Root Cause
1. Unsafe storage of material on the edges of building/scaffolding.
2. Lack of supervision and fail to notice loose materials on scaffolding.
3. Could have cause of operatives are mishandled the material during shifting (human error).
4. Activity could have been started without obtaining permission.
5. No method of preventive measure of falling objects.
Control measures to be implemented but not limited to:
1. Ensured no loose material kept on incomplete scaffolding.
2. Good housekeeping shall be maintained and ensure floor edges are free of loose materials
3. Permit to work system and proper control measures shall be enforced for when working at the edge of the
building.
4. Weather report checks shall be carried out on daily basis & advice the site supervision team for preventive action
5. Proper supervision control shall be ensured to avoid recurrence the scenario.
6. Isolation and adequate signage underneath required where scaffolding erecting in progress.
7. MS-RA for the particular activity should be refreshed randomly and followed.
8. Manuel handling shall be strictly follow to avoid falling of material from height.
