    PREPARE THE ORIGINAL AND PHOTOCOPY OF THE REQUIREMENTS.
    PHOTOCOPIES WILL BE GIVEN TO THE COURT.

     Barangay Clearance (Original and Photocopy/Xerox)
     Community Tax Certificate o Sedula
    (Original and Photocopy/Xerox)
     Valid ID ng kukuha ng clearance(Original and Xerox)
     PH passport  TIN Card
     Driver's license  Postal ID
     GSIS / SSS UMID  Voter's ID
    Card  PRC ID
     PhilHealth ID  Company ID
     Clearance Fee – Php 60 (Sixty pesos)

    2. For Probation / Bail / Detainee (Nakakulong)


     INFORMATION of the Case o
    PROBATION Order (Photocopy/Xerox)
     Valid ID ng kukuha ng clearance (Original and Xerox)
     PH passport  TIN Card
     Driver's license  Postal ID
     GSIS / SSS UMID  Voter's ID
    Card  PRC ID
     PhilHealth ID  Company ID
     Clearance Fee – Php 60 (Sixty pesos)

    UMID Card  PRC ID
     PhilHealth ID

     Kung representative ang kukuha ng clearance:


     Valid ID ng owner o may-ari ng lupa
