1. Pursuant to Regional Memorandum No. 394, s. 2015, entitled "2015

Healthy You - Nutri Trivia Quiz Tilt", the Healthy You Secretariat & Tulong
Dukha Foundation, Inc. are inviting all elementary pupils, secondary
students and teachers to both public and private schools to join.

2. Please be reminded that attendance to this activity shall be on

voluntary basis only.

Healthy You Nutri Quiz Tilt

NUTRI QUIZ TILT MECHANICS
1. This HEALTHY YOU Nutri Quiz is open to all Elementary and Secondary School both public and

private sectors.

2. The objective ofthis Healthy you Nutri Quiz are the following:
> to reiterate the significance ofproper nutrition and the right physical activity to prevent
the disease ofobesity.
> to emphasize the importance of right nutritional values ofevery single food to be eaten

to nourish the body.

> to encourage the learners to have physical exercise as part of bodily regimen.

3. Every division may send (1) one School Representative per school in the elementary and
secondary level in the public and private schools.

4. School which will participate in Healthy You Nutr

i Quit 2015 must conduct their own school
wide Nutri Quiz Tilt to choose who will represent their school to the Regional wide Competition.
At least 25 learners mustjoin in the School wide Quiz. The Healthy You Secretariat will provide
25 copies of Certificate of Participation and (1) one copy of SCHOOL CHAMPION Certificate.

5. The Healthy You Secretariat will provide 5 copies of Nutrition Booklet Reviewer entitled as
Healthy You: a Handbook on Nutrition Trivia for the Young Ones to the school and 30% ofthe
questionnaire in the Regional Quiz will be derived from the said booklet.

6. Deadline of Pre-Registration isJuiv 15.2015 (Monday). Registration fee per school per level Is
(P2,500)twothousandfive hundred pesos, inclusive ofthe materials to be sentto the school
plus freight charges incurred. Delivery of materials will be sent thru LBC or any other courier
available in the area. Contact the HealthyYou Secretariat: (02} 411-93-11 or (02) 416-93-11 or
