LAWRENCE D. GERZOG, an attorney admitted to the Bar of the State of New York,
and o the Bar of this Court, mindful of the penalties for perjury, affirms as follows:
1. I am the attorney for appellant DEWEY TAYLOR, having been appointed by this
pursuant to the Criminal Justice Act on March 11, 2014. Mr. Taylor was appealing his
con iction and sentence after trial in the U.S. District Court for the Western District of New
Y or . His appeal was decided in a decision dated March 2, 2016. The mandate in the case was
2. I prepared the voucher in a timely fashion, but had not prepared the required Form 27
tog along with the voucher. I inadvertently believed that I had submitted the voucher, and
I went to check as to why the voucher had not yet been paid, I realized that I had not
pre ared the Form 27 and therefore had not submitted the voucher. The voucher, along with the
req isite Form 27, has now been submitted. I apologize for my error and for the delay in
