INTRODUCTION

The past few years saw rise of accidents in the construction industry; many of which

were serious while in some, fatal.

The Guidelines on construction Safety of Department Order No. 13 signed in 1998

mandates the OSH requirements for this sector. Site safety officers play key roles in the

implementation of effective OSH programs.

In the interest of ensuring the protection and welfare of workers employed in the

construction industry, the protection and welfare of the general public within and around

the immediate vicinity of any construction worksite as well as the promotion of

harmonious employer-employee relationships in the construction industry, and after

consultations with the stakeholders in the construction industry, taking into consideration

industry practices and applicable government requirements, the following guidelines are

hereby issued for all concerned.

We will promote in our workplace a work ethics and culture that will enhance the

personal conviction of management, employees and contractors to comply reasonably

with all applicable health, safety and environment protection standards, codes and laws.

We will pursue continuous improvement and strive to establish a high standard of

accident and illness prevention and environment protection.


SAFETY AND HEALTH PROGRAM

Safety and Health Personnel

The designated personnel will administer the safety and health program for the company.
The responsibility is as follows:

A.) Safety Man

1. Implementing the safety policies stipulated in the company’s


Health and Safety Manual by insisting that workers observed
and obey all rules and regulations necessary to maintain a safe
Workplace and safe work habits and practices.

2. Being knowledgeable of potential jobs hazards.

3. Assuring compliance with construction safety and health


Standard requirements.

4. Making regular safety inspections.

5. Establishing safety procedures.

6. Maintaining safety records.

B.) FIRST-AIDER

1. Give immediate temporary treatment in case of injury or illness,


before the services of a physician becomes available.

2. Maintains first-aid kits and required contents in the project site.

Job Site Inspection

The safety in-charge or other designated person will tour the job site and observe
potential safety/health hazards and develop a plan for safeguarding the workers
which may include the following:

1. Removing the hazard.


2. Guarding against the hazards as required.
3. Providing personal protective equipment and enforcing its use.
4. Instructing workers is safe work practices.
5. Coordinating protection of workers with other contractors, if any.
Periodic Safety Meetings

Safety meetings will be conducted during the duration of the site work activities
To convey safety information and discuss the content of the safety program, work site
Hazards, and other serious concealed dangers. The safety in-charge or safety designated
person will review the company’s safe work practices or other safety information with
workers. Whenever a new practice of procedure is introduced into the workplace, it will
thoroughly reviewed for safety. Minutes of the meetings shall be recorded and distribu-
ted by the safety in-charge or safety designated person and signed by all attendees. A
Daily toolbox meeting will also be conducted among workers and their respective super-
visors for the purpose of instruction, discussion and proper briefing on the planned work,
the assessment of past work, the possibility or actual occurrence of accidents at the work
areas, tips and suggestions on how to prevent accidents and etc.

Personal Protective Equipment

1. Head protection will be worn on job sites when there are potentials of falling
objects, hair entanglement, burning, or electrical hazards.

2. Eye protection will be worn when there are potential hazards from flying
objects or particles, chemical, glare, or dust.

3. Protective footwear shall be worn to protect falling objects, chemicals, or


stepping on hard objects.

4. Protective gloves or clothing shall be worn when required to protect against


hazard.

5. Harnesses and lanyards shall be utilized for fall protection required.

Safety Rules

1. All workers shall follow the safe practices and rules in the company’s
Health and Safety Manual and such other safety rules and practices
Required to safety execute any specific tasks.

2. All workers shall report all unsafe conditions or practices to the proper
Authority including but not limited to their immediate supervisor, and if
Corrective action is not taken immediately, a government authority with
Proper jurisdiction over such practices.

3. Keep your mind at work at all times, Horseplay, scuffing, and other acts
Which tend to have adverse influence on safety or wellbeing of other
Workers are prohibited.
4. Suitable clothing and footwear must be worn at all times. Personal safety
Equipment (hard hats, eye protection gloves and safety shoes) must be worn as
prescribed for each job.

5. Watch where you are walking. Don’t run.

6. Anyone under the influence of intoxicating liquor or drugs, including


Prescription drugs which might impair motor skills and judgment shall not
Be allowed on the job. Inform your supervisor if taking strong prescription
Drugs that warn against driving or using machinery.

