Dear Sir:
Pleasant day!
The undersigned would like to request your permission to conduct a survey regarding the
following.
1. What do the parents know about the K-12 program?
2. What are the parents and students' reactions about the implementation of K-12
program when it comes to rationale?
3. What are the parents, students, and faculty members know about the
implementation of the K-12 curriculum in terms of its advantage and disadvantage?
4. What do the parents and students expect upon graduating from grade 12?
In line with this, may I ask permission to conduct a survey to the Senior High School
Graduating Students, the Faculty, and the Staff for a total of 100 respondents (50
students, 25 faculty, and 25 office staff). The questionnaires are attached with this letter.
Your permission and approval to conduct this survey will be greatly appreciated and will
help us to pass our subject.
