Method Statement for Installation of FM-200 System

PURPOSE
The purpose of this method statement is to describe the sequential approach for Installation of
FM-200 system in accordance with the contract requirements. This method statement shall
ensure that the installation of FM-200 System is executed safely. This method statement aims to
ensure that quality assurance and quality control activities are adopted by Contractor M/s JTC in
systematic manner accordance with project requirements, works are inspected by M/s ATKINS
and conformance is verified and documented.

The Contractor M/s JTC reserves the right to adapt the methods of execution as contained in this
document from time to time, according to specific site conditions, in agreement with the Engineer.

1. SCOPE OF WORK
1.1. This method statement describes:
As per contract Scope of Works, Project Specifications, QCS & Manufacturer recommendations the
Subcontractor (JEM) will carry out the installation of FM-200 System using the approved materials,
approved shop drawing & work will be done as per standards for the construction of “MAIN WORKS
IN QETAIFAN ISLAND NORTH AT LUSAIL QATAR, PHASE 2, PACKAGE 8 - INFRASTRUCTURE &
LANDSCAPE”.

This Method Statement specifies the work, testing methodology, Health, Safety, and Environment
requirements for the installation of FM-200 System.

1.2. System Details:


 FM-200 System: Clean agent Fire Suppression System, it rapidly extinguishes most commonly
found fires through a combination of chemical and physical mechanisms. Zero ozone depleting
potential.

WIR- Work inspection Request

4. RESPONSIBILITY

4.1. Project Director (PD) – project director will assume the role of Contractor’s representative
and will be responsible for every activity in the project. JTC PD shall ensure that all necessary
safety and protective equipment are provided along with properly trained and qualified
personnel dedicated to work on the task at hand. He shall request external services and
equipment and control quality of delivered items and delivery schedule.

4.2. Construction Manager (CM) - is overall responsible for the implementation of this
method statement. CM shall ensure that the progressing works are carried out according to the
planned program and all the equipment’s required to execute the works are available and are in
good condition.

He will co-ordinate with the Consultant, contractor’s Project Manager, HSE Manager, QA/QC
Manager, Project Engineer, Safety officer, and Sub-contractor (if any) for the safe and proper
execution of the work. He will pay specific attention to all safety measures and quality control
activities and ensure that the activities are in line with HSE plan and Project Quality plan.

4.3. Quality Manager (QM)/SR. QAQCE - shall be responsible for all quality assurance and
control activities in the project and proper implementation of the project quality plan. He shall
ensure that all inspections and tests are carried out as per project requirement and in turn are
recorded. He will monitor that all quality records relevant to verification of records are
maintained properly.

4.4. QC Engineer/ Inspector (QCE/ QCI) - will be responsible for the inspection activities,
tests & records and shall co-ordinate with the Employer QC/ inspection personals for all pertinent
activities. He will ensure that all works are carried out as per the specification, drawings, and
the related quality procedures (ITP, MS etc.). QC Engineer/ Inspector will ensure that the work is
completed and satisfied, prior to requesting for the Consultant’s inspection/ approval.

4.5. HSE Manager (HSEM) - will be responsible for day-to-day activities on maintaining safety on
site complying with the requirements of project HSE plan. Ensure the implementation of all safety
measures in accordance with the HSE plan. Conduct incident/accident investigation and prepare a
detailed incident report for the Engineer
He shall ensure all the implemented safety measures are adequate to maintain the safe working
on the site. He shall frequently Inspect all the site activities and train the persons to prevent any
accidents occur. He shall maintain proper reporting for HSE matters to the project manager and
the Employer/ Consultant.

4.6. Deputy HSE Manager -

 Ensure health, safety and environmental working conditions are followed at all times.
 Responsible for the implementation, maintenance, and monitoring of the Health and Safety
Plan and Procedures on site.

 Enforce safe working practices and to ensure incidents and critical conditions to be eliminated
and reduced
 Perform regular audits and monitor HSE performance to identify areas for Improvement and
ensure that appropriate corrective or preventative actions are taken, including follow-up
reviews. Conduct accident/ incident.
 Investigation and report to client/ consultant on an immediate basis.
 Maintain proper coordination and cooperation in line with client/ consultant Representatives
 He shall cover the duties of the project HSE Manager during His absence.

4.7. Environmental Manager (EM) - Daily inspection of the Work Site in cooperation with JTC
and Subcontractor’s Environmental Officer.

Ensure that the monthly environmental report is issued on time and sent to PMC/JTC for eventual
issue to the MME. Organization and performance of weekly inspections and maintain record of
weekly inspection and follow up of corrective actions, all procedural activities and preparation and
distribution of Environmental Statistics and follow-up status; Fostering and maintenance of
good relations with government and local authority Environmental officers.

Plan and execute necessary Environmental risk assessments.

