1. Introduction
This Health, Safety and Environment Plan (hereinafter referred as HSE Plan) is established in
order to outline the Health, Safety and Environmental management measures that shall be
followed during the execution of “Turnaround Maintenance Services at QatarEnergy Chemical
Plant”. The HSE management system and safe working practices mentioned in this plan shall
be in line with QatarEnergy requirements, HSE regulations and applicable laws in State of
Qatar.
In addition to the HSE measures elaborated in this plan, Galfar is committed to protection of
health of personnel, safety of existing assets and as well as the mobilization resources,
vehicles, equipment and machinery and the existing natural environmental factor for the utmost
care and attention in all activities.
This HSE Plan is an active document. Formal updating will be done in the event of new
procedures or change in the scope of work or changing environment/conditions. Action points
found irrelevant may be deleted and new ones added as and when the need arise.
Amendment to the HSE Plan shall be authorized by the Project Manager and approved by the
QatarE. Revised copies of the HSE Plan will then be documented and issued to registered
recipients. Previous issues will be marked as “Superseded” and removed from point of use.
Controlled copies of HSE plan shall be issued to all key personnel.
Turnaround maintenance of the QatarEnergy Chemical Plant (Linear Alkyne Benzene) Units
70, 71, 72, 73, 74, 75,76, 77 and Utilities & Offsite 21, 22, 29, 50, 52 which is located at
Mesaieed Industrial Area.
The intention is to undertake shutdown of Chemical Plant for complete Turnaround for
thorough internal inspection of critical equipment, overhaul/test associated mechanical,
instrument, electrical equipment and for carrying out other miscellaneous works and put the
Plant back into operation. The brief scope of turnaround maintenance services consists of
decontamination of the Plant, internal inspection of static equipment, overhauling of valves, civil
works, piping works, purging of the Plant with nitrogen and assisting QP to put back the Plant
into operation after completing all the activities
Audit

Unsafe Condition
A condition or a set of conditions or circumstances in the workplace that have the potential to
cause injury or ill health
Consequence is the severity of injury, illness, damages to assets or harm to the environment
resulted from the event.
Control measures are those proposed to reduce the gravity of consequences and regain
control, if an undesired event does happen.
Task is any work conducted on the basis of a fixed procedure.

2. HSE Legislation
Galfar shall comply with applicable legal and other requirements such as Qatari Laws and
Decrees, Labor Law, Codes and QatarEnergy Standards throughout the entire duration of the
project scope of work.
Applicable legal and other requirements specific to the project shall be identified and a “Legal &
other requirements register” will be maintained at project site and the evaluation of compliance
to these requirements will be done half yearly basis. The requirements shall be communicated
to relevant project personnel for compliance.
The QatarEnergy operates as Business Management System (BMS) covering ISO 9001:2015
for Quality Management System, ISO 14001:2015 for Environmental Management System,
ISO 45001:2018 Occupational Health and Safety Management System. Galfar’s Management
System has been developed by combining essential elements from:
• Management Leadership and Employee involvement in HSE matters
• Allocation of competent HSE Personnel and other required resources
• Galfar Business Management Systems and its procedures
• Worksite Hazard Analysis of all activities
• Hazard Prevention & Control procedures
• HSE Training to all Employees
• Complying with all Legal and other Safety Requirements, Procedures & Standards

The project level HSE objectives shall be further elaborated/revised in accordance with the
prioritization of risks. These objectives shall be monitored and reviewed periodically.
Galfar’s HSE Objectives are:
• Completion of the shutdown without INCIDENT
• Zero tolerance on non-compliance of PTW system throughout the shutdown
• 100% HSE training compliance rate with shutdown specific HSE training schedule
• Reduction in Significant Environmental Impacts
Managing HSE is a line responsibility. Galfar is committed to HSE in all its businesses and this
commitment is realized throughout the organization. Job assignment, responsibility and
authority are delegated in such a way that all employees are responsible for human safety and
health, conserve the environment and ensuring the security of its assets. The Senior
Management shall be ultimately accountable for the activities and results of the operations.
Galfar is a multi-disciplined Engineering Construction QatarEnergy with a proven record of
safety.
Galfar has an Alcohol and Drug abuse policy in place and does not allow any of its
workers to violate the same. The following points shall be treated as a serious violation.
• The use, possession, distribution, offering, or sale of illegal drugs, drug paraphernalia, or non-
prescribed drugs while on Galfar premises.
• Having in the body a presence of illegal drugs, or non-prescribed drugs or their metabolites while
on Owner property of business.
• Being unfit for work due to the aftereffects of alcohol, illegal drugs non-prescribed drugs or the
intentional misuse of medication.
• Breach of these rules will normally result in summary dismissal, and only in exceptional cases will
be given the reduced disciplinary action of a final written warning to be applied

requirement for the effective implementation and monitoring of arrangements for health and
safety, and environmental protection.
• Implement, monitor and maintain arrangements for effectively managing HSE in the
shutdown.
• Identify the medical/ health requirements for the personnel involved in shutdown, ensure
workforce are fit to work. Refer FTW certificates from MOPH approved Clinic
• Make adequate arrangements for First Aid, Fire Watch and Emergency Rescue.
• Ensure all risks are captured in the risk register.
• Effectively fatigue management as per this plan.
• Take part in the development of JHA and review the same when required
• Liaise with QatarEnergy on HSE issues at site
• Take part in Progress review meetings and render the HSE related information to the
QatarEnergy as needed.
• Ensure all activities are undertaken as per the procedures and/or method statements
• Ensure Permit to work system followed as per QatarEnergy requirements.
• Ensure adequate PPEs and RPEs are being used as required. Toxic clips, escape mask are
available with all employees.
• Render necessary advice on HSE to site supervisors during site visits and follow up.
• Ensure plant and equipment inspections are as per QatarEnergy requirements
• Reporting HSE related incidents to Galfar QHSE and QatarEnergy. Conduct investigations
and review of all incidents and provide support to the concerned departments to implement
corrective actions.
• Compiling and maintaining HSE statistics pertaining to the shutdown and rendering report to
the QatarEnergy as per requirement.
• Submit Monthly HSE reports to QatarEnergy and Galfar through Project Manager
• Ensure adequate resources in terms of competent Safety Officers & Inspectors, Hygiene
Supervisor, First Aiders, Fire Watch, Confined Space Entry Hole Watch, and measuring
instruments are available throughout the duration of shutdown.
• Reporting functionally to Head (QHSE) & administratively to Project Manager
Galfar shall ensure that appropriate communication method is established by organizing HSE
briefings and meetings, induction/ orientation, newsletter, posters, e-mails, suggestion
boxes/schemes, and notice boards to communicate the following HSE related subjects to
employees and subcontractors, e.g.:
• HSE Policies, Procedures
• Workplace HSE rules and regulations
• Workplace hazards and alarms, siren, PAGA

