STUDENT PORTFOLIO IN WORK IMMERSION

I. Description:

A student portfolio is a collection of documents that provides a description/overview of the work


immersion experiences.

Title Page

The title page is the first of the student portfolio. It includes the title, the submission
statement, grade level, academic track and strand, full name of the author and the month and
year of expected graduation.

Letter to the Readers
It is a one-page letter addressed to the readers informing them of what to expect from
the portfolio. The name of the author should be printed in capital on the right side of the page
four spaces after the last line of the last paragraph.

Acknowledgement
The acknowledgement is one-page expression of appreciation for the assistance given to
the student in the course of her/his work immersion. The name of the author should be printed
in capital on the right side of the page four spaces after the last line of the last paragraph.

Table of Contents
The table of contents lists the chapter titles, the first and second level headings and the
pages where they are located. The titles are placed near the left margin and the page number
near the right margin. The title and page numbers are separated by light dot lines. Example
format:

B. THE BODY

Introduction (1 to 2 pages)
The introduction contains a brief background regarding the work immersion in the K to
12 Curriculum and its importance, application or impact to the grade 12 students.

1. Objectives of the Work Immersion


The student enumerates what are to be expected from them at the end of the work
immersion program.

2. Duration and Place of Work Immersion


The student writes down the place and duration of the work immersion.

The Training Area (minimum of 2 pages)


The student identifies where the work immersion program was conducted. A picture is then
placed after the first paragraph. The photo will be labeled as:

1. History (Described the brief history of the training area.)


2. Mission (Write the mission of the training area/office.)
3. Vision (Write the vision of the training area/office)
4. Oganizational structure

The Training Experience


The student will write one paragraph that will be a brief description of the work immersion (2 to
3 sentences will do).

1. Journal (Minimum of 8 pages)


a. A daily narration of the experiences of the student as a trainee
b. Photo documentation supporting your journal. Not all journal entry is required to
have a picture
c. Each day will be separated by writing the date and day. Example of format:

November 25, 2019/Monday


Today was the first day of our work immersion program. All the students were
gathered as the Training Center for orientation with the OJT Coordinator from the
Human Resource Department, Mr./Mrs…………

November 27, 2019/Tuesday


I was deployed to work at the ………… , I metd my supervisor for the training,
Mr/Ms……..

2. Observation and Insights (1 page)


a. Narrate the observations in the training area and in the training program itself.
b. Discuss your own perception regarding what you have observed regarding the
conduct of work immersion.
3. Overall Reflection and Self-Appraisal (2 to 3 pages)
It is an account highlighting the overall reflection and self-appraisal of the learnings of the
student from the training. Discussion should include:
a. Reflection on the experience
b. Review of the experience
c. Evaluation of the training program
d. Appraisal of yourself regarding positive changes that you think you have undergone
after the training.