Communicate the details of JTC and their subcontractor’s Environmental performances to


PMC/JTC on a frequent and regular basis.

4.8. HSE Officer/ Inspector - HSE team will coordinate with the project team to create and
maintain safe working conditions. Assist project/ site engineers to organize the work area in order
to perform their duties with maximum efficiency and minimize risk to them or to equipment.
Conduct incident/accident investigation and prepare a detailed incident report for the
Engineer.Inspect equipment and tools before use to ensure correct storage and proper
utilization. Maintain record of all injuries and damages on site. Maintain and submit HSE reports
as required to the HSE Manager.

4.9. Accident Prevention Officer -

 Assigned to prevent accidents, respond to emergencies, and evaluate the Effectiveness of the
company’s safety programs.
 Accident Prevention officer identifies safety hazards, investigates them thoroughly, and
controls them before someone gets hurt.
 Conduct ongoing review of operations and processes to identify potential hazards, risks and
control measures that should be implemented to reduce these risks, including all costs involved
in implementing such measures.
 Investigate workplace accidents and injuries and refer them to the proper authorities

4.10. Temporary Works Co-Ordinator - shall be responsible to site activities including


excavations, formwork, and falsework and back propping for concrete. He shall ensure that work
register is established and to ensure that a permit for load and unload is issued.

4.11. Section Project Engineer/ Site Engineer (PE/SE) - shall be responsible for supervising and
monitoring all the concreting activity in the project. He shall organize the work force in a safe
manner and to ensure that safety and protective equipment are properly utilized.

He must also ensure that the method statement is followed and shall observe any hold points
noted on applicable ITP and notify Employer/Consultant for inspection in coordination with JTC QC
team. He will co-ordinate with the Safety Officer to ensure that the works are carried out in a safe
working practice.

He shall coordinate with QC Engineer for the tests to be carried out and initiate for the inspection
for the completed works, on timely manner.
4.12. Land Surveyors Manager - shall manage and coordinate surveys and investigations of land
surfaces. Ensures compliance to all applicable regulations and requirements including boundary
calculations and legal issues & ensures projects are completed on time and within budget.

4.13. Land Surveyor - Shall be responsible for most of the surveying activities on site e.g.:
 Setting out, coordinates and levels as per the project approved drawing and data.
 Prepare and maintain sketches, map, and legal descriptions of survey in order to describe,
certify, and assume liability for work.
 Verify the accuracy of survey data, including measurements and calculations conducted at
survey site
 Calculate height s, depths, relative position, property lines, and other characteristics of terrain.
 Record the results of surveys, including the shapes, contour, location, and dimensions of land
or land features
 Prepare or supervise preparation of all data, chart s, plots, maps, records, and documents
related to surveys
 Write description of property boundary surveys for use in deeds, leases, or other legal
documents
 Plan and conduct ground surveys designed to establish baselines, elevation, and other geodetic
measurements
 Search legal records, surveys records, and titles to obtain information about property
boundaries in areas to be surveyed
 Coordinate finding with the work of engineering and architectural persona l, clients and others
concerned with projects

4.14. Supervisors/Foreman - Be familiar with those parts of the Schedule applicable to the work
on which subordinate workers are engaged.
 Incorporate safety procedures into routine tasks and see that they are adhered to (Pre-Job
Safety toolbox talks).
 Conduct daily work site inspections to identify and correct any existing unsafe conditions.
Document and coordinate the safety inspection activities and findings with the job site Safety
Officer or Safety Manager.
 Correct unsafe acts such as horseplay or the taking of unnecessary risks. Issue warning letters
or any other form of disciplinary action required.

 Report accidents, unsafe conditions and equipment defects into their immediate Superiors.
Responsible for the safety performance in his area of responsibility. He will be responsible to
the Project Engineer.
 Implement and observe the company and client's safety requirements within his scope of
works.
 Conduct Pre job Safety Instructions to determine the most appropriate order and method of
work with safety as a prime consideration.
 Directly responsible for maintaining safe working conditions, practices and the safety of
personnel under his supervision.
 Enforce compliance of all safety rules and regulations in the workplace.
 Conduct investigation of all accidents in his area of responsibility with the cooperation of the
safety officer.
 Indoctrinate new hires and transferred workers concerning the hazards of the work or task to
be performed
 Have a thorough knowledge of safe work procedures, legislation's applicable to the work being
performed.
 Develop a proper and cooperative attitude toward safety among his men.
 Maintain an effective line of communication on safety matters to his men.
 Set a personal example for others to follow