Galfar ensure that the appropriate communication techniques shall be used to make the
personnel aware of HSE issues. Techniques like notice boards, posters, leaflets, Corporate
HSE newsletter and HSE alerts shall be used to make the employees aware. A monthly safety
campaign shall be conducted to create or reinvigorate specific HSE actions.
Other HSE Promotion program reflected in Section 8.1 Rewards and Recognition
Galfar shall coordinate, arrange, and attend HSE meetings throughout the duration of the
shutdown. Minutes of meeting shall be kept on a regular basis.
HSE meeting shall be held regularly, subjects for reporting and discussions may include:
The intention on Management Review in Galfar is for senior management to review the
continuing suitability and effectiveness of the management system in meeting set objectives
and demonstrate commitment to continual improvement. Management Review is the
opportunity to review and reflect on recent performance to identify improvements to the
management system. Inputs from the project is collected for Galfar organizational Management
Review conducted at least annually. Details from Section 10 of this Plan included in such
reviews, for agenda refer Galfar BMM-01 Rev 01.
In addition, the project specific reviews shall be conducted by the Project Manager, HSE
Manager involving the key members of the project team to evaluate the HSE performance

results and the actions for continual improvement. These reviews shall be conducted every six
months and shall not exceed 12 months interval.
Identification of work-related Hazards and their management is the heart and soul of HSE
Management in the successful safe execution of works. It forms the core of the HSE
management system. The main objectives of hazard management process are:
Risk assessment for all activities shall be prepared and submitted to QatarEnergy for approval
before commencement of Turnaround.
Administrative control such as job rotation, limiting exposure, permit system, safe operating
procedure & training etc. are also adopted to control risks. Cross Functional Team (CFT) of
Construction Manager/Site Engineer, HSE team will be responsible for this process. Worker
representatives shall be involved in the process of risk assessment. The HSE Risk Register is
a live document and updated periodically.
Work activity specific Job Hazard Analysis shall be established based on the scope of services.
Approved JHA shall be communicated as part of toolbox talk (TBT) with the work team prior
starting the job. JHA will be attached to the corresponding work permit.
Galfar shall comply with QatarEnergy requirements to deal with management of change (MOC)
and the approval of QatarEnergy shall be obtained for the proposed MOC. The changes could
be process, technology, organizational change, or changes to method of work or scope of
woks. The impact of changes should be reviewed with respect to HSE in coordination with
QatarEnergy. The HSE and environmental risk register should be reviewed, if required
updated. The changes should be reflected in relevant method statement/procedure, and the
risk and control measures to be included in JHA for implementation and monitoring.
Galfar shall develop Walk through and Inspection program for QatarEnergy approval prior to
implementation on site. This will provide opportunity to conduct structured walkthroughs
throughout the duration of the shutdown. Management walkthroughs to demonstrate
management commitment and leadership shall be performed by representations from the
QatarEnergy, Galfar. Non-Management walkthroughs shall also be conducted to identify and
abate hazards. These walkthroughs shall be conducted on daily basis.
Galfar shall follow QatarEnergy PTW system during the contract execution. Employees to be
involved in activities that required PTW shall be trained in QatarEnergy PTW system prior to
work commencement.
Galfar HSE personnel shall monitor the workplace temperature during summer and implement
controls as necessary. HSE site assessments focusing compliance with JHA, heat stress
measures, welfare requirements etc. shall also be performed at frequent intervals.
Daily and periodical HSE inspections by HSE personnel will be carried out to ensure the site
HSE compliance as per the QatarEnergy and the Galfar HSE requirements. The identified

deviations will be rectified with suitable corrective actions to ALARP level within the proposed
timeframe. Inspection checklist shall be generated and maintained for records.
Galfar HSE personnel shall join QatarEnergy representative for joint safety inspection of the
shutdown as per QatarEnergy requirement.
QatarEnergy colour coding system shall be complied for lifting appliance, tools, and tackles.
6. HSE Training
Galfar ensure that HSE related training shall be given to all level of employees to enhance their
awareness, skills and competence level on Health and Safety requirements. The training shall
be given by either a QatarEnergy approved third party or by site HSE Personnel (in-house
training) as per the QatarEnergy requirements to all level of employees engaged in the
shutdown.
Safety induction conducted by QatarEnergy shall be attended by all employees engaged the
shutdown.
Galfar shall ensure that any incident ie., accident or near miss to personnel or involving the
QatarEnergy equipment shall be reported immediately. A detailed report on the incident
communicating the event causes, remedial actions shall be submitted to QatarEnergy within 24
hours from the event.

Galfar ensures that all HSE incidents will be reported to QatarEnergy as per requirements. All
incidents reported with an immediate notification to QatarEnergy, followed by an INR and Final
Investigation Report. Incident investigators shall be trained and experienced to apply
investigation techniques to determine the root causes.
If anyone is injured, First Aider shall respond immediately and IP shall be shifted to Clinic for
evaluation and management. The extent of injury may be assessed by site medical person/
male nurse / doctor / first aider as applicable etc.
As per Galfar BMP-24, Site supervisor/engineer will generate First Information Report (FIR)
and inform site HSE personnel, Engineers and Project Manager. FIR will be forwarded to
Corporate HSE Manager for review and will also be forwarded to plant department if it is
vehicle related incident. Detailed Investigation Report will be prepared by Cross functional
team for specific FIR (Injury cases and Near miss / High potential cases) and will be sent to
corporate HSE manager for review & recommendations. The outcome will be implemented by
site supervisor/engineer and monitored by HSE personnel at site. Time frame for incident
reporting and investigation provided below for reference.

8. Safe Work Practices, Procedures and Instructions
Incentive program is an effective tool to enhance worker’s HSE consciousness and to increase
their interest and awareness in health, safety and environmental matters which also helps to
eliminate accidents.
The following programs for HSE promotion shall be implemented at the project site.
HSE Incentive Program
Proactive compliance and positive attitude by all our employees towards HSE requirements are
the key element for us to reach and sustain the HSE goals of our organization i.e., Zero Harm.
In order to keep our staff and workers motivated towards HSE compliance, the following HSE
incentive program will be implemented by Galfar in promoting proactive compliance and
positive culture towards HSE.

• All recommendations will be accompanied with the evaluation sheet of each category
duly signed by the concerned personnel, forwarded to HSE representative to process
the incentive and release the certificates.
• Workers nomination would be 1 person per every 100 workers.
• Foremen category nomination would be 1 per every Quarter.
• Above foremen category nomination would be 1 per every Quarter.
• This scheme is only for the workers/staff directly working in the site not for the office
staff.
• If an office staff is nominated for this award, his contribution to improve HSE should be
clearly explained and approved.
• HSE incentive award comprises recognition certificate and cash reward which will be
deposited in individual’s salary account
The best safety conscious employee shall be nominated by Safety Lead, approved by
Construction Manager on weekly basis and appreciated with Gift Voucher / Hala Card.
These appreciation awards will be distributed in different categories and will based on site HSE
observations, Management walk downs & daily site HSE Inspections.
Galfar HSE personnel shall review the observations, the best observations shall be award with
gift voucher to motivate employees on reporting any unsafe act/condition noticed.

