service provider in Kenya. Our goal is to become a global centre of excellence that promotes personal empowerment,
professional development, business growth, capacity building and technical assistance in Emergency Medical Services.

We are pleased to inform you that the we have developed a proposed Scheme of Service for Emergency Medical
Technicians which forms an appendix to this letter. The Scheme of Service has been finalized and is ready for adoption
and implementation.

The proposed Scheme of Service establishes four (4) grades of Emergency Medical Technician - Basic, six (6) grades of
Emergency Medical Technician - Advanced, and eight (8) grades of Emergency Medical Technician - Paramedic and
provides clear and concise job descriptions and specifications at all levels within the grading structure. Provision of
these details will no doubt greatly assist in the recruitment, deployment, retention and general development of the
Emergency Medical Services Function in Kenya.

SCHEME OF SERVICE FOR EMERGENCY MEDICAL TECHNICIANS, 2018.

1. AIMS AND OBJECTIVES


1.1. To provide for a clearly defined career structure which will attract, motivate and facilitate retention of suitably
qualified Emergency Medical Technicians in the Civil Service.

1.2. To provide for clearly defined job descriptions and specifications with clear delineation of duties and
responsibilities at all levels within the career structure which will ensure proper deployment and utilization of
Emergency Medical Technicians and to enable them understand the requirements and demands of their job.

1.3. To establish standards for recruitment, training and advancement within the career structure on the basis of
qualifications, knowledge, competence, merit and ability as reflected in work performance and results.

1.4. To ensure appropriate career planning and succession management.

2. ADMINISTRATION AND TRAINING SCOPE OF THE SCHEME


2.1. Responsibility for Administration

The Scheme of Service will be administered by the Principal Secretary, Ministry of Health in conjunction with the
Public Service Commission of Kenya and in consultation with the Permanent Secretary, State Department of
Public Service and Youth as well as the Salaries and Remuneration Commission. In administering the Scheme, the
Principal Secretary will ensure that its provisions are strictly observed for fair and equitable treatment of officers
and that officers are confirmed in their appointment on successful completion of the probation period. However,
in order to maintain standards in the profession, the provisions of the scheme may be applied by the County
Governments as appropriate.

2.2. Training Scope

In administering the Scheme, the Principal Secretary will ensure that appropriate induction, training
opportunities and facilities are provided to both entrants into the Scheme and serving officers in order to meet
the requirements prescribed by the Scheme. In particular, the Principal Secretary will ensure that serving officers
acquire necessary qualifications / specialization and experience required for both efficient performance of their
duties and advancement within the career structure. In addition, the Principal Secretary will ensure that all new
entrants into the Scheme undergo appropriate induction, coaching and mentoring. Officers should also be
encouraged to undertake training privately for self development. However, in all matters of training, the Principal
Secretary administering the Scheme will consult with the Public Service Commission of Kenya and the Permanent
Secretary, State Department of Public Service.

3. EMERGENCY MEDICAL SERVICES FUNCTION / SCOPE OF PRACTICE


The Emergency Medical Services function entails: emergency, urgent and non-emergency response to medical cases in
pre-hospital and out-of-hospital settings for all patients with minor injuries and illnesses as well as major trauma and
severe medical conditions. The Emergency Medical Services process involves a timely access to patients, rescue /
extrication, patient assessment, diagnosis, stabilization, emergency medical treatment, transport / specialized retrieval
to definitive care facilities and inter-facility medical transfers / referrals. Moreover, Emergency Medical Technicians offer
“Stand By” services at mass gatherings, high risk activities and remote environments away from definitive care centres
like hospitals.