4.15. Skilled/Unskilled Employees - Shall take reasonable care to avoid injury to him and others.
 Shall minimize any damage to the environment caused by JTC operations.
 Must comply with the rules and regulations made by JTC or by the Qetaifan Projects/ATKINS
about H & S.?
 Should use the personnel protective equipment supplied by JTC as required.
 Should cooperate in joint consultations.
 Must accept the need for H & S training.
 Must keep all working areas clean and tidy.
 Should not remove any guards, protective appliances/equipment or interfere with plant,
machinery or vehicles in any way which may be dangerous to him or to others.
 Should seek first aid treatment for all injuries sustained at work.
 Shall inspect all equipment prior to use and report faults to his immediate supervisor.
 Shall report accidents/near miss to the Project Management.
 Must be fully conversant with the COMPANY Emergency Procedures which are to be posted on
the notice board (s).
 Where a ‘Permit to Work’ System is in operation, it is his duty to ensure that he fully
understands the procedure. If in doubt, he should consult his supervisor.
 Shall report damage to any vehicle, plant, or machinery to his supervisor immediately.
 Are to accept that the final level of responsibility is that of every individual employee.

4.16. Technicians - Inspecting, analyzing, and troubleshooting systems and equipment.


 Running tests and interpreting results to make effective recommendations.
 Writing up reports, safety regulations, and preventative maintenance plans.
 Negotiating with suppliers and clients. Providing time and material cost estimates before
starting a job.

4.17. Site Electrician - Perform regular inspection and maintenance of lighting equipment (as
directed by field supervision). Make sure that cables and other material are in a way that
prevents trip hazards.

5. PERMITS AND LICENSES REQUIRED


Covers any permit to work deemed to be required in carrying out the installation of FM-200
System works for the project. This includes any applicable HSE related permit to work system and
any permits & licenses required by local authorities with regards to the installation process.

6. HSE REQUIREMENT
JTC is committed to provide a comprehensive Occupational Health and Safety Oriented and Lost
time injury (LTI) free for all the workplaces in the construction sites. Please refer to Health and
Safety Plans for more details.

6.1. General Health and Safety Policy


 Ensure the working area is in safe conditions and maintain good housekeeping.
 Ensure to carry out the first aid and emergency response according to Hazard Risk Assessment.
 Equipment and machineries shall be in good condition and shall have a valid certificate (Third
Party Certificate).

 Safe access and egress route must be provided form the work area that separate vehicles and
walkways.
 Toolbox talks over hazards and risks involved shall be conducted to all personnel employed in
the works prior to starting the activities.
 The equipment shall be left in clean condition and free from debris. All disinfectant materials
will be disposed in accordance with local regulations. Waste like empty drums, maintenance
waste, etc. will be sent back to stores for further disposal.
 Employees shall not be allowed to work above or in positions exposed to protruding reinforcing
steel without proper protection. Protruding steel, on to and into which employees could fall
shall be guarded to eliminate the hazard of emplacement.
 No construction load shall be placed on a concrete structure or portion of it unless the
employer determines, based on information received from a Qualified Engineer that the
structure portion is capable of supporting the load.
 In the block works (Working at Height), fall protection system shall be provided which will
prevent personnel from slipping, falling and prevent personnel on lower levels from being
struck by falling objects. Provide a safe working platform (scaffolding) with guardrails, fences,
toe boards, etc. that are strong enough to prevent fall.
 Dangerous exposed protruding steel that can be susceptible of creating an accident must be
covered with a Cap / Cover.
 All materials shall be transported by telehandler equipment.
 Construction works should be suspended when any undue movement of the false work occurs.
 Investigation of the causes of the undue movement should be carried out by competent site
management personnel and corrective measures should be taken.

6.2. Risk
 Hazard identification and risk assessment shall be carried out within 2 weeks of mobilization of
the project.
 Projects shall establish, implement, and maintain the on-going identification of hazards,
Assessment of their risk and determining the necessary controls. Keep the records up-to-date
and shall be reviewed periodically.
 Job safety analysis shall be done to integrate accepted safety and health principles and
practices into a job or task. Each job or task will be analyzed by breaking them down into
distinct steps to identify the hazards and precautions and implemented with the aim of
ensuring a safe job execution.

6.3. Health and Welfare


 Sanitary Facilities
 All basic sanitary facilities will be available on site. c) Health Care first aid Facilities
 JTC shall provide the number of First aid boxes furnished with the medicines, tools and
equipment. The First Aid box shall be available in offices of Clients, JTC, Satellite and res
shelters depending on number of employees.
 A first aider shall be provided for every 25 workers.

6.4.5. Safety Signage - Proper facility signage can save more than just a life. Concise and consistent
signs make it simple for Employees and visitor to easily identify hazards. This reduces
workplace injuries, promotes safety awareness, and increases the effectiveness. Accidents
detract from productivity and schedules and can have high costs, by way of medical fees, legal
fees and regulatory fines. JTC will ensure your safety signs are compliant.