8.2 Housekeeping
All plant and installation areas shall be maintained as per the highest possible stands of
housekeeping. Regular housekeeping shall be practiced at workplace to prevent slip, trip and
falls, fire and other undesired events. Floor and edge openings shall be barricaded to prevent
fall of personnel or materials. Clear access shall be maintained to emergency exits, firefighting
equipment, alarm points, entrance and exit. Work area shall be cleaned after finishing a job
and at the end of each shift. Materials will be stored in orderly manner in designated place, and
the heavy objects placed near floor level. Stacking shall be limited to personnel reach and
supported adequately.
To improve the HSE performance Galfar shall encourage a no blame culture for a positive and
effective feedback from the involved direct and indirect personnel.
This provides employees opportunity to intervene and notify the supervisor when a perceived
unsafe condition or behavior may result in an unwanted event. If it is not safe, then the work
can be stopped and resumed after rectification or application of appropriate control measures.
Galfar shall follow below mentioned measures to execute electrical operations safely.
• Approved powered tools and equipment driving on electric power shall only be utilized in
the project, where applicable reduced voltage requirements shall be fulfilled.
• Consideration shall be given to the hazardous area classification when electrically powered
equipment usage is planned
• Authorized persons only to carry electrical installations and repair works at site premises
• Permit to work procedure shall be followed prior to start work
• Lighting, electrical distribution boards, electrical tools, fans, air-conditioning units and
electrical cables shall be inspected and color coded by competent person on monthly basis
• All electrical connections and equipment shall be provided with round fault circuit interrupter
• All electrical equipment and connections shall be grounded properly
• Power distribution panels shall be inspected bi-weekly or monthly as per requirement
• Distribution panels shall be provided with rain protection covers, closing doors and rubber
mats
• Panel board location shall be illuminated and maintained in an easily accessible condition.

• Any kind of material (Including flammable and combustible) stacking nearer to electrical
installation shall be prohibited
• All cables, wires and cords shall be placed over the height of people or laid in ground level
with cable protectors to prevent trip hazards
• Cables and wires shall be free from splices and damaged insulations
• Electrical equipment should not be used at wet or waterlogged areas without any
precautionary measures
• Heavy duty (Industrial) plug tops only shall be used at site premises
• Defective electrical equipment or cable and cords shall be eliminated from the work activity
• Overload or over trapping shall be prohibited
• Suitable rating of electrical equipment shall be used, and unauthorized replacements
prohibited
• Appropriate hazard sign boards shall be posted near to the electrical installations to alert
the work personnel.
• In addition to the above measures, we will comply and follow QatarEnergy guidelines on
temporary site electricity
• Tipper Trucks and Buses used on the Project will be no more than five (5) years old
• Vehicles fitted with audible reversing alarm

In addition, driver performance shall be monitored as part of safe driving program. All drivers
shall be trained in defensive driving program, speed limit implementation across the project.
Galfar ensure that all employees are medically fit prior to deployment on site. Examination
records shall be submitted to QatarEnergy. Galfar ensures that the periodic (annual) medical
examinations shall be done for Welders, Painters, Drivers, Food handlers and Work personnel
who continuously exposed to noise at workplace through medical commission approved
medical centers.

Galfar ensures that internationally recognized personal protective equipment will be provided to
employees to protect them from occupation health and safety hazards. Also, the PPEs will
comply with workplace standards, QatarEnergy Personal Protective Equipment (PPE)
requiremetns. Employees will be monitored for proper utilization of Personal protective
equipment while working on site. All Supervisors will be responsible for ensuring that suitable
PPEs are available, in serviceable condition and that it is used at the required times.
Project employees shall receive training in the proper use, care, and maintenance at time of
hire or use of a new type of PPE. Site Management will enforce the proper use of PPE in the
project.
Personnel not in possession of the appropriate work wear (mentioned in below list) will not be
allowed to be on site. Followings are mandatory to wear at worksite:
• Safety Helmet
• Safety spectacles / Goggles
• Hand Gloves (As per work requirement)
• Respirators/Masks (As per work and location requirement)
• Safety Shoes
• Full-body harness (While working at height)
• H2S Monitor, EEBD (Wherever required);
• Cover-all
• Dosimeter (Wherever required)
Besides that, Job Specific PPE to be worn as and when required, e.g. Ear plug, Earmuff in high
noise area. Heavy duty leather gloves while handling sharp edge object. All PPE shall be
accordance with the regulatory standards and QatarEnergy requirements. All personal
protective equipment shall be inspected frequently to ensure the fitness for use. Defective
PPEs shall be eliminated and replaced with new PPE.
Galfar shall consider and implement the following measures during manual handling:
• Wherever possible, manual handling shall be avoided by using mechanical means (i.e.
Crane, forklift, trolley etc.). In accordance with work nature, proper lifting aids can be
utilized.
• Where manual handling cannot be avoided the risk of injury shall be reduced as far as
possible by undertaking an assessment and identifying suitable controls
• Workers shall be trained in proper lifting techniques, Work postures, Repetitive and
Awkward moments, Hazards, and risks. Toolbox talks to be organized.
• Work-personnel shall never be allowed to handle or lift more than 25Kg as per local and
international standards
• Team lifting shall be preferred rather than individual handling and weight, size and shape of
the load shall be assessed prior to assign of handling
• SDS shall be briefed to workforce prior to start handling hazardous substance
• Workplace shall be provided with adequate illumination and ventilation to reduce fatigue
and stress

• Housekeeping to be maintained at workplace to prevent slip, trip and falls. Floor and edge
openings shall be barricaded to prevent fall of men or material
• Safe distance to be maintained from the vehicle or equipment while loading and unloading
• Personnel shall wear safety gloves to prevent injuries from cuts, sharp edges, contact with
hot and cold surfaces etc. Safety boots shall be worn to protect the feet.
• Medical surveillance to be carried out for workers who continuously engaged in material
handling process
Galfar shall ensure that all worksite supervisory personnel are trained in relevant PTW system.
All works shall be planned appropriately JHA prepared, relevant type of permits applied enable
performance of the intended works safely.
Works that is not causing heat, sparks or flame that can possible ignite/flammable gases or
vapors or other flammable matters. Cold Work Permit – white in colour. Example:
• Bolting and unbolting of piping and vessel flanges
• Removal of instrumentation or electrical equipment, which have been de-energized or
deactivated.
• Removal of pumps, compressors and similar equipment
Performance of any work where heat, sparks or flame are used or generated that it has the
possibility to ignite flammable, vapor or gases or other flammable matters. Hot Work Permit -
red in colour. Example:
• Gas burning, welding and brazing
• All naked flames and fires
• Electrical arc welding, leas burning and soldering
• Use of resistance electric heaters or coils in hazardous areas
• Use of electrical or battery operated equipment which are not compatible with the
classification of hazardous areas. i.e. not explosion proof or intrinsically safe
e.g. torches, radios, cameras, ultrasonic thickness meter, ultrasonic flow detection,
ultrasonic leak detector, Radioactive level detector, multimeter
• Use of electrical drills
• Grit blasting
• Caulking, chipping and grinding in hazardous areas
• Drilling in steel structure
• Power driven cutting tools in hazardous areas
• Use of non-explosion proof electrical tools (extension cords, plugs) in hazardous areas
A confined space entry permit is required before personnel are allowed to enter or partially
enter any confined space, which might contain hydrocarbons, any health harmful materials
or be deficient in oxygen. Confined Space Entry Permit - green in color.

