EXTRA CURRICULAR ACTIVITY FORM

August 31, 2017

Dear Parents,

With the forthcoming Investiture of our Scout on September 22, 2017, we prepare to have them
experience an overnight indoor camping activity within the school premises (auditorium) right after the
investiture that will last until 10am the next day, September 23, 2017.
We prepared activities as part of their scouting task, to apply what they have learned from their activities
in school. In line with this, we will request each scout to pay P150 (deadline SEPTEMBER 11) to cover the
following expenses:
 food (merienda & breakfast) *they will bring their materials to cook for dinner
 souvenirs and prizes
 materials for activities
Things to bring:
Bed rolls, Personal Hygiene Materials, Extra clothes, Slippers, food to cook for dinner, kitchen utensils (stove),
cap, art materials
We rest assure that the safety and orderliness will be well-supervised by the teachers.
We trust that there will be merit your kind approval.

Having considered the benefits of the activity, I am allowing my child to join. It is understood that appropriate
precautionary measures will be taken to ensure the student’s safety and security. Therefore, I do not hold the
leaders of the school responsible for any untoward incident that may happen beyond their control.