6.4.6. Safety to the public - JTC will ensure public safety during construction is by managing site
access. Define site Boundaries clearly and physically with suitable fencing. A fence can help
to prevent people from entering the site and prevent particular objects from escaping.

6.4.8. Storage - All material shall be stored in assigned laydown area.

6.5. Interface Management - The Project Engineer shall be held responsible for interface with
another contractor. Provide advance Notice of special Environment, Safety, Health and Quality
requirements and include service providers in facility/pre-job meetings to ensure work is
performed safely/meets project requirements.

6.6. Site Night Works


 Working in the night shift will be arranged with all the necessary Safety and Health facilities.
 Site Representative for supervision will be available and Client approval will be secured for
working at night.
 JTC shall provide lighting to avoid visual fatigue and prevent glare and reflection into people
eyes.
 During night operations only clear or amber colored safety glasses shall be worn. Contact lenses
are allowed to be worn with eye protection except while using a respirator.
 In every part of the area under which JTC has control and where persons are working or
passing, JTC shall ensure lighting, whether natural or artificial. This measure shall also prevent
the formation of shadows. In particular access & escape routes to be lighted properly.
 Any general lighting source shall be mounted at a height sufficient to keep it well above the line
of normal vision.
 Any local lighting source shall be mounted and arranged so as to confine the light to the
immediate area to be illuminated.
 Barricade the Hazardous area and use flashing lights.
 Keep combustible materials away from lights.
 Personnel to wear high visibility reflective vest.

This Method Statement outlines the requirements (minimum levels of illumination) and
responsibilities for any work activity during the hours of darkness in areas where general
Illumination / lighting is required.

6.7. Work Duration and Schedule


 The frequency at which the task must be performed and how much time is required to perform
the work? How much time is required for preparation, set- up, actual task performance, and
any normal breaks? Excessive overtime work, piece work, and machine-paced work are
additional factors to consider. Each can cause fatigue and increase heat stress potential.
Regular Site timing will be 06.00 am to 5.00 pm and night shift will be 05.00 pm to 4.00 am.

6.9. Emergency Response


 JTC will follow a Contingency Plan and Emergency Procedure during the period of the work and
shall ensure that its staff is fully familiar with the emergency procedures, use and location of
the essential emergency equipment, such as breathing apparatus, fire extinguishers, fire hoses
etc.
 Safe assembly point (Muster Pont) is located near entrance of each site location.
 In case of emergency, the work force will assemble in the safe assembly point in consideration
of wind direction (Safe assembly point is provided with sign boards).
 Timekeeper / Security Personnel shall carry the attendance register of specific location for head
counting of attendees.
 Safety Officer shall verify the attendance register to ensure that all the personnel have
evacuated from the hazard area.
 Attempt to put out the fire / rescue operation shall be made only when it is safe to do so. Fire
shall be put out under supervision of trained personnel.
 All flammable material in the vicinity of fire shall be removed.
 Safety Engineer / Safety Officer shall call Fire Brigade, Ambulance, Police, etc., and Contact
Project Manager / Health Safety Manager for further action.
 First aid shall be administered to the injured personnel, if any.
 Project Director / Project Manager shall ascertain the risks and ensure control measures are
implemented prior to resuming work.
 JTC shall notify LRDC’s management of the said fire or incident, either directly in person or by

6.9.1. An emergency can include any of the following circumstances that may require a halt to
business or an evacuation of the premises: (Fire, Chemical spills, medical emergency, Building
Evacuation, Work over or near water rescue, Excavation, Heat Stress)

a) Fire
1. Preventative Measures - JTC shall take reasonable fire safety and emergency
prevention measures, including but not limited to:
 Compliance with Qatar fire prevention laws, insurance requirements and other
requirements regarding the storage of flammable and combustible materials,
electrical installations, building materials, and ventilation.
 Enforcement of JTC No Smoking Policy.
 Monthly workplace inspections.
 Meeting health and safety training standards.
 Communication with local Fire, Police and Ambulance services.
 Installing and maintaining the appropriate number, type and size of portable fire
extinguishers.
 Maintaining emergency lighting, where required.
 Maintaining clear corridors, employee assembly points and emergency exits
free from obstruction.
 Providing on-going training to all employees.
 Having an up-to-date floor plan of facility available.

2. Fire Extinguisher Use - If the fire is small (and you are confident it can be easily
controlled), use a fire extinguisher of suitable type. Always point the extinguisher at the
base of the fire. Remember PASS:

 P_ Pull the safety pin at the top of the extinguisher.


 A Aim the nozzle, horn or hose at the base of the flame while holding
extinguisher vertically.
 S Squeeze or press the handle to release the extinguisher agent.
 S _____Sweep from side to side at the base of the fire and at least six inches past the
edges of the flames until completely extinguished.

