    SUBJECT : VALIDATION REPORT ON THE 100% ACCOMPLISHMENT eNGP SITE


    PREPARATION AND PLANTING ACTIVITIES BY BAGONG BUHAY FARMERS
    FISHERMEN WOMEN IP’S OSY RELIGIOUS SECTOR ASSOCIATION AT BRGY.
    BAGONG BUHAY, ALICIA, ZAMBOANGA SIBUGAY.

    In compliance with the approved travel order, the undersigned hereby submit our
    inspection/validation report relative to the above-mentioned project. Attached are the supporting
    documents which form an integral part thereof, viz;

    A. PO request for inspection and payment (Form I)


    B. 100% inventory of the PO’s Physical accomplishment
    C. Form II Accomplishment and Inspection report.
    D. Pictorials
    E. Statement of Accounts.
    F. Statement of Work Accomplishment

    In view thereof, please be informed that per inspection/validation conducted, disclosed


    that the Bagong Buhay Farmers Fishermen Women IP’s OSY Religious Sector Association had
    accomplished the following;
