    AFFIDAVIT
    1. That I, JACKIELOU TORRES CRUZ, of legal age, single, Filipino
    and with resident address at 432 Lot 4 Block 2 Villa Marciana
    Subdivision, Barangay Daang Amaya I, Tanza, Cavite, Philippines,
    after having duly sworn to in accordance with the law do hereby
    depose and state:

    2. That I have been employed by the Chilis, Singapore from July 6,


    2012 until January 6, 2013.

    3. That I worked as Management Trainee in the said company


    located at Resort’s World Sentosa Singapore and I was
    responsible for the following duties:

     Job Description/ Duties and Responsibilities


    - Responsible for taking orders and delivering food to the guest
    tables.
    - Responsible for welcoming guest into the establishment and
    seating them accordingly and monitoring the guest waiting list.
    - Cashiering, bussing table and checking back on guest
    satisfaction at its’ finest.

    4. That I worked at least 44 hours per week and received a basic


    salary of 1,200 SGD per month.

    5. That the said business cannot provide a detailed Certificate of


    Employment because the company only provides a standard
    Certificate of Employment for their employees.

    6. That due to reason stated above, a detailed certificate of


    employment from the said company cannot be obtained even if due
    diligence and care were exercised in trying to secure the same.

    7. That I am executing this affidavit in order to attest of the foregoing


    facts of the above statement and for all legal purposes it may serve.