If a hot work or a cold work is required inside a confined space, the relevant permit shall be
issued for the activity to be performed and be associated to the entry permit. Example: reactor,
vessel, drum, tower, tank, furnace, boiler, valve box, sump, sewage pit, column, any excavation
work over 1.2 meter in depth etc.
Excavation Permit
An excavation permit is required for earth moving work to protect extended electrical cables,
pipelines and/ or any other facility at the excavation site. In the case of non-availability of
drawings indicating the routing of the electrical cables, electrical isolation shall be considered
and the individuals executing the work shall wear the proper personal safety equipment to
avoid electrical shock. Excavation Permit - yellow in color. Examples:
• Excavation for laying down or laying bare cables, pipes
• Removal of foundations or buildings
• Removal of earthy or concrete floors
• Building of sewer pits, valve pits, etc.
• Drilling in walls or ceiling/roof
• Opening in walls or ceiling/roofs.
Boxing Up Permit
Boxing Up Permit shall be issued prior to closing / boxing up confined spaces, and both parties
shall ensure that work of all parties on its interiors have been completed and that it is clear from
any foreign bodies or materials. On issuing this permit all other permits related to the same
confined space shall be considered as canceled and no one shall be permitted to enter this
confined space. Boxing up permit- dark yellow in color.
Vehicle entry/portable heavy equipment permit is required for the entry and use of motor
vehicles and other internal combustion engine equipment (only diesel driven) within process
units and associated roads, hazardous areas, etc. which can pose unintended ignition hazards
or hazardous conditions.
The permit shall not allow work activities if it is used as an associated permit to operate
motorized equipment such as radiography, welding, loading and unloading of chemicals in
column etc.
The permit shall allow work activities only if no other permit is associated ie, forklifts operating,
loading and unloading with crane mounted trucks, removal of dismantled trucks, removal of
dismantled pumps or equipment, sewer waste removal vehicle, road cleaning vehicle.
Examples: Mobile cranes, car, vans, forklifts, earth moving equipment, heavy tractors industrial
truck, heavy mobile equipment or truck entering hazardous area in the operational unit etc.
The work shall be stopped when control measures furnished in the permit are not fulfilled or
followed. Job safety analysis shall be developed and provided along with PTW for proposed
work activity. Briefings shall be given to workers regarding the specific job or activity prior to the
start of work activity. Permit board shall be installed in prominent location of site premises.
Permit board shall be installed in visible location of site premises.

• Prior to start any hot work, work permit shall be obtained from QatarEnergy.
• Gas test shall be done by Operating Authority(Plant) for work in confined space and
continuous monitoring by designated Fire Watch.
• Trained and competent Fire Watch shall be engaged and shall not be assigned to other
duties. Continue monitoring open flame area at least 30 minutes after the work is
completed to allow hot surfaces to cool and for the work area to be deemed safe
• Erect appropriate physical barriers to confine the ignition sources (open flame, sparks,
and heat) and eliminate flammable gas ingress
• Ensure emergency preparedness (Fire Watch are in place, appropriate
firefighting equipment and first aid kits are available, emergency contact details are in
place, means of communication are available, etc.)
• Conduct a toolbox talk and task briefing
• Ventilation plan shall be complied for works in confined spaces.
Galfar shall follow below mentioned conditions during the execution of Arc welding
• Welding cables may be subjected to severe abuse if they are dragged over work under
construction and across sharp corners or run over by vehicles. Replace or repair defective
cable immediately.
• Arc welding may require precautions be taken to provide proper respiratory protection,
positive ventilation, local exhaust removal, or approved respiratory equipment.
• In confined areas, cover or arrange cables to prevent contact with falling sparks.
• Earth or ground the frames of portable or stationary welding units.
• Keep welding cables dry and free of grease and oil to prevent breakdown of insulation.
• Welding cables shall be placed at safe distance from electrical power supply and high-
tension cables
• Welding cables shall be placed at overhead distances if cable runs a considerable distance
from welding machine
• Protect cables that shall be laid on the floor or ground so they will not interfere with safe
passage or become damaged or entangled.
• Flammable or combustible material shall be removed or stacked away from the hot work
carrying workplace.
• The area shall be barricaded and signed to alert people if the hot work took place at height
• A fire watch and a fire extinguisher shall be maintained throughout the welding operation
and continued for at least 30 minutes after job completion
• Mandatory Personal protective equipment shall be given to workforce.
• Use artificial ventilation systems in confined areas, such as tanks and pipeline areas. This
precaution shall maintain toxic gases, fumes, or dusts below the maximum allowable
concentrations.
Galfar shall follow below mentioned conditions during the execution of Gas welding
• Permit to work procedure shall be followed, prior to start gas welding or cutting process
• Gas test shall be frequently done as the workplace have tendency to contain flammable
atmosphere
• Trolleys shall be used to shift the gas cylinders from one place to another place

• Welding and cutting gases commonly used include oxygen, acetylene, and argon.
Reference shall be made to the relevant material safety data sheets.
• Compressed gas cylinders shall be lifted by means of cradles secured with guard-rails.
• Load cylinders to be transported to allow as little movement as possible. Secure them to
prevent violent contact or upsetting.
• Cylinders shall be stored in a safe, dry, well ventilated place prepared and reserved for that
purpose.
• Cylinders shall not be stored near pathways, stairways, or other places where they can be
knocked down or damaged
• Cylinders of oxygen stored indoors shall not be within 6 meters of cylinders containing
flammable gases or highly combustible materials.
• Acetylene and liquefied fuel gas cylinders shall be stored with the valve end up.
• Defective pressure gauges shall not be permitted on regulators.
• Flash-back arrestors shall be installed at both ends of hose (Torch end and cylinder end) to
prevent back-flow of fire or sparks.
• Before changing torches, shut off the gas at the pressure reducing regulators and not by
crimping the hose.
• The torch shall not be left down until the gases have been completely shut off.
• Do not hang torches from a regulator or other equipment which allows them to come in
contact with the sides of gas cylinders.
• When extinguishing the flame, close the acetylene and oxygen valves in the order
recommended by the torch manufacturer.
• Compressed gas cylinders shall not be kept at inside confined spaces
• When gas welding is completed it is essential that the shut off valve on the cylinder shall be
closed
Galfar shall take adequate precaution measures to prevent fire during and after the completion
of any kind of hot works. Following measures to be consider since the workplace having
hydrocarbon vapor
• Flammable or combustible material at workplace shall be removed.
• Gas test is to be conducted to determine the explosive limits at workplace
• Determine whether a hazardous atmosphere exists.
• Provide fire barriers such as metal sheets or fire blankets and fill cracks or crevices in floors
to prevent sparks and slag from passing through (Consultation shall be made with
QatarEnergy)
• Fire extinguishers shall be provided at workplace. Location of FE and method of using shall
be notified to all working personnel
• Adequate number of fire watchers shall be deployed in accordance with the work
• All gas utilizing equipment shall be provided with flash back arrestors.
• Drums, tanks, and closed containers that have held flammable or combustible materials
shall be thoroughly cleaned before welding or cutting. As an added precaution, purge with
an inert gas such as nitrogen or carbon dioxide.