Remember, fire spreads quickly! If you can’t find an extinguisher, or if you are unable to put

out the fire for any reason, evacuate immediately. Your reaction in the first few seconds is
critically important. If the fire is out of control, leave the area immediately and initiate a fire
emergency response.

b) Chemical Spill - When a chemical spill occurs, action must be taken as quickly as possible to
protect individuals in the area and to contain the spill. Small spills shall be dealt with
immediately by the supervisor in charge according to the Material Safety Data Sheet (MSDS)
for the product.

1. A Chemical Spill - is defined as the following:


 Any hazardous substances that can safely, cause adverse health effects, cause
property damage or cause environmental damage.
 In the event of a chemical spill:
o Isolate the surroundings to prevent anyone from entering the area and
remove anyone who may be in the vicinity.
o If toxic fumes are present, the supervisor will evacuate the building
immediately.
 The supervisor will then do the following:
o Contact 999.
o Place signs and caution tape to secure the area, if necessary.
o If it is safe to do so, place absorbent material near the area where the spill is
moving.
o Clean up the spill as per the requirements of the MSDS.

c) Medical Emergency
1. In event of a medical emergency the following will be adhered to:
 Evaluate the accident area to ensure that it is safe to render first aid.
 Do not move the victim unless greater danger exists.
 The first aid responder must provide first aid to the victim(s).
 Identify and evaluate the injury.
2. Contact 999 by sending someone to the nearest phone, or provide
immediate transportation to the doctor’s office, hospital (Qatar University Health
Center) or home as required.
3. At the time of the call, provide the following information:
 Location of the injury scene.
 Nature of the injury and the number of victims.
 Stay on the phone until advised to hang up.
 Send someone to notify management of the emergency.
4. After the emergency situation has subsided:
 Complete the Interprovincial Insul ati on A c c i d e n t /Incident Investi gati on
Re port and conduct a thorough investigation including all details surrounding the
accident. Included in the report will be recommendations to prevent a recurrence.
 The supervisor must initiate a JTC Accident/Incident report. The Head Office will
make sure that the Form is completed.

d) Evacuation Process
1. What People / Staff Should Do If They Discover a Fire?
2. Raise the alarm by operating the air horn/megaphone
3. Evacuate to a safe place at designated assemble point near to Security guard house
4. Trained personnel to tackle the fire only where appropriate.
5. Where appropriate check toilets and close windows and doors on the way out.
6. Leave the project site by the nearest exit.
7. Do not stop or return to collect personal belongings.
8. Ensure visitors are escorted from the temporary assembly point.
9. You must remain at the assembly place.
10. Return to the project site only when authorized to do so.

JTC shall notify LREDC’s/Qetaifan/ATKINS management of the said fire or fire incident, either
directly in person or by telephone communication on Emergency Telephone Number. Measures
to be taken in the event of an accident or occurrence:

In the event of an accident, the following action shall be taken: Whoever first notices that an
accident has occurred shall immediately notify the qualified first aider, who will attend to the
injured person, render first aid treatment (if trained to do so) and notify JTC’s representative of
the accident / incident.

11. JTC site management shall immediately notify LREDC’s/Qetaifan/ATKINS Management


of any accident / incident or emergency on site on the above number.
12. JTC representative shall determine whether the area of the accident remains
hazardous, and if so, take immediate steps to evacuate all personnel and remove the
injured person to a safe place.
13. The JTC first aider & HSE manager shall immediately stop all works in the area until
such times as adequate investigations have been concluded.
14. The JTC first aider shall assess the extent of injury. If the individual is not able to stand,
walk, move unaided, or is unconscious, no attempt shall be made to relocate the
individual without first getting medical attention / advice.
15. If the injured person is unconscious, the JTC first aider shall check for signs of breathing
or a pulse. Should the victim show no signs of breathing, or if no pulse can be traced,
the JTC first aider shall immediately call the Lusail Command and Control Center and
Qatar Emergency Medical Unit for an ambulance or for any other alternative transport
to attend site.
16. Upon arrival to site, the Qatar Emergency Medical Unit shall apply medical assistance to
the injured person and determine the seriousness of the injury, they will advise JTC site
management and the injured person if further treatment is required.
17. Tools or other construction equipment which may have contributed to the accident
shall not be moved unless there exists a situation of imminent personnel injury or
equipment damage.
18. JTC’s responsible supervisor shall prepare and submit to client a preliminary report,
within ONE hour of the accident occurrence.

e) Work over/Near water rescue

1. Rescue/evacuation teams (including first aiders) of suitable capacity should be organized


to deal with emergency situations.
2. The occurrence of an emergency situation should be informed immediately to the
rescue team for immediate launching of appropriate rescue procedures.
3. A lifebuoy with a buoyant safety line of 30 meters long should be available for use, and
the locations of lifebuoys should be at less than 50-metre intervals along the edges of
places, in vicinity for access aboard vessels.
4. To avoid any delays to rescue operations, lifebuoys should not be tightly tied to posts.
5. Serious emergency situations should be reported immediately to the public emergency
authorities, i.e. Fire Services Department and/or Police, for assistance.
6. Rescue facilities, including sufficient stretcher(s), portable resuscitation equipment and
first aid facilities, should be provided and kept readily accessible for emergency use.

