SHORT CIRCUIT IN LIGHTING DB AT CEMENT MILL-2 RP PANEL ROOM

BREAK DOWN AND ROOT CAUSE FAILURE ANALYSIS REPORT

DATE OF OCCURANCE: 28/04/2020 TIME: 10:00 PM

1. INTRODUCTION: On 28/04/2020, 10:00 PM, Electrical Short Circuit occurred in DB installed at Roller
Press Panel Room; for Lighting of Z2 and Z3 area.

2. SUMMARY: The main Magnetic Contactor and its associated RCDs got damaged.

3. PLANT STATUS PRIOR TO STOPPAGE: Area Lights and stated magnetic contactor were in ON and
Normal condition since 7:30 PM.

4. EVENT: At 10:00 PM, the Lights went out, the DB was inspected and main Incoming Breaker found trip
due to Short Circuit.

5. ROOT CAUSE FAILURE ANALYSIS:

Following points to be considered before analyzing the situation:

KEY POINTS:

 The DB was installed during RP Project, so Operational Life of equipment is not a concern.
 Magnetic Contactor and CBs were as per installed Lighting Load, so equipment being of under
capacity and getting heated is to be ruled out.
 Short Circuit occurred at magnetic contactor which damaged the nearby RCDs. The outgoing
conductors were found OK, so any abnormality in field wiring is not the cause.
FINDINGS:
Followings reasons seem to be the probable cause of this breakdown.
 The energizing coil of contactor melted and produced enough heat to melt the insulation, in
such case the quality of contactor is not satisfactory since coil issue generally don’t cause this
much mess. (The wire energizing the coil was damaged all along its way, point towards such
issue.
 Poor contact inside contactor due to Dust accumulation since the contacts of low rating
contactors are not accessible for cleaning therefore their health is an issue to predict.
 The dust at input terminals, and looseness of terminals of contactor cannot be ruled out, as the
contactor was installed with intend to operate the DB from Field, resulting in lesser visits of DB.
6. ACTIONS TAKEN:

Following corrective actions have been taken:

 The scheme of “Operation from Field and Contactor Circuitry” has been permanently
removed.
 Breakers of better quality have been installed for the burnt ones.
 From now onwards, Lights to be switched ON/OFF directly from DB through breakers,
resulting in increased visual Inspection.
 The circuitry of all such DBs for field operation to be modified, to avoid any such incident in
future.
 Thermography test to be instantly performed on all such DBs by Planning Department and
maintain the practice in future as per schedule.(after every 3 month for DBs with older wiring
and every 6 months for newly installed DBs (in span of 3 years) .

7. FINAL STATUS:

DB has been overhauled during General Shift of 29/4/2020 and system energized.