As per project requirement Galfar shall carryout pneumatic and hydro test on spools, valves,
piping, pipelines as and when required. Method statement shall state the type of testing,
equipment to be used, exclusion zones including calculation methods, disposal means for
water during hydro testing, competency of personnel and barricading requirements, whip
checks for pressurized hoses, test pressure and holding period.
• Personnel required using hand and power tools shall be competent in the selection and use
to particular tools and aware of risks and control measures
• Know the correct use of hand and power tools. Use the right tool for the job
• Do not use tools with split, broken, or loose handles, or burred or mushroomed heads.
• Pre-use checkups should be made and damaged tools to be removed from service
• Hand tools should not be thrown from one place to another, use tools boxes/ containers
• Keep all tools away from the edges of scaffolding, platforms, shaft openings, etc.
• Proper guards or shields shall be installed on all power tools and the same never be
dislodged or modified. Under any circumstances improvised/ “homemade” tools or
extensions shall not be used
• All electrical power tools (unless double insulated), extension cords, and equipment shall
be properly grounded
• All electrical equipment shall be connected via GFCI
• When working in flammable or potentially explosive atmospheres specially protected
lighting and intrinsically safe equipment shall only be used
• Periodic inspection and tagging of shall be performed on all power tools
• Employees shall be checked for fitness prior to deployment into height works
• All personnel shall wear full body harness while working above 1.8 meters from ground
level.
• Scaffoldings / ladder shall be placed in an even, compacted ground
• Scaffoldings shall never be constructed proximity to HV electrical installations / cables

Scaffolding
• Scaffoldings shall be erected by certified scaffolders and it shall be inspected and signed
by concern supervisor and SCAFF-TAG system to be followed
• Working Planks shall be provided with required thickness while working at scaffold and they
shall be sufficiently supported by transoms
• All scaffolds and ladders shall be following the 4:1 height and with ratio. Independent
scaffolds shall follow 3:1 ratio
• All scaffolding erection and Dismantling areas shall be provided with barriers and signage.
Scaffoldings at third party or public areas shall be provided with screening and fall
protection system
• All materials used for the scaffold construction shall be inspected and ensured that in good
condition. Defectives should be eliminated
• Scaffoldings shall be inspected by competent persons and scaff-tag system shall be
followed. The scaff-tag shall contain following details
- Location of the scaffold inspected
- Name of the person making Inspection
- Date that the inspection was completed
- Intended and capacity of scaffold
- Required re-inspection frequency for scaffolding
- Working shall be prohibited while Red Scaff-tag placed on scaffolding
• Authorized persons only dismantle or alter the scaffolding arrangement at site premises.
Users prohibited to dismantle the scaffold
• The scaffolding shall be relieved from usage prior to start dismantling activity. The red Scaff
tags shall be placed prior to start dismantling activity
Ladders
• Place a ladder so that the horizontal distance from the base to the vertical plane of the
support is approximately one-fourth the ladder lengths between supports.
• Do not use ladder in a horizontal position as runways or as scaffolds. Single and extension
ladders are designed for use in a nearly vertical position and not be used in horizontal
position.
• Never place a ladder in front of a door that opens towards the ladder unless the door is
locked, blocked, or guarded.
• Place the ladder feet on a substantial, firm and level base, and not on any other objects,
like barrels, wooden boxes etc.
• When using a ladder for access to high places securely ties or otherwise fastens the ladder
to prevent it is slipping.
• Ladder rungs and side rails shall be in good condition. The rung spacing shall not be more
than 12”

• Ladders shall not be hung from brackets as it tends to pull out the rungs
• Secure both bottom and top to prevent displacement when using a ladder for access to a
scaffold
• Extend the ladder side rails at least 3 feet above the top landing.
• Do not use a metal ladder close to live electric wiring or any operational piping like acid,
gas, etc. which could be damaged.
• While ascending or descending, the user shall face the ladder, use both hands, and place
his feet near the ends of the rungs rather than the middle.
• Extension ladders shall be sufficiently propped.
- The loss of consciousness of any person at work arising from an increase in body
temperature
- The loss of consciousness or asphyxiation of any person at work arising from gas, fume,
vapor, or the lack of oxygen
- Serious injury to any person at work arising from a fire or explosion
Where anyone of these criteria applicable it can be defined as a confined space, it is not
necessary to have all criteria applicable.
• Galfar shall comply and implement the QatarEnergy confined space entry requirements.
• Permit to work system shall be adhered and signed by concern authorities prior to the
commencement
• A dedicated safety watch shall be deployed. Responsibilities of safety watch shall be
educated to the safety watch.
• Entry will only be permitted where a competent person is appointed to plan and directly
supervise the work.
• After the initial gas test, further test shall be completed when resumed after a break, more
than 30 minutes has passed without entry, the ventilation was interrupted for more than 15
minutes, minimum SIMOPS distances were breached, the work permit requires
revalidation, the work permit was withdrawn, such as to allow higher priority SIMOPS
activities to progress. Records of monitoring shall be retained.
• The person who plans, supervises, or enters a confined space shall ne received a formal
training in the safe methods of entry and working in a confined space and the emergency
and rescue arrangements by an accredited training organization.
• Mechanical and electrical isolation of equipment shall be provided where there is a risk of
harm
• Sufficient well-lit access/egress shall be provided. The access/egress shall be wide enough
to allow workers wearing all the necessary equipment to climb in and out easily
• When working in flammable or potentially explosive atmospheres only non-sparking tools,
specially protected lighting and intrinsically safe equipment shall be used.
• Every person that enters a confined space will be given a briefing by the appointed
competent person covering the specific hazards of the work and the control measures to be
implemented.
• It shall be ensured that the rescue arrangements avail at workplace

