We are currently conducting a study entitled, “Decision- Making of Grade 10

Student of San Juan National High School in their Chosen Track”. This questionnaire

ensures your full confidentiality so it is optional to put your name in the given space. The

purpose of this survey is to identify your track preferences, your demographic profile, to

formulate a proposal plan to guide students and ease confusion in choosing the right track

for Senior High School. Please answer TRUTHFULLY as the data gathered in this survey

will be important for the researchers.

III. What is the level of influence in decision-making of the respondents?
The track that you have chosen would help you in pursuing further specialization
towards your preferred career. A list of factors could have influenced you in choosing your
