    G - CERTIFICATE OF COMPLETION OFBASIC or ADVANCE ROTC/BCMT
    H - COLLEGE DIPLOMA & TRANSCRIPT OF RECORD(If Xerox, must be Authenticated by School Registrar)
    I - BOARD CERTIFICATE/BOARD RATING/PRC LICENSE/AFPSAT Result
    J - PHYSICAL EXAMINATION REPORT(Note: the executive check-up or annual physical exam results can be validated by The Surgeon
    General, AFP):

    K - COMMAND CLEARANCE (will be issued later; the following will be submitted with the application which will be used for the subsequent
    issuance of this Command Clearance)
    - MILITARY:
     Duly accomplished OA-2 Clearance Application Form
     Duly accomplished Personal History Statement (PHS)
     DLO
    - CIVILIAN:
     Duly accomplished A2 Clearance Application Form
     Duly accomplished Personal History Statement (PHS)
     Barangay Clearance
     Mayor’s Clearance
     Police Clearance
     NBI Clearance
     CFI/RTC Clearance

    L- LATEST DRUG TEST RESULT from duly recognized drug testing center of DOH
    M- CERTIFICATE OF EMPLOYMENT

    Compile these documents in four (4) folders with one (1) original and three (3) authenticated photocopy
    with proper index tabs.

    Note: Applicant must be a graduate of Basic or Advance ROTC
