INSTRUCTIONS: Please complete the enclosed O-1 worksheet to the best of your ability. Also note
that certain criteria may not apply to your case or situation, so it is okay to leave some sections blank.
WE MUST BE ABLE TO COMPELLINGLY ARGUE AT A MINIMUM THREE OF THESE
CRITERIA.

IMPORTANT: U.S. Immigration considers primary, independent evidence to be the strongest. In


most cases, reference letters should only be used to supplement the petition and are not strong evidence of
accomplishments. Photographs that do not obviously demonstrate an accomplishment (such as a picture
of you at a podium accepting an award) are also not acceptable evidence. Examples of acceptable
primary evidence are included under each criterion below. If you cannot provide the requested examples
of evidence, let us know what you may be able to provide so that we may advise if it is permissible or not.
Please do not provide us with any Wikipedia links.

1. AWARDS – Please list all nationally and internationally-recognized prizes or awards for
excellence in the field that you have personally received. DO NOT PROVIDE: University-level
prizes, or awards given by employers. Please only provide evidence of substantial grants
(ideally over 100k).

EXAMPLES OF ACCEPTABLE EVIDENCE:


 Photos of award (your name must be clearly legible), scan of award certificate,
congratulatory letter from organization confirming receipt of award;
 Print-outs from organization websites or publications discussing criteria used for
giving prize, number of entries/applicants the year you won, jury members or
information on the body who judged the awards, big name past winners, etc.;
 Press in major publications covering award criteria and your win of the award.

2. MEMBERSHIP – Please list your memberships in associations in the field which require
outstanding achievements of their members, including advisory boards, executive boards, and
editorial boards. DO NOT PROVIDE: Any dues-paying organizations or standard professional
organizations.

EXAMPLES OF ACCEPTABLE EVIDENCE:


 Copy of membership card, print-out from online database listing you among
members, copy of membership card, or letter from association confirming
membership;
 Print-outs from organization websites or publications discussing criteria for
selection for membership, national or international experts who make
determination about membership, other big name members of the organization;
 Press discussing the organization and its selective membership.

3. PUBLISHED MATERIALS ABOUT YOU – Please describe any media attention you have
garnered. Interviews and profiles on you and your work are preferable. Please indicate if you are
not mentioned by name in an article or media feature.

TYPE HERE:

EXAMPLES OF ACCEPTABLE EVIDENCE:


 Copies of all press materials about you and your work, including copies of articles,
or evidence of interviews/appearances on television, radio, or podcasts. This
evidence must contain the title of the publication/media source and date
published/aired. If published online, please be sure that the link to the article is
included;
 Evidence of circulation rate, TV ratings, audience reach of website or radio
program, or other evidence of the distinguished nature of the press source from
other media sources or organizations/websites that track such information.

4. PANEL/JUDGE – Please describe your participation, either individually or on a panel, as a judge of


the work of others in the field.

EXAMPLES OF ACCEPTABLE EVIDENCE:


 Email requests or letters inviting you to serve as a judge of the work of others in
your field;
 Print-outs from organization websites or press listing you among members of a
jury/judges panel;
 Print-outs from organization websites or press discussing criteria used to select
judges/reviewers/jury members;
 Independent press or print-outs from organization websites discussing event you
were asked to serve as a judge for, including information such as number of
submissions for a competition, past prestigious jury members, attendance numbers
for festivals and conferences, etc.

5. ORIGINAL CONTRIBUTIONS – Please list evidence of original contributions of major


significance to the field. For this criterion, we can use reference letters to discuss these
contributions in more detail.

TYPE HERE:

EXAMPLES OF ACCEPTABLE EVIDENCE:


 Professionals in your field who can provide reference letters on your behalf – The best
referees are independent (i.e. have not worked at an institution you are or have been
affiliated with) and high-profile in your field;
 Evidence from Google Scholar or ResearchGate demonstrating a high citation rate of
your authored scholarly work;

 Patents (not pending);


 Any additional evidence that your work is being used by others in your field, such as
contracts and press about your contributions.

6. PUBLISHED ARTICLES AUTHORED BY THE APPLICANT – Please describe your


authorship of scholarly articles in the field, published in professional or major trade publications or
other major media.

 Copy of publications, including your name, title of the article and publications, and date
published
 Evidence of ranking, circulation rate or audience reach of website your work was
published on, or other evidence of the distinguished nature of the publication source from
other media sources or organizations/websites that track such information.

7. LEAD/CRITICAL ROLE – Please describe how you have performed in a lead or critical role for
organizations or establishments with a distinguished reputation. . For this criterion, we can use
reference letters from managers, supervisors, department chairs, or executives to discuss your
role in more detail.

EXAMPLES OF ACCEPTABLE EVIDENCE:


 Managers, supervisors, department chairs, or executives from current or past positions
that can provide letters attesting to your leading and critical role for the organizations;
 Official organizational charts demonstrating how many people you directly manage;
 Evidence of internal company/university awards you have received, such as photos of
award (your name must be clearly legible), scan of award certificate, or congratulatory
letter from organization confirming receipt of award;
 Independent press articles or organization press releases/website print outs discussing
information on the distinguished nature of the organization, such as number of total
employees, net worth/endowment, distinguished members of executive/leadership board,
awards won by organization, etc.

8. HIGH COMPENSATION – Please list evidence of your high salary or other significantly high
remuneration in relation to others in the field. If you are not sure that you qualify, let us know
what your annual salary/hourly rate is so that we may help you determine if it is considered
high for the purposes of this criterion.