Emergency procedures, including rescue/evacuation procedures, should be formulated and


reviewed regularly in the safety plan for, but not limited to, adverse weather (typhoon,
thunderstorm, heavy rainstorm, etc.), fire, injuries of workers, etc. An emergency contact list
(internal and external) should be displayed on board.

JTC shall implement suitable procedures to cater for emergency situations, including rescue
operations. The foreseeable emergencies would include, but not limited to: - Fire explosion,
collapse of ground conditions, collapse of structure or plant, etc.

In case of any emergency, the Qetaifan Projects/ATKINS Representative/ Site H & S Manager will
be immediately informed by radio or telephone. The Qetaifan Projects/ATKINS Representative/
Site H & S Manager/ Site H & S Officer Name and phone number shall be clearly posted on the
Site Notice Boards.

JTC H & S Manager, when informed of an emergency, will proceed to the scene and supervise the
workforce to take appropriate action to avoid further damage. The H & S Manager should
prepare the scene for arrival of the emergency services that can then commence rescue
operations as soon as possible.

The appropriate action taken may be limited to ensuring that all persons are cleared from the
area of danger and kept at a safe distance. It is not expected that the H & S Manager or any
other person put themselves or other persons at risk to contain the emergency situation.
It is envisaged that the major rescue and emergency force will be the local emergency services.
The JTC Emergency Response Team will act as support to the services by providing information,
any plant or equipment and render any assistance as requested by them.

In the case of any emergency in the work area, the work will be suspended. The Site H & S
Manager or Officer will check that equipment has been made safe and ensure that unauthorized
persons move out of the area.

Before returning to work after an emergency, the Project Manager shall direct all managers and
line supervisors to check damage and loss facilities, and also take measures to rectify any unsafe

conditions.

JTC shall notify Client/Consultant Representative of the said incident, either directly in person or
by telephone communication on Emergency Telephone Number of Client/Consultant
Management.

f) Excavation - The following procedure applies to normal working conditions. In the event of
an emergency, anyone in the excavation who is physically able to must exit immediately,
providing assistance to others only when not endangering his or her own safety. If the
emergency is life-threatening, call 999 and provide accurate detail (see permit if needed).
Any rescue action that can be performed safely from outside the excavation, such as hoisting
a harnessed victim, may be undertaken while waiting for rescue personnel.
1. Establish an outer perimeter circle (Cold Zone):
 Eliminate sources of vibration out to 500 feet from the collapse site.
o Stop and shut off all construction equipment.
o Halt any drilling and/or blasting operations being conducted in the area.
o Stop and/or re-route traffic.
 Identify and communicate with witnesses to the incident.
 Contact the job foreman and engineer if on site, or obtain the phone number of the
engineer in charge.
 Determine availability, quantity and size of onsite shoring material.
 Determine the time when the accident occurred
2. Establish an inner perimeter circle (Hot Zone):
 Establishing an incident inner perimeter circle, a minimum of 100 feet around the
trench collapse site. Complete outlining of the inner perimeter circle with fire-line
tape and establish an entry/exit point into the collapse site.
 Approach site from the narrow end of the extrication.
 Conduct a visual inspection of the trench walls to determine type of soil involved in
the trench collapse.
3. Attempt to identify the victim(s) location.
 Determine if available onsite shoring material is of sufficient quantity and size.
 Identify the number of victims.
 Establish the victim(s) condition if possible. (Do not enter any trench over 4 feet deep
without adequate shoring for any reason.)
4. How is the victim trapped? (Rescue vs. Body Recovery)
 Totally buried.
 Depth buried.
 Trapped by utilities, if so notify proper utilities.
 Establish and maintain contact with victim(s) if not totally buried.
5. Incident Control:
 Immediately get non-entrapped personnel and workers out of the trench.
o Place a ladder in the trench at the short end to assist non-injured victims out of the
trench.
 Confirm the notification of proper utilities.
 Begin clearing and levelling of the site for ground pads (4 X 8 sheets of plywood)
utilizing:

o Fire department rescue personnel (leaving apparatus in staging area).


o Construction personnel can assist at ground level gathering lumber, hand digging,
moving spoil piles, and moving excess construction materials away from emergency
site, etc.
 Ground pads (4 X 8 sheets of plywood) need to be installed ahead of the work areas.
Personnel shall not walk around the trench site without the ground pads in place.
 The location of any tension cracks shall be immediately transmitted to the Site HSE
Manager and the safety officer.