Emergency Medical Technicians further provide technical advice and emergency medical care in various areas of
specialization that include but are not limited to Basic and Advanced Airway Management, Basic and Advanced Life
Support, Management of Obstetric and Gynecological Emergencies, Environmental Emergencies, Psychiatric and
Behavioral Emergencies, Circulatory Emergencies, Endocrine Emergencies, Neurological Emergencies, Geriatric,

Bariatric and Pediatric Emergencies, Musculo-skeletal Emergencies, Intravenous Therapy, Administration of Medication
(Pharmacological Interventions), Ambulance Operations (including Emergency Vehicle Operations and Emergency
Medical Dispatch), Disaster and Public Health Incident Management (using Incident Command System principles) as well
as Emergency Medical Leadership and Management.

More still, the Emergency Medical Services function entails the implementation of the Emergency Medical Care Policy,
2018 - 2030, formulation, implementation, review and interpretation of Emergency Medical Services legislation and
policies, standards, protocols, standard operating procedures, curricula, syllabi, guidelines and programs; management
and coordination of ambulance services at all levels; working with other members of the healthcare team in conducting
all health interventions; maintaining healthcare records and confidentiality of information relating to patients; provision
of safe patient care in a conducive environment; educating, training and mentoring of students both in clinical areas and
community services; referring patients to appropriate health facilities; referring experts, specimen and organs to
designated facilities; conducting research, quality assurance and improvement; and coordinating Emergency Medical
Services initiatives.

4. GRADING STRUCTURE
4.1. The Grading Structure

The Scheme of Service establishes four (4) grades of Emergency Medical Technician - Basic, six (6) grades of Emergency
Medical Technician - Advanced, and eight (8) grades of Emergency Medical Technician - Paramedic who will be
designated and graded as follows:-

4.3. Serving Officers

Serving officers will adopt and convert as appropriate to the new grading structure and designations provided in the
Scheme of Service though they may not be in possession of the requisite minimum qualifications and/or experience
prescribed in the Scheme of Service. For advancement to higher grades, however, officers must possess the prescribed
minimum qualifications and/or experience required for appointment to the grade.

5. PROVISION OF POSTS
A scheme of service does not constitute authority for creation/upgrading of posts. Any additional post(s) required under
the new grading structure must be included in the establishment proposal for consideration and approval by the Public
Service Commission of Kenya or the County Public Service Board as the case may be.

6. ENTRY INTO THE SCHEME OF SERVICE


6.1. Direct Appointment

Direct appointment will normally be made in the grades of Emergency Medical Technician - Basic III, Job Group
‘G’; Emergency Medical Technician - Advanced III, Job Group ‘H’ and Emergency Medical Technician - Paramedic I,
Job Group ‘K’; provided that the candidate is in possession of the minimum academic qualifications prescribed in
the Scheme of Service for that grade .

In exceptional circumstances, however, direct appointment may be made beyond these grades by the Public
Service Commission of Kenya / County Public Service Board on the recommendation of the Principal Secretary /
Chief Officer responsible for the Emergency Medical Services Function provided that the candidate is in
possession of the prescribed minimum qualification(s) and/or experience required for appointment to the grade.

6.2. Incremental Credit

Incremental credit(s) for approved experience acquired after obtaining the minimum prescribed qualifications
for the grade may be awarded at the rate of one increment for each completed year of approved experience
provided the maximum of the scale is not exceeded.

In granting incremental credit(s), any period of service stipulated as a basic requirement for appointment or
promotion to a particular grade will be excluded.

7. ADVANCEMENT WITHIN THE SCHEME OF SERVICE


The Scheme of Service sets out the minimum qualifications and/or experience required for advancement from one grade to
another. It is emphasized, however, that these are the minimum requirements, which entitle an officer to be considered for
appointment to the next grade. In addition, advancement from one grade to another will depend on:-
7.1. existence of a vacancy in the authorized establishment;
7.2. merit and ability as reflected in work performance and results; and
7.3. approval of the Public Service Commission of Kenya or the County Public Service Board as the case may be.