7. No one shall be permitted to work while the worker’s ability or alertness


is so impaired by fatigue, illness or other causes that it might expose the
worker or others to injury.

8. Do not distract the attention of fellow workers. Do not engage in any act
Which would endanger another worker.

9. Sanitation facilities have been provided for your use. Defacing or


Damaging these facilities is forbidden.

10. Good housekeeping must be practiced at all times in the work area. Clean
up all waste and eliminate any dangers in work area.

11. Dispose of all trash, waste, and debris properly carefully. A good job is
a clean job, and a clean job is the start of a safe job. So keep your working
Area free from rubbish and debris.

12. Never moved an injured person unless it is absolutely necessary. Further


injury may result. Keep the injured as comfortable as possible. All
injuries should be reported immediately to the immediate supervisor
and to the safety in-charge so that arrangement can be made for medical
or first-aid treatment.

13. When lifting heavy objects, use the large muscles of the legs instead of
the smaller muscles of the back.

14. Nobody but the operator shall be allowed to ride on the equipment unless
proper setting is provided.

15. Do not use power tool and equipment until you have been properly
instructed in the safe work methods and become authorized to used them.

16. Workers should be alert to see that all equipment guards and other
protective devices are in proper places and adjusted. Do not removed,
displace, damage, or destroy any safety device or safeguard furnished
or provided for used on the job, nor interfere with the use thereof.

17. Before servicing, repair, or adjusting any powered tool or piece of


equipment, disconnect it, lock out the source of power, and tag it out.

18. Use only extension cords of the three-prong type. Use ground fault
circuit interrupter at all times and when using tools in wet atmosphere
( e.g. Outdoors) or with any temporary power supply. Check the
electrical grounding system daily.

19. Never enter an area that has been barricaded.

20. If you must work around earthmoving equipment including trucks


make sure the operator can always see you.

21. Never oil, lubricate, or fuel equipment while engine is running.

22. Barricade danger areas. Guard rails may be required.

23. Keep out of trenches or cuts that have not been properly shored or sloped.

24. The use of harnesses with safety lines when working from unprotected
heights is mandatory. Always keep your line as tight as possible.

25. Know where firefighting equipment is located and be trained on how to


use it.

26. Know what emergency procedures have been established for the job site
(location of emergency phone, first aid kit, fire extinguisher locations, etc.)

Job Safety Instruction

A. After the job site, the safety in-charge or other designated person will
identify and evaluate all potential hazards for:

1. Injury severity potential


2. Probability of an accident
B. The safety in-charge or safety designated person will also appraise the skill
and knowledge level of exposed workers.

C. Appropriate instruction will be given on the following:


1. Where hazards are pointed out.
2. Necessary precautionary measures will be explained.
3. the higher the hazards the more detailed will be the instruction.
D. Records will be maintained for all instruction sessions.
Safety Discipline

A. Four-step System

First violation: Oral warning; notation for worker’s file

Second violation: Written warning; copy for worker and worker’s file

Third violation: Written warning; one day suspension without pay

Fourth violation: Written warning; one week suspension, or termination


If warranted

B. A record will be maintained of all with the Personnel Office.

Safety Procedures

1. Housekeeping

The practice of proper housekeeping and cleanliness is highly enforced. Poor


housekeeping is a major factor in many accidents. A cluttered area is both unsafe and
inefficient. Every worker is responsible for keeping a clean area and ensures that all
areas remain clean and orderly.

Worksites must be clean and orderly. Work surfaces must be kept dry or
appropriate means taken to assure the surface are slip resistant. Spills must be cleaned
up immediately. All combustible scrap, debris, and waste must be stored safely and
removed promptly. Combustible dust must be cleaned up. The accumulated combustible
dust must be removed routinely.

Covered metal waste can are provided for oily and paint-soaked waste.

Make sure all pits or excavation and floor openings are either covered or
otherwise guarded.