g) Heat Stress

AL Jaber Trading & Contracting (JTC) shall manage heat stress and heat stress emergencies by
implementing Heat Stress Management Program agreed and approved by the Client whenever
workers are likely to be exposed to hot working environment and during summer heat period
cleared by Qatar Government as per Draft Law from Council of Ministers Regular Meeting (40) of
2020 held on 21/10/2020 whereas:
Decision of the Minister of Administrative Development, Labor and Social Affairs No. 17 for the

h) Precautionary Actions for Heat Stress

The following checks shall be carried out whenever there is possibility of exposure to heat stress:
 Heat Stress monitoring regime is in place and status updated to all on site using site
procedures
 Suitable work/rest rate regime in place
 Adequate supplies of water and electrolyte freely available and regularly replenished (with
ice)
i) Working at Height Rescue

 Only competent rescuer and authorized rescuer shall respond for emergency rescue
operations.
 Competent rescuer shall avoid prolonged suspension trauma on the victim by promptly
responding using the Rescue kit, or equivalent for rescue working at height.
 Suitable emergency equipment’s such as Rescue basket, man lift, man basket with mobile crane
will be used for rescue in the event of emergency.

 All Emergency equipment will be inspected by competent person by every month to ensure
the integrity of the equipment.
 The rescue should be complete as soon as possible to protect the victim from suspension
trauma.
 Every employee potentially exposed to a fall should be trained and made aware of the rescue
plan.
 Avoid hyperextension by trying to prevent the head from being hyper extended (down toward
to feet). This prevents impinging vessels in the neck, ensuring adequate blood flow to the brain.
 Once rescued, the conscious/unconscious victim should never be fully reclined to horizontal
under way circumstances.
 Whether resting the rescued worker on the ground, try to keep the victim’s upper spine vertical
or no more than 60 degrees from horizontal.
 Keep the victim warn either a crouch or sitting legs-out for at least 45 to 60 minutes.
 Lower the victim’s spine angle by degrees over a long period of time.
 Horizontal positions should never have attempted on a suspension – traumatized victim’s until
they have reached a well – equipped hospital and placed under the diagnostic care of a
physician.
 Fully inform the medical team of the time this worker was suspended as well as their initials
symptoms.
 Rescuer must be trained by 3rd party training center specialized in working at height rescue

 All applicable government requirements and regulations pertaining to environmental


protection and pollution shall be adhered.
 To avoid damage to the water table or sewage systems, all areas contaminated by spillages will
be cleaned up immediately; waste will be disposed of at designated waste areas.
 Avoid eye and skin contact.
 Avoid contamination of waterways.
 Good housekeeping should be followed at all times to prevent occurrence of accidents and
consequent injuries.
 MSDS of cement will be displayed near storage.
 Empty cement bag will be dealt as a hazardous waste.
 Material will be stored inside shaded area.
 Fire extinguishers will be available near the working areas.

7.2. Material Inspection

 All materials for the installation of FM-200 System delivered to the site shall be Stored as per
manufacturer recommendations & shall be transferred from storage area to workplace either
mechanically or manually according to the quantity & the weight of the material & inspected
properly for approval of material and possible damages/defects.
 Designated storeroom shall be built specifically to handle appropriate storage of materials, and
will be situated in an accessible location.
 Material inspection request shall be issued for Consultant approval.
 Work shall be carried out on site by staff under the strict supervision and guidance of the
concerned Supervisors/Engineers.
 Panels shall be properly packed and mounted in a pallet or skid to facilitate proper handling.
 Panels shall be delivered and wrapped by cartoon boxes shall be opened and checked by the
site engineer and QAQC engineer.
 Handles shall be protected carefully to prevent breakage.
 Rejected materials during inspection shall be stored in a separate place marked as “Rejected
Material” and shall be removed from the site / yard later.

7.3. Site Planning & Preparation

 Ensure that the shop drawing used are approved and latest before commencing works on site.
 Ensure that all materials to be used are as per the approved material submittal.
 All works shall follow the approved shop drawings and shall comply with project specification,
QCS and QCDD (Civil Defense Department) requirements.
 Suitable type of scaffolding or A type ladders shall be checked & inspected for safety assurance
before using, A type ladders shall be used only for short activity working on heights, otherwise
scaffolding is recommended.
 Evaluate and ensure that the operatives, technician, electricians & specialist involved in the
activities are skilled and competent for the job.
 Ensure that all personnel involved in the activity shall be provided with the complete and
appropriate PPE.
 Before commencing the installation of FM-200 System the following checks should be carried
out.
(a) Installation procedure shall be as per manufacturer recommendations.
(b) Confirm material submittals are approved by consultant.
(c) Confirm all approved materials are all available for installation.
(d) Confirm that the drawings are approved for construction and latest revision.
(e) Confirm site bench marks.
(f) Ensure that work area is clean and safe for work with adequate lighting provided.
(g) Ensure all site personnel have undergone site safety induction.
 Method Statement and Risk assessment must be approved and properly communicated to
operatives involved.
 Site equipment’s, lifting and transportation equipment and tools to be use shall be properly
inspected and get approval from the contractor HSE department.
 Contractor to confirmed and ensure that the mobile scaffolding is relevant and applicable for
the activity all throughout the completion of the task.