8. RECOGNIZED QUALIFICATIONS
The following are the recognized qualifications for the purpose of this Scheme of Service:-

8.1. A certificate of completion in any of the following areas: Emergency Vehicle Operations Course; Emergency
Medical Dispatch Course; Critical Care Course; or any other equivalent qualification from a recognized training
institution by the Emergency Medical Care Council.

8.2. A certificate of completion in Emergency Medical Technician - Basic Course; Emergency Medical Technician -
Advanced Course; Emergency Manager’s Course; Emergency Medical Services Instructor Course or any other
equivalent qualification from a recognized training institution by the Emergency Medical Care Council.

8.3. Diploma or a Post-Graduate Diploma in the following disciplines: Emergency Medical Services, Emergency
Medical Technology, Emergency Medicine, Pre-hospital Medicine, Critical Care Paramedicine; or any other
equivalent qualification from a recognized training institution by the Emergency Medical Care Council.

8.4. Degree (BSc.) in the following disciplines: Paramedical Sciences; Pre-hospital Medicine or a Degree in other
health related fields from a recognized institution by the Emergency Medical Care Council; .

8.5. Masters degree in any of the following disciplines: Emergency Medicine; Health Systems Management;
Emergency Management; Critical Care Paramedicine; or its equivalent qualification from a recognized institution.

8.6. Enrollment Certificate, Registration Certificate, Certificate of Completion or a valid Practicing Card / License in
Emergency Medical Services issued by the by the Emergency Medical Care Council.

8.7. Proficiency in driving and computer applications from any recognized institution.

8.8. A Supervisory Level Course lasting not less than two (2) weeks; a management course lasting not less than four
(4) weeks; or a Strategic Leadership Development Programme lasting not less than six (6) weeks from a
recognized training institution.

8.9. Such other qualifications as may be approved by the Public Service Commission of Kenya.

9. IMPLEMENTATION OF THE SCHEME OF SERVICE

The Scheme of Service will become operational with effect from 1st September 2018. On implementation, all serving officers
will automatically become members of the Scheme of Service.

10. JOB AND APPOINTMENT SPECIFICATIONS

EMERGENCY MEDICAL TECHNICIANS - BASIC: APPENDIX A


10.1. Emergency Medical Technician - Basic III. Job Group ‘G’

10.1.1. Job Summary - Duties and Responsibilities

This will be the entry and training grade into this cadre. An officer at this level will normally work under the
guidance of an experienced officer and under medical oversight.

The primary focus at this cadre is to initiate immediate lifesaving care to patients who access the Emergency
Medical Services System. Particular duties and responsibilities at this level will be of limited scope and
complexity. They include emergency response, patient assessment; diagnosis and treatment of minor injuries or
ailments.

This individual possesses the basic knowledge and skills necessary to provide basic lifesaving interventions with
minimal equipment while awaiting additional Emergency Medical Services response as well as to assist the
higher level personnel at the scene and during transport.

Other duties and responsibilities include but are not limited to:-

 being deployed to a Sub-County or Community Emergency Station or in an equivalent institution;


 working in a Basic Life Support unit as a patient attendant or an ambulance operator (driver);
 assessing patients, history taking, documentation and establishing emergency medical care needs;
 diagnosis and management of patients in Basic Life Support units and other pre-hospital care settings;
 undertaking appropriate referrals of patients and experts to designated facilities;
 ensuring safe custody for patients’ belongings.

10.1.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-

a) a certificate of completion in any of the following areas: Emergency Medical Technician -Basic Course;
Emergency Vehicle Operations Course; Emergency Medical Dispatch Course; Emergency Medical Responder
/ First Responder Course or any other equivalent qualification from a recognized training institution by the
Emergency Medical Care Council; and

b) Certificate in Computer Application Skills from a recognized institution.