2. Temporary Facilities Electrical Supply

All temporary electrical installation will be inspected and certified as safe by


an Electrical Engineer which will be based on the following requirements:

◘ Conduit system whether PVC or galvanized iron shall be used whichever


Is appropriate.
◘ Circuits are to be provided with rated MCB for protection.
◘ Circuit panel boards must have an earth leakage circuit breaker of
30mA tripping current.
◘ Lighting fixtures (Fluorescent, etc. ) must be installed with insulated
separator against wooden ceiling at the point of support in order to avoid
heat buildup on the surface of the wood to ballast operation.
◘ Electrical conduits in kitchen shall galvanized iron and all accessories
shall be metal clad. Installation shall be exposed and electrical accessories
shall be located a distance away from source of heat.
◘ Electrical outlet installed shall be based on actual requirements only.
◘ Unauthorized extension is prohibited which if in case shall be done by
competent person/s only.

3. Work Permits

Works permits must be secured all the time before starting any work so the
details of the applicable safety requirements as formulated for the project will be strictly
followed.

Safety Equipment

The use of warnings signs and signage, barricades, and caution tapes and other
safety equipment necessary to execute the work safety will be enforced. Dangerous
areas will be barricaded and the use of guard rails or perimeter cables if required will
be used.

Personal Protective Equipment

Appropriate personal protective equipment and clothing will be always worn


according to the activity, specifically:

◘ Where there is danger of flying particles, workers must wear protective


goggles and/or face shields.
◘ Workers are required to wear safety glasses at all times in work areas
where there is risk of eye injuries.
◘ Hard hats must be worn especially in areas subject to falling objects,
and at all times while in the project site.
◘ Appropriate footwear including steel toed shoes must be worn in areas
where there is any risk of foot injury from hot, falling objects, crushing
or penetrating action.

Accordingly to the activity, specifically:

◘ Where there is danger of flying particles, workers must wear protective


goggles and/or face shields.
◘ Workers are required to wear safety glasses at all times in work areas
where there is risk of eye injuries.
◘ Hard hats must be worn especially in areas subject to falling objects, and
at all times while in the project site.
◘ Appropriate footwear including steel toed shoes must be worn in areas
where there is any risk of foot injury from hot, falling objects, crushing
or penetrating action.
◘ In case where the noise level exceeds certain levels, ear protection is
required.

4. Excavation

Excavation must be barricaded to protect workers and vehicles.

Trenches over five feet deep must be shored or sloped as required. Excavated
or other material will not be stored nearer than two feet from the edge of the excavation
and a required one (1) meter strip on the both sides will be leveled and kept clear to
avoid objects from falling into the trench. Personnel will not be allowed to walk close
to the edges of excavation to prevent them from falling. Excavation less than five feet
may also require cave-in protection in some instances. Trenches will be inspected
every day before work commences and after heavy rains or any other occurrences
which could affect stability.

Men working in trenches or pits are cautioned and required to protect themselves
with the proper PPE against falling objects. Men working in trench will be cautioned
to stand and work at a safe distance to avoid being hit by other workmen’s tools. Men
working will be provided with ready access to and egress from the trench by secured
ladders or steps.

In case where the operation of a jackhammer is needed the operator is required


to wear the appropriate PPE and comply with poise protection regulation.

Equipment or vehicles will not be allowed to approach closer that what would
be the edge of the face it battered in excavation.

5. Hoarding
The worksite will be fully barricaded by protective hoarding so that the
General Public will be protected from working in progress. The hoarding should be
able to protect not only the public from dangers within the site but also act as a
barrier or security to prevent persons from trespassing into the site.
There will be an adequate safety distance between the hoarding and the
worksite.
Where the distance from a public to the building being constructed is
such that there is the likelihood of falling materials striking pedestrian or vehicular
traffic protection is necessary.

The entrance to & exit from the worksite will be located in such a manner
as to prevent danger and inconvenience to the public. The entry of unauthorized persons or
the public into the worksite will be prevented.
Suitable warning signs will be posted in conspicuous places inside the
hoarded area.
No bills except warnings signs will be allowed posted in the hoarding.
All construction materials will be stored and handled within the hoarded
area.
Vehicles will be parked in designated areas within the hoarded area while being loaded
or unloaded. Where it is authorized for vehicles ferrying materials to the
worksite to park outside the hoarded area, suitable safety measures will be undertaken
such as cordoning off such parking areas and suitable warning signs and flagman will
be provided.

