    Dear Respondents,

    The Alejo M. Pacalso Memorial National High School (AMPMNHS) with the
    implementation of the K to 12 Senior High School Program, particularly one of the requirements
    in the Subject Applied Research I, is undertaking a comprehensive survey of the Psychological
    Stress Management of Teachers of Alejo M. Pacalso Memorial National High School. I
    consider your cooperation in this undertaking to be very valuable. Specifically, the study aims
    among other things; (a) To determine how high or how low are the stress levels of teachers of
    Alejo M. Pacalso Memorial National High School, (b) to determine the extent does the workload
    and classroom discipline influence the level of stress among the teachers, and (c) to find out what
    are most of the teachers main coping mechanisms. The following attached questionnaire and
    checklist is designed to elicit the information that is needed.
    Moreover, this is to assure you that all the information obtained in this study will
    be kept in strict confidence. The identity of the institution will not be revealed in anyway as the
    report will only deal with aggregates as much as possible, please do not leave any item in the
    questionnaire and checklist blank.
    No total in-depth study has been far completed. It is my experience that the reason
    for this is that the questionnaire and checklist given are not answered and returned on time. The
    return of this questionnaire and checklist in 1 or 2 days is therefore earnestly requested.