10.2. Emergency Medical Technician - Basic II. Job Group ‘H’

10.2.1. Job Summary - Duties and Responsibilities

Duties and responsibilities at this level entail:


 being deployed to a Sub-County or Community Emergency Station or in an equivalent institution;
 working in a Basic Life Support unit as a patient attendant or an ambulance operator (driver);
 assessing patients and establishing their emergency medical care needs;
 planning and implementing care interventions;
 administration of basic medication to provide better outcomes;

10.2.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-


a) Served in the grade of Emergency Medical Technician - Basic III, for a minimum period of two (2) years;
b) Certificate of completion in Emergency Medical Technician -Basic Course or any other equivalent
qualification from a recognized training institution by the Emergency Medical Care Council;
c) Certificate in Emergency Vehicle Operations Course and a valid class BCE drivers license with Public Service
Vehicle license;
d) A valid practicing license / operating card from a recognized licensing institution by the Emergency Medical
Care Council;
e) Certificate in Computer Application Skills from a recognized institution; and
f) Shown merit and ability as reflected in work performance and results

10.3. Emergency Medical Technician - Basic I. Job Group ‘J’

10.3.1. Job Summary - Duties and Responsibilities

Duties and responsibilities at this level entail:


 being deployed to a Sub-County or Community Emergency Station or in an equivalent institution;
 working in a Basic Life Support unit as a patient attendant or an ambulance operator (driver);
 assessing patients and establishing their emergency medical care needs;
 planning and implementing care interventions;
 administration of authorized medication to provide better outcomes;
 ensuring a tidy and safe clinical environment and ensuring safe custody of patients’ belongings;
 maintaining and evaluating healthcare outcomes on patients and preparing individualised reports.

10.3.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-


a) Served in the grade of Emergency Medical Technician - Basic II, for a minimum period of three (3) years;
b) Certificate of completion in an Emergency Medical Technician -Basic Course and Emergency Medical
Dispatch Course; or any other equivalent qualification from a recognized training institution by the
Emergency Medical Care Council;
c) Certificate in Emergency Vehicle Operations Course and a valid class BCE drivers license with Public Service
Vehicle authorization;
d) A valid practicing license / operating card from a recognized licensing institution;
e) Certificate in Computer Application Skills and Basic Life Support from a recognized institution; and
f) Shown merit and ability as reflected in work performance and results

10.4. Senior Emergency Medical Technician - Basic. Job Group ‘K’

10.4.1. Job Summary - Duties and Responsibilities

Duties and responsibilities at this level entail:


 being deployed to head a Sub-County or Community Emergency Station or in an equivalent institution;
 providing emergency medical care education on identified needs;
 coordinating and facilitating the referral of patients, specimen, experts and organs to appropriate
facilities;
 creating a support supervisory plan, supervising and keeping records of Emergency Medical Technicians
under his or her care;
 maintaining Patient Care Records and preparing periodic reports;
 ensuring a tidy and safe clinical environment and ensuring safe custody of patients’ belongings.
 carrying out emergency care outreach activities.

10.4.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-


a) served in the grade of Emergency Medical Technician - Basic I, for a minimum period of three (3) years;
b) certificate of completion in an Emergency Medical Technician -Basic Course, Emergency Vehicle Operations
Course and Emergency Medical Dispatch Course or any other equivalent qualification from a recognized
training institution by the Emergency Medical Care Council;
c) a valid practicing license / operating card from a recognized licensing institution;
d) a valid certificate in Computer Application Skills from a recognized institution; and
c) shown merit and ability as reflected in work performance and results

EMERGENCY MEDICAL TECHNICIANS - ADVANCED: APPENDIX B


10.5. Emergency Medical Technician - Advanced III. Job Group ‘H’

10.5.1. Job Summary - Duties and Responsibilities

This will be the entry and training grade into this cadre. An officer at this level will normally work under the
guidance of a senior experienced officer and under medical oversight.

The primary focus at this cadre is to provide advanced emergency medical care using advanced equipment and
procedures to service users. This individual possesses advanced knowledge and skills necessary to provide
patient care and transportation for critical, emergent, urgent and non-emergency patients who access the
Emergency Medical System.