6. Scaffoldings

Every scaffold and every part thereof will be of good construction, of suitable sound
material and of adequate strength for the purpose of which it is intended.
Scaffolds, bracing, and support shall be carried out in accordance with
Standard safety procedures by competent scaffolders.
No scaffolds will be erected or be substantially altered or be dismantled
Except under the direct supervision of a designated person.
All materials to be used for the construction of scaffold will be inspected
By a designated person on each occasion before being used.
Every scaffold will be properly maintained and every part thereof will
Be kept so fixed, secured or placed in position as to prevent, as far as practicable
accidental displacement.

7. Concreting
Formwork and shoring will be properly braced or tied together so s to
maintain position and shape.
The formwork including the shores, braces and other support will be
inspected by the designated person during erection, during and after concreting
work, and after any event likely to have affected their stability, for example after
heavy rain or following strong wind.
Workers involved in the mixing and the pouring of concrete are required
to wear proper PPE and specialized PPE for workers contact with fresh concrete.

A. Electric Welding and Gas Cutting

Only authorized and trained personnel are permitted to use welding


And cutting equipment.
Compressed gas cylinder will be regularly examined for obvious signs
Of defects, deep rusting, or leakage. Care will be used in handling and storing
cylinders, safety valves, relief valves, and the like to prevent damage. Precaution must
be taken to prevent mixture of air or oxygen with flammable gasses except at a burner
Or a standard torch. Only approved apparatus (torches, regulators, etc.) will be used.
Cylinders will be kept away from sources of heat. Empty cylinders
Will be appropriately marked and their valves closed.
Cylinder, cylinder valves, regulators, hoses and apparatus will be kept free of oily and
greasy substances. Care will be taken not to drop or strike cylinders.
The open circuit (No Load) voltage of arc welding and cutting machines will be set low
as possible and not in excess of the recommended limits. Grounding of the machines
frame and safety ground connections of portable machines will be checked periodically.
Electrodes will be removed in the holders when not in use. All electric power will be
shut off when no one is in attendance.
Suitable fire extinguishing equipment will be available for immediate use before
starting to ignite the welding torch. Welders will be strictly forbidden to coil or loop
welding electrode cable around their bodies.
Fire watchers will be assigned when welding or cutting is performed in locations where
serious fire might develop.
Personnel exposed to the hazard created by welding, cutting or brazing operations will
be protected with proper PPE and clothing.
Adequate ventilation will be checked where cutting or welding is performed.

B. Painting Works

Painting operation will be carried out in manner that will minimize dispersal of paint
mist and solvent vapor in public areas.
Adequate ventilation will be checked where painting works are conducted and painters
required wearing proper PPE.

8. Electrical Works

Electrical works will be performed by experienced and skilled working using proper
tools and equipment. The safety standards contained in the Philippine Electrical Code
will also be adopted in order to safeguard any person employed with the project.
Final inspection will be conducted and permit taken from the local utility company
before any service or power supply will be connected to the electrical system of the
project.

9. Driving

All workers within the project site will be advised to Drive Safely. If vehicle are used
during the work day, seat belts and shoulder harnesses are to be worn at all times. The
speed limit imposed within the project site will not be exceeded. Vehicles will be
parked in legal spaces and must not obstruct traffic. Defensive driving will be practiced
by all workers at all times.

10. Equipment

General for all equipment.


All operators of mobile equipment have obtained a license for that class of machine and
the certified or trained in the operation of that machine they are to operate.
Maintenance and check-up schedule will be strictly followed for each item of equipment
as establish and part of the equipment operational system of the company.
Machine operator will not carry passengers unless proper setting is provided with the
equipment.
Machines will not be left unattended while the engine is running. When machines are
unattended the engine are to be stopped and parking brakes applied or the wheels choked;
blades, buckets and the hydraulic attachments lowered to the ground before the operator
leaves the machine and the ignition key removed from the ignition switch.
If ever the operator of the machine cannot see the area all around the machine during the
operation, attendant is to be positioned to direct and assist the operator.
All moving machinery is to be guarded, especially gears, pulleys, vee-belt drives, fans
and revolving shafts common to most static equipment used in the construction site.

