ICU stay was shorter, however, this difference did not correlate with SRS-22. The intraoperative and postoperative parameters did not reveal a correlation with SRS-22 scores. Conclusion: SRS-22 scores revealed that surgical treatment improved the quality of life in scoliosis, irrespective of the duration of ICU stay. However, the type of scoliosis and co-morbidities may limit the functional outcome provided by surgery