• Persons entering a confined space will wear a full body rescue harness that is attached to a
rescue line
• Adequate communication arrangements shall be provided; checked before entry and at
regular intervals when anyone is inside a confined space.
• Openings and areas around confined spaces shall be protected, and suitable signs
displayed warning others of the danger.
• Arrangements for emergency rescue and resuscitation of workers shall be included within
the safe system of work. A first aider shall always be on site and rescuers are not to enter a
confined space to rescue someone unless they have been trained and have the correct
rescue equipment.
8.15 Lifting
The lifting equipment, lifting tools & tackles shall be third party certified and inspected by
QatarEnergy. The same shall be inspected by Works Supervisor/Rigger prior to use. Lifting
activities shall be in compliance with the QatarEnergy approved lifting plan. Valid colour coded
tools and tackles shall only be put in service.
8.16 Excavation
• A confined space entry certificate shall be obtained when the excavation depth exceeds 1
meter
• Mandatory documents such as TPI certificate, insurance and registration document for
excavator and license, test certificate for operator shall be produced, prior start work
activity
• Manual digging only be performed in areas where there are underground utilities or utilities
present in close proximity
• Presence of utilities or any kind shall be identified, reference to available drawings and
scanning the area to be completed before proceeding with any excavation works
• Adjacent lines if any shall be protected adequately as per requirements and End User/
Owner concurrence

• Excavations shall be protected from collapse by being benched, sloped, and shored as
appropriate.
• Instruction and training shall be provided to workers in safe digging techniques and a
competent person shall be appointed to supervise all excavation works
• Continuous solid barriers shall be installed around all open excavations and suitable
access/egress (Steps, ramps, secured ladders) shall be provided at regular intervals
• Spoil (excavated material) shall be placed a minimum of one meter clear of the edge of the
excavation, this distance may need to increase depending on the overall depth and the
methods adopted to prevent collapse.
• Materials, plant and heavy construction equipment shall be kept clear of the edges of the
excavation.
• Work shall not be carried out in an excavation unless the excavation and any work
equipment and materials which affect its safety, have been inspected by competent person
• At the start of each shift in which the work shall be carried out
• After any event likely to have affected the strength or stability of the excavation and
• After any material unintentionally falls or dislodged
• Excavations shall be backfilled as soon as possible and not be left open any longer than
necessary
• Excavated material shall be disposed properly or deposited at designated area
• Appropriate personal protective equipment to be provided for the workers involved in the
work activities
• Heavy construction equipment such as excavator, grader and other associated equipment
shall be inspected & records shall be maintained.
• Vehicle speed limits should be derived in-accordance with QatarEnergy road safety manual
for implementation
• Dedicated routes pedestrian movement shall be defined
• Movement of equipment shall be initiated under a trained Flagman
• Reverse alarm, flame arrestors, seat belt, beacon light etc. to be provided as required
• Vehicles, equipment shall be inspected daily and recorded in vehicle checklist
• Drivers work hours and rest period should be defined
• Planning of journeys and reduction of un-necessary trips to reduce the fatigue to due to
continuous driving

• Smoking strictly prohibited any area with the Plant premises.
• All flammable materials shall be stored at designated area located away from the other
structures, areas where combustible materials stored and areas having ignition sources
• The storage area (Fuel/flammable & combustible material) will be fenced, illuminated,
ventilated and Information and warning signs will be displayed
• All the employees will be educated about the fire hazards, firefighting methods, and
precautionary measures specific to project.
• Adequate number of portable fire extinguishers will be placed at strategic locations.
• All employees will be demonstrated the operation of portable fire extinguishers
• Workplace housekeeping will be strictly followed to minimize the fire hazards.
Prior to paint removal, ensure that the surface is free from Lead. Galfar shall ensure that
abrasive blasting agents do not contain silica.
• All abrasive blasting equipment shall be built with safety features and shall be maintained in
accordance with manufacturer’s instructions.
• Approved Type CE abrasive blasting hood/cape supplied with Grade D breathing air,
hearing protection and appropriate body protection shall be worn by the blasting operator
and other personnel within 50 feet of blasting operation.
• The breathing air filter shall be replaced periodically and ensure that clean air is always
supplied to the helmet.
• Every component of abrasive blasting set up shall be checked and its proper function
assured before being put into service.
• The blasting gun shall be provided with a dead man’s handle and it shall be operational.
• Necessary work permit shall be obtained prior to the commencement of the job.
• The filter case shall be equipped with a petcock-type bleeder on the bottom to allow
expulsion of water condensate. This petcock must be left partially opened during use (Note:
Airline helmet must also contain a filter).
• The airline helmet and other protective equipment shall be cleaned regularly and shall be
maintained in good condition and free from dust.
• A trained standby man shall be available near the blasting pot to keep a watch on the
operator.
• The blasting hose must be of the anti-static type.
• The operator shall wear a strong and shot resistant coverall, apron, long safety boot and
hand gloves to protect against the rebound grit.
• Only trained personnel shall be engaged to perform the operation.
• Ensure all hoses used are of sufficient pressure rating and they are in good condition and
free from damages. Hoses shall be examined by feel; if a soft spot is found by flattening
hose between the hands; it should be taken out of service.
• Ensure all hose joints and couplings are secured properly with proper clamps, R- clips /
binding wire and anti-whiplash retentions.