a. Portable Power Tools

Equipment will be checked before use. All grinders, saws, and similar equipment will be
equipped with appropriate safety guards. Power tools will not be use without the correct
shield, guard, or attachment, recommended by the manufacturer. All rotating or moving
parts of equipment will be guarded to prevent physical contact.
All cords connecting electrically-operated tools and equipment will be double insulated
or effectively grounded.
Proper PPE will be worn when operating these tools.

b. Small Concrete Mixers

The mixer will be operated by qualified personnel given instruction and training by a
competent person to operate such machinery. Effective guards will be in-placed over
belts, pulleys, chains or at appropriate places. The mixer will be properly set up on firm
level ground when in use and the wheels are properly choked to prevent movement while
in operation. A regular inspection and maintenance will be performed especially on the
drum and pulley gear in order to ensure that the machines is in good working condition at
all times.
Proper PPE will be worn when operating the machine.

c. Excavators/Pay loaders

Operator of these units will have proper training and be specially authorized to operate
the machine.
Drivers of dump truck being loaded by the machines will be cautioned to stand clear of
their units and the excavator/loader while loading is in progress. When working around
the machines, workers are cautioned to make sure that the operator can see notice them
by all means face the machine.
Operators will be advised not to moved loads over the heads of workers, truck cabs or
any vehicle.
Wheel loader when traveling will carry its bucket low so that it does not block the vision
of the operator and to maintain stability.
Proper PPE will be worn when operating the machine.

d. Dump Trucks (D Trucks)

Drivers will drive defensively, obey road signs and will never race other vehicles.
Dump trucks will not be driven at speed exceeding the authorized speed limit and loaded
beyond the authorized load limits.
During loading operation driver will not enter or leave the cab while loading is in
progress; only to moved off when signaled that loading is complete.
Where material is to be transported on a public road, local government regulations will be
followed.
Dump trucks carrying materials, and excavated materials, is ensured to be clean, well
maintained and in good running conditions. If carrying loose materials, they will be
covered and properly sealed to ensure that there will be no spillage of materials to public
road outside the project site. The wheels of the vehicle leaving the site will be free of
mud and ensure that mud will not be carried into other areas.
Backing is the most hazardous in truck operation. Reversing alarms will be installed and
fitted in trucks and drivers will only back off when under the direction of a signalman or
when satisfied that the way is clear and will remain clear.
During unloading, drivers will not raise the trucks tray unless the truck has fully stopped
and will not move the truck unless the tray is fully down.
Every measure will be taken to suppress any dust generated by the vehicle.
Care will be taken to protect overhead utilities, if any especially electrical cables.

11. Fire Prevention

Upon arrival at a new job-site, the safety or designated person shall evaluate the site and
make sure all personnel are aware of emergency phone numbers for notifying the local
fire department and other emergency agencies.
Portable fire extinguishers shall be accessible at all workers are instructed in the use of
extinguishers and the fire protection procedures adapted for the project site.
Construction areas will be maintained in a fire-safe and accessible to emergency response
forces. Proper construction fire safety practices will be utilized during all construction
activities. Excessive combustibles (paper) will not be stored in work areas.
All combustible scrap, debris and waste materials (oil rags, etc.) will be stored in covered
metals receptacles and removed from the worksite promptly. Proper storage to minimize
the risk of fire will be practiced. Only approved container and tanks are to be used for the
storage and handling of flammable and combustible liquids. All flammable liquids will
be kept in closed containers when not in use. Gas cylinders and oxygen cylinders will be
stored separated with a fire resistant barrier.

First Aid (Health Care)

First aid kits and required contents are maintained in a serviceable condition. A poster
will be fastened and maintained either on or in the cover of the first aid cabinet and
plainly stating the cell phone numbers of the first aider, the designated health and safety
person, the City’s Medical and Fire Response Teams.

Workers Welfare Facilities

Existing clean sanitary facilities including washing, bathing, and drinking facilities are
provided for the use of all persons employed for the works by the Client. Dining hall will
be provided with access to clean drinking water.

