    ACTIVITY COMPLETION REPORT

    I. TITLE OF THE ACTIVITY: First Quarter NSED 2019


    II. LOCATION/DATE: Ybañez Memorial Elementary School, Cantur-aw,
    Inangatan, Tabango, Leyte / February 21, 2019
    III. SUMMARY: Pursuant to Division Memorandum No. 063,s.2019 ( Conduct of
    1st Quarter earthquake and fire Drill/multi hazard CY 2019.

    The Ybañez Memorial Elementary School conducted the


    simultaneous school-based earthquake drill on February 21, 2019 at
    09:00 o’clock in the morning. Before the drill each grades section
    headed by their teacher advisers undertook an orientation inside the
    classroom where all the pupils of the school from Kindergarten to
    Grade Six should follow strictly the three important things during an
    earthquake. These are Covering, Holding and Ducking. Watching out for
    falling objects and keeping calm and focus to avoid panic. When the
    bell rung all pupils from Kindergarten to Grade Six should follow the
    three important things (Covering, Holding and Ducking) and hold for a
    period of time.
    Considering, if the shaking is done, all pupils and teacher advisers
    should smoothly and without rushing out from their respective
    classroom and vacate to the designated evacuation area in the school.
    The drill was done accordingly as expected. All the pupils were properly
    guided.
    The activity was successfully done and it also intensified the
    pupil’s awareness and preparedness regarding risk reduction during the
    natural disaster specially an earthquake.
    After the drill, Mrs. Mhalalil C. Lubat, Master Teacher I, initiated a
    post realization about the drill. She emphasized about the occurrence
    of an earthquake as well as the needs to conduct an earthquake drill to
    enhance pupil’s awareness on the safety measures before, during and
    after the disaster.
    IV. ACTUAL TARGET / OUTCOME: All the pupils from
    kindergarten to grade six undergo the conduct of the simultaneous 1st
    Quarter Earthquake Drill 2019 with the guidance of the teachers.
    V. PROBLEM ENCOUNTERED: Some of the pupils were panic and
    out of control by their adviser.
    VI. LESSONS LEARNED: We learn that next quarter earthquake drill
    before the conduct of the drill every pupil must be aware of the proper
    precaution during the drill that should be oriented by their advisers.
    Pupils during the actual drill should be guided to avoid rushing out.
    VII. SUSTAINABILITY / REPLICATION: The school, the staff and
    the pupils always undergo the earthquake drill with collaboration and
    discipline.