• Ensure that the air supply valve is kept closed when equipment is not in use.
• Open the air supply valve gradually, while starting.
• Personnel exposed to high noise level shall wear hearing protection.
• Ensure that all moving / rotating parts of the compressors are provided with guards.
• Avoid fuel spillage; remove flammable materials in the surroundings, Keep ABC powder
type fire extinguisher ready for immediate use.
• Ensure good housekeeping, secure the materials & equipment properly, route the hoses
safely to avoid tripping hazards.
• Proper enclosure with suitable cloth to be made to contain the flying particles. and dust.
• Spent grit to be collected and disposed regularly off as per QATARENERGY Safety
regulations
• Barricade the area with warning tape and display warning sign board "Grit blasting in
progress - Keep away".
8.21 Painting
Painting Supervisor shall ensure the following safety precautions as a minimum but not limited
to:
• Affected employees must wear proper respiratory protective equipment (RPE) as per
MSDS.
• Personnel handling thinner and other organic chemicals shall wear hand protection,
goggles, and face shield in addition to RPE
• Paint and its associated chemicals are highly flammable. Extreme caution shall be
exercised while handling them.
• Personnel not directly involved in paint application shall not enter the area.
• Work area shall be cleared of combustible materials before the spraying of paint
commences. Portable firefighting appliances shall be always kept standby.
• Necessary work permit shall be obtained prior to the commencement of the job. No one
shall be allowed to paint in an unventilated area. Never spray paint in an area that has a
potential for fire.
• Storage of paint, thinners and other combustibles shall be in accordance with that specified
on the MSDS.
• Containers shall be closed when not in use to prevent fire hazards. Leakage or spillage
shall be disposed of promptly and safely.
• Ensure adequate natural or mechanical ventilation is provided to dispense the flammable
vapors.
• All empty containers shall be disposed of as per QatarEnergy requirements.
All personnel that may come in contact with hydrocarbons containing benzene such task of
draining/blinding/opening equipment and or vessel entry the provision listed below shall be
established:
• 0.5 - 5 ppm = Half Face Respirator with organic vapor (black or yellow) or multi gas
cartridge
• 5 - 25 ppm = Full faces Respirator with organic vapor (black or yellow) or multi gas
cartridge
• Over 25 ppm or unknown concentration = Supplied air (or SCBA in emergencies)
b) Use of nitrile gloves and chemical resistant clothing (Kappler suits) and boots for probable
body contact shall be engaged.
c) Chemical goggles and face shields shall be engaged to prevent probable splashes into
eyes.
d) Draining/blinding/opening equipment containing hydrocarbon streams with >0.1% benzene
will require defined controls established:
• Hydrocarbon containing benzene should be marked with barrier tape and signage.
• Anyone entering a hydrocarbon contaminated area will be required to wear respiratory
protections.
• Establish a "regulated area" (with red "Danger" tape) and post signs where the level more
than TLV.
• Access to the regulated area should be restricted to authorized personnel.
• Regulated areas may be required during equipment preparation, mechanical work on
unprepared equipment, inside vessels or tanks or during spills.
e) Decontamination:
• Wash contaminated skin immediately with soap and water
• Ensure employees are familiar with the locations of eye wash stations in close proximity
• Contaminated clothes shall be washed separately.
• Gas tests should be completed to verify if additional requirements (e.g., sign posting) apply
when process streams contain toxic components such as hydrocarbon, CO, H2S, etc.
• Reactor space must be ventilated during entry into the reactor.
• Avoid the formation of metal carbonyls with proper vessel preparation.
• Vessels containing nickel or cobalt catalysts should be held at high temperatures (>400F)
and lowest possible pressures until Carbon Monoxide (CO) containing gases are purged
from the vessel.
Refractory/gunite material may contain various types of hazardous materials like asbestos,
manmade mineral fibers, silica, and ceramic/mineral wool. The removal, repair and/or
installation of these materials can result in exposures if not properly handled.
• Identify presence of asbestos prior to disturbance, repair or application. If asbestos is
present, only approved abatement contractors may perform removal.
• Entry into furnaces or heaters with refractory or any handling of refractory material
requires the use of HEPA type respirator and disposable flame resistant coveralls.
• Cutting/chipping out concrete or refractory should be done with water mist or by
dampening the material to minimize dust.
• Mixing of refractory grout or gunnite material containing silica will require special
precautions to control dust from the mixer. Appropriate respirators shall be required.
• Debris should be cleaned up with HEPA vacuums or wet sweeping.
• Face shields and chemical gloves compatible to the substance are required to be worn
during process equipment opening and opening container containing chemicals.
• All chemical containers must be properly labeled and the original label to be maintained
till the point of disposal.
• Galfar shall inform QatarEnergy before bringing in the chemical for use. A completed copy
of hazardous chemical approval forms along with SDSs, must be forwarded to Environment
& Hygiene before bringing chemicals onsite.
• Contractors shall not take contaminated work clothing such as disposable coveralls, gloves,
etc. outside the process units. This limits the probability for contamination.
• Galfar shall demarcate chemical cleaning jobs with red barrier tape, relevant signage to
keep out nonessential personnel.
• Spray painting and epoxy applications in enclosed space generally requires supplied air
respirators
• Continuous LEL monitoring for the area
• Restriction of hot works in nearby areas, any such works shall be reassessed and
acknowledged by Permit Issuer.

Galfar anticipates probability for the use of respiratory protection for the protection of
employees engaged for the Turnaround/Shutdown. All static equipment works shall commence
only after necessary arrangements made in compliance with VENTILATION PLAN.
9.6 Welding
• Prior to welding/burning on coated metal (painted, galvanized, etc.) the surface must be
prepared by removing the coating 6 inches on each side of the cut line.
• For confined space welding/cutting, use of local exhaust ventilation providing a minimum
capture velocity of 100 feet/minute in the welding zone.
• CSE- any welding/cutting where base metal, fluxes and filler metals may contain a
potentially hazardous material like chromium where release of hexavalent chromium is
released, fluorides, zinc, cadmium etc. r for inert gas metal arc welding or oxygen cutting of
stainless steel, breathing air is required if local exhaust is not effective in preventing
exposure levels from exceeding respective limits
• For confined space welding/cutting not involving hazardous materials (carbon steel)
general mechanical dilution ventilation may be used if local exhaust is not feasible.
• Confined space inert gas metal-arc welding on stainless steel (MIG, TIG) welding, shall
require continuous monitoring for O2.
• Confined space welding/cutting shall be periodically monitored for carbon monoxide.
Galfar shall maintain industrial health and hygiene standard by implementing adequate control
measures to control the workplace hazards and risks. Some of the more serious risks are
caused by the inhalation of dust, toxic fumes, exposure to high temperature, noise, vibration,
radioactive substances, ergonomic hazards etc. Project management shall be responsible for
maintaining healthy working conditions for all our employees and sub-contractors by taking risk
reducing measures (Engineering and administrative). If it is not possible to remove the cause of

harm, then suitable and sufficient Personal Protective Equipment (PPE) shall be provided to
work personnel to protect them from health and safety risks.
Galfar ensures that fitness to work examination certificate of all personnel obtained prior to the
deployment to site work. Galfar ensures that the periodic medical examinations shall be done
for Welders, Painters, Drivers, Food handlers and Work personnel who continuously exposed
to noise at workplace through medical commission approved medical centers.
(a) Noise
Noise-Induced Hearing Loss (NIHL) is caused by overexposure to high levels of noise from
plant, machinery, or construction processes. Equipment such as Hydrojetting machines, Diesel
Generators, venting Static equipment, and Compressors generates high noise.
• Noise labels indicating the maximum noise generated by the equipment shall be provided
on the equipment along with signage.
• Noise enclosures shall be provided as an engineering control.
• Workplace shall be monitored for noise level and controls will be undertaken if
abnormalities found
• Employees shall be provided with proper hearing protection against the noises which
exceeds the limits. The allowable limits 85 dB for 8 hours.
• The noise meters shall be calibrated properly, and records retained
• Employees at high noise area shall be provided with proper protection, intermittent work
breaks