Disposal of Waste

Construction debris resulting from the construction operations (including food scraps and
the like) will be properly collected, segregated, recycled and disposed from time to time
as necessary. Suitable waste and garbage disposal methods and procedures will be
adapted as approved by the Client.

Environmental Control

The conduct of operations throughout the performance of the work will be observe in
such a way as to minimize the impact upon the natural environment by:

a. Providing dust control within work and all other areas including
dust control in common areas. Speed limit will be imposed;
the covering of construction materials during transport will be
required and imposed. Spraying of water during dry weather
will also be implemented to avoid dust emission.
b. Providing working machinery and equipment with efficient noise
suppression devices and every effort will be made to avoid unnecessary
noise emissions from the project site. Particular care will be undertaken
that ineffective exhaust mufflers are placed immediately.
c. Providing equipment that is in good state of repair before assigning to
the project.
d. Material storage areas will be kept in a neat and tidy manner to mini-
mize hazards to persons and the environment.
e. Precautionary measures will be undertaken to prevent soil erosion
from any lands during the execution of the works.

Emergency Procedures

In the event of an accident the health and safety designated personnel and the first aider
will take immediate control of the incident. The injured person shall not be moved unless
the injured is in further danger. If electrical power is involved the power injured should
not be touched. The power supply will be switch off or arrange or arrange to have power
switch off before touching the injured person.
Emergency medical assistance will be immediately ask for so the same can be provided
right away by the Client or by the nearest outside medical facility. Emergency cell phone
numbers of Medical/Fire Department, the health safety designated personnel and of the
First Aider will be posted in the field office especially in the cover of the first aid cabinet
and above the fire extinguisher.
In case of fire, attempt will be made to extinguish the fire using available fire fighting
equipment and raise the alarm by calling the City’s Fire Department.

Policy (CHSP)

Construction safety and health Reports

All general construction shall be required to submit a monthly constructing safety and
health report to the BWC or to the DOLE Regional Office concerned. The report shall
include a monthly summary of all safety and health committee agreements, a summary of
all accident investigation/reports and periodic hazard.

◘ In case of any dangerous occurrence of major accident resulting in death or


permanent total disability, and concerned employer shall initially notify the DOLE
Regional Office within twenty four (24) hour for occurrence. After the conduct of
investigation by the concerned construction safety and health officer, the employer shall
report all permanent total disabilities to DOLE Regional Office on or before the 20th of
the month following the date of occurrence of accident using the DOLE/BWC/HSD-IP-6
form.
◘ “Work environment” - refers to one combination of all physical, biological, chemical,
medical, physiological, psychological, social, technological, and technical elements that
directly affect the workplace.
◘ “Physical factor or hazards” – refers to temperature, humidity noise, ionizing
radiation, abnormal pressure (due to attitude ), ventilation, illumination and the like
which may place added stress on the body.
◘ “Chemical substance” – means any organic or inorganic substance of a particular
identity, any element or uncombined chemical and any combination of such substances;
or any mixture or two or more chemical substances.
◘ “Hazardous materials or substances” – refer to substances in solid, liquid or gaseous
forms know to constitute toxic, flammable, explosive, corrosive, oxidizing and
radioactive properties.
◘ “Establishment” – refers to any single proprietorship, corporation or similar entity
operating an economic activity and engaging the services or workers.
◘ “Hazardous establishments” – in one where the employee is exposed to potential risk
of disabling injury, illness or death, either because of the presence of biological agents,
radiation, chemical substances, hazardous materials or physical hazards in the work
processes performed or equipment used therein.
◘ “Hazardous work processes” – refer to work operations or practices performed by a
worker in the establishment or workplace with or as an incident to such operations or
practices and which expose the worker to hazards likely to cause any disabling injury,
illness, death or physical or physiology harm.
◘ “Imminent danger” – means a condition or practice that could reasonably be expected
to cause death physical harm before abatement under the normal enforcement procedures
can be accomplished.

The plans to achieve worker safety and health program through the following:
◘ Using a qualified safety person

◘ Making regular jobsite safety inspections

◘ Enforcing the use of safety equipment

◘ Following safety procedures and rules

◘ Providing on-going safety training, if necessary