Specific duties and responsibilities at this level entail:


 being deployed to a County or Sub-County Emergency Station or in an equivalent institution;
 working in an Intermediate Life Support unit as an emergency care provider or an ambulance operator
(driver);
 assessing, history taking, diagnosing, planning, implementing emergency medical care interventions and
reassessing patient outcomes;
 administration of authorized medication; initiating basic intravenous therapies, providing autonomous
Basic Life Support interventions and supervised Advanced Cardiac Life Support interventions;
 maintaining the Patient Care Reports and keeping patient records;
 identifying, managing and referring emergency medical care cases; and
 referring patients, experts and specimen to appropriate facilities.

10.5.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-


a) A valid certificate in the Emergency Medical Technician - Advanced Course or a Diploma in any of the
following disciplines: Emergency Medical Services, Emergency Medical Technology; or any other equivalent
qualification from a recognized training institution by the Emergency Medical Care Council;
b) A valid class BCE driver’s license with Public Service Vehicle authorization; or any other equivalent
qualification from a recognized training institution;
c) A valid practicing license from a recognized licensing institution;

10.6. Emergency Medical Technician - Advanced II. Job Group ‘J’

10.6.1. Job Summary - Duties and Responsibilities

Duties and responsibilities at this level entail:


 being deployed to a County Emergency Station; to an Info-Desk at the national level or to an equivalent
institution;
 working in an Intermediate Life Support unit as an emergency care provider or an ambulance operator
(driver);
 assessing, planning and implementing emergency medical care interventions;

 diagnosing common emergencies, providing appropriate emergency medical care and reassessing patient
outcomes;
 administration of authorized medication, initiating basic intravenous therapy, Basic Life Support, and
Advanced Cardiac Life Support interventions;
 training Emergency Medical Technician - Basic personnel and first aid students;
 maintaining the Patient Care Reports and keeping patient records;
 identifying, managing and referring emergency medical care cases;
 managing community rescue centres; and
 referring patients, experts and specimen to appropriate facilities.

10.6.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-

 promoting collaboration and linkages with partners and stakeholders in Emergency Medical services;
 domesticating and ensuring compliance with emergency medical care standards, guidelines and ethics;
 providing technical advice on matters pertaining to Emergency Medical Services;
 formulating, reviewing and facilitating the national research agenda and promoting innovations in
Emergency Medical Services;
 ensuring performance management, technical assistance and capacity building in Emergency Medical
Services; and
 advocating for and mobilizing resources for the implementation of Emergency Medical Services as well as
planning, budgeting and managing facilities, assets and resources.

10.18.2. Requirements for Appointment

For appointment to this grade, a candidate must have:-


a) served in the grade of Deputy Director of Emergency Medical Services, for a minimum period of three (3)
years;
b) a Bachelor’s Degree (BSc.) in Paramedical Sciences from a recognized training institution by the Emergency
Medical Care Council;
c) Masters degree in any of the following disciplines: Emergency Medicine; Health Systems Management;
Emergency Management; Critical Care Paramedicine; or its equivalent qualification from a recognized
institution;
d) a valid practicing license from a recognized licensing institution by the Emergency Medical Care Council;
e) certificate in Senior Emergency Manager’s Course or a certificate in Strategic Leadership Development
Programme lasting not less than six (6) weeks from a recognized institution;
f) a valid certificate in Advanced Trauma Life Support, Pediatric Advanced Life Support, Basic Life Support,
Advanced Cardiac Life Support and a valid certificate in Computer Application Skills from a recognized
institution; and
g) shown distinguished administrative capability as well as possession of a broad experience in setting and
evaluating emergency medical care and healthcare standards and programs; and
h) demonstrated a high degree of professional competence, thorough knowledge and understanding of
national goals, policies and objectives and the ability to relate them to the management and functions of
Emergency Medical Services.