(b) Dust
• No personnel will be exposed to dust unprotected. All employees will be provided with
balaclava.
• In any case no personnel will be exposed directly to harmful airborne contaminants of
Silica, Rust (ferrous oxide), Blasting grit, Asbestos, Glass wool & Paint solvent mist.
• Respiration protection against dust from excavation, during handling and storage of cement
and concrete mixing preparations
• Adequate personal protective equipment will be provided for protection against the above-
mentioned hazards.
• Dust eruption from the work area will be controlled by spraying water on the area around
the work site.
(c) Heat Stress Management
Scope of services includes performance of works in areas which are confined, heat
generated as a byproduct of the welding, cutting, excessive physical activities etc. Galfar
shall monitor such activities and arrangements shall in place to avoid adverse effects. Work
rotation, frequent breaks, provision of drinking water, rest area with mist fan and seating
arrangement shall be provided.
Galfar ensures that the personnel engaged for the shutdown shall work for maximum 12 hours
only on workdays. No personnel shall work consecutively for 14 days without day off.
• If same personnel is required to continue work beyond 12 hours, the same shall be
coordinated to QatarEnergy Authorized/ Responsible Person for approval and proper
compensation to be implemented. In such cases, certain conditions to be met:
- Additional manpower to be arranged to replace existing personnel at the end of the
work shift
- Necessary transportation shall be arranged for such personnel to return to the camp
upon completion of their duties
- Such jobs shall be continuously monitored. If required, food will be arranged for such
employees. Care shall be taken to ensure that during such works for extended hours
shall not exceed 16 hours.
- Food serving time will be altered accordingly to prevent interruption in sleeping
especially for those working in night shift duties, if any.
• Further any FRIDAY/ HOLIDAY Works should be properly notified to QATARENERGY well
in advance and approval shall be obtained before proceeding with the activity.
If someone experiences fatigue it is important to identify the factors that are contributing to
fatigue, discuss any issues with the supervisor, make changes as required, including sleeping
patterns, workloads, rosters and lifestyle behaviors and seek professional help if necessary.
• All containers shall be marked "FOOD" and shall be marked with the date and time of
preparation.
• All vehicles used to transport fresh, frozen or pre-prepared food shall be mechanically
refrigerated to the temperatures required of freezers or refrigerators.
• Thermo boxes should be clean, closed and in good condition to store food.
• The required temperatures should be maintained for the food kept inside which is minimum
65°C.
• Checklist should be available for the transport vehicle and filled with the required
information to check the above items (temperature, hygiene, cleanliness, pest, etc.).
Water igloos, thermos water bottles shall be provided and replenished regularly with quality
drinking water from trusted sources. Galfar shall comply with the requirements of the WHO
Potable Water Guidelines and State of Qatar laws and regulations and the water analysis
results should be available.
9.11 Accommodation
• Assess for suitability and adverse effects referring valid SDS for materials brought on site
• Safety briefing shall be provided to all workers who might be exposed to hazardous
materials. Briefings conducted to increase awareness of hazard potential and prevention of
excessive exposure to hazardous materials.
• Hazard warnings and information as per NFPA or GHS labels shall always be available on
hazardous materials containers/ packing.
• Hazardous materials if any, shall be stored in limited quantity at designated storage areas
only. Secondary containment shall also be arranged. Refer to relevant SDS for specific
requirements.
• PPE shall be available and worn at all times during the task
• Continuous gas or air monitoring to ensure that atmosphere of the workplace is still within
the safe threshold limits
• Spill containment or kits and clean up recovery equipment are available
• Access, egress, and emergency route of storage area must be kept clear
• Portable fire extinguishers shall be provided in the handling and storage areas.
Galfar has developed a comprehensive Pandemic Response Plan Covid-19 doc no. GAL-HP-
01, to contain the spread of Covid-19, as well as prevention of the disease and also to be
prepared for emergency situations that may occur within the facilities or the workplace.
• Compliance to all ministerial/MOPH directions and Galfar pandemic response plan shall be
ensured through various awareness program, posters, and informative signage.
• Regular communication to be given to personnel through toolbox talk, email, display in
notice boards and alert to keep personnel up to date.

• Galfar is also providing the necessary and adequate arrangement as and when required to
ensure testing and controls.
10. Waste Management
Galfar ensure that the QatarEnergy waste management guidelines shall be adhered. Wastes
generated during the Shutdown shall be reported to QatarEnergy in the prescribed format.
Non-hazardous waste generated like food waste, spent gloves, packing materials etc. shall be
disposed through approved service provider. For wastes generated in the Plant, disposal will
be as advised by QatarEnergy.
• Waste transit areas are identified in each Plant for short term temporary storage of wastes,
before being shifted to QatarEnergy Waste Yard or any other approved location.
• Workplaces shall be well maintained at all the time in the access areas, common areas,
toilets, rest areas, eating areas, drinking water facilities.
• The concerned supervisory personnel are responsible to remove the waste and
unwanted materials from workplace at the end of each shift or as soon as their work is
completed.
• The flammable wastes such as empty containers of paint, and adhesives etc. should not
be disposed-off in the scrap bins.
• The garbage bins provided in the temporary facilities area will be cleared daily
• A skip will be maintained on site to dispose of all solid waste materials, and it will be
removed through authorized agencies at regular intervals.
Hazardous Waste:
• Hazardous wastes shall be labelled appropriately, stored within the unit as and when it is
generated.
• No hazardous waste shall be mixed with any other category of waste or discharged to a
common or other internal or external sewerage or other drainage system.
• Galfar shall keep records of hazardous wastes generated in work areas. The records shall
be submitted on a daily basis.
• Wastes as expected from the Plant during the Shutdown includes: Spent Catalysts (All
types of Catalyst except precious metal bearing catalyst), Absorbents, Pyrophoric Waste,
Catalyst Support Material (Ceramic Balls), Waste Sulfur, Waste Oils (Lube Oils /
Transformer Oils), Oily Sludge, Biological Sludge, Contaminated Soil (Oil or Chemical
Contaminated), Empty Metal Drums, Empty Plastic Drums, Coke, Resin, Asbestos Waste,
Sand from Filters, Activated Carbon, Used Batteries, Incident related wastes, Oil soaked
gloves / rags, Grit Blasting Waste – As per QP-GDL-V-001
Galfar shall perform internal audits at monthly intervals to determine the level of compliance
with applicable requirements. Deviations identified will be dealt as per internal procedures for
recording nonconformity, corrective actions. All actions shall be tracked for effective closeout.
Adequate root cause analysis shall be completed to address the issue and determine
corrective actions. The effectiveness of actions shall be monitored through the established
methods for performance monitoring. Risk assessments shall be updated if required.

Subcontractors are treated as integral part of the safe execution of the shutdown and they are
advised in kick off meeting to follow the approved HSE Plan and procedures. Copy of such
documents shall be shared for reference and compliance. Subcontractor focal point shall report
to the Project Manager functionally and provide details or evidences of compliance to project
HSE requirements to the HSE personnel from time to time. Subcontractor focal point to attend
HSE meetings as required by Galfar.
Subcontractor employees shall participate in all HSE activities like training and awareness
programs, promotional activities along with Galfar personnel. Subcontractors are evaluated
based on the risk control measures adopted, results of regular HSE inspections and lagging
indicators. These results will be captured in Galfar internal system for retention of
Subcontractor for future projects.
Upon completion of the Turnaround and restoration of the site, Galfar shall submit overall HSE
closeout report highlighting successes, lessons learned and areas for improvement. HSE
closeout report shall include as minimum:
In any emergency situation, anybody observing such situation shall call emergency services.