7.4. Material Handling/Storage

1- Carefully transport the materials to the installation location by proper tools /equipment
considering the location of installation.
2- Equipment to be used for handling the materials will be inspected and will be in good
condition.
3- Delivered materials will be covered with polyethylene sheets while storage to ensure free from
dirt and dust.
4- When off-loading, materials will be lowered and not dropped to the ground.
5- All materials delivered on-site will have relevant supporting documents complying with the
specification of the project.
6- Visually inspect the panels for physical damage.
7- No materials will be permitted to enter the work location without prior inspection and
approval.

7.5. Work Methodology

7.5.1. System Wiring / Cable Pulling


 Survey and check the actual condition of the location area before starting the job, to ensure no
obstructions.
 Approved Cables shall be installed according to approved shop drawings / manufacturer’s
instruction complying with project specification.
 Pull conductors (Signal Circuits) into raceways / GI conduits between FM-200 system panel
sounders & strobes, from any point to the next point if conduit between is ready and as per
approved shop drawings.
 Pulling compound / lubricant shall be used wherever necessary.
 Compatible back box shall be used. Weatherproof back box shall be used for external devices.
 Connecting outlets and fm-200 system devices to wiring as per manufacturer recommendations
with tightening connectors and terminal including screws and bolts.
 Conductor Size: Wire size shall be sufficient to prevent voltage drop problems. Sizing of
conductors shall be in accordance with the manufacturers wiring specifications.
 The required personnel protective equipment will be issued to all the operative staffs prior to
start of the fabrication works.
 All the material delivers to the site should be visually inspected and checked by consultant prior
to installation.
 Minor deviation from the approved construction drawing and M&E drawing would be
determined on site with the consent of consultant site staff.
 The scaffolding/ladder for the erection work should be arranged on the site and verified by
safety officer.
 Cabling routing shall follow as per the approved shop drawing.
 Once completed the cable pulling, start checking of cable continuity and make sure the cable
conductors are free from earth connection and any damages.
 Mark the device/panels’ location and leave enough length of cable at both ends for termination
 Device installation & termination shall be as per manufacturer recommendation.

7.5.2. System Devices Installation


8.5.2.1. FM-200 System Control Panels.

• Secure approval of required materials and shop drawing.


• Inspect area if ready for installation
• Review approved shop drawing intended for the area.
• Pull-in the approved pulling wire on all the pipe layout in particular area.
• Install the required number of cables on the conduit based on the approved wiring lay-out.
• Verify the installed cables are free from damage.
• Verify that the installations are proper and in accordance with approved drawing and as per method
statement.

8.5.2.2. Smoke Detector Installation

• Installation of smoke detector shall be in accordance with the NFPA 72 at location shown in the
approved shop drawings.
• Identify the location of the device based on approved shop drawings
• If the location is within the specification, mark the surface and install the device to the surface.
• Ensure that wires to be terminated have been tested and passed the cable integrity tests prior to
start of any termination.
• Visually check every detail connection and its mounting as per approved drawing and layout.

8.5.2.3. HORN / STROBE:

• Installation of Horn Strobe shall be in accordance of the NFPA 72 at location shown in the approved
Shop drawings.
• Ensure/verify the Horn Strobe nameplate information match with the name and location as specified
in the Shop drawings.
• Identify the installation location of the notification device; use the portable drilling tool to make
holes, and install or fix the device at the location using the anchor bolts and nuts. Installation of the
device enclosure should be in accordance to the approved Shop drawings and diagram below. Fix
and tighten the device using the supplied expansion anchor bolts and nuts as per manufacturer’s
installation instruction.
• Apply labels and tags as per approved material.

8.5.2.4. Manual Call Points (Dual Action Type)


• Installation of Manual Release Station shall be in accordance of approved shop drawing.
• Check for the approved mounting detail and the location from the approved shop drawing.
• If the location is within the specification mark the surface and install to the surface.
• To be ensure that wires to be terminated have been tested and passed the cable integrity tests prior
to start of any termination.

• Screw the device completely and take the levelness by using the level bar.
• Never removed the plastic covering of the device until proper commissioning takes in.

8.5.2.5. System Installation


• Check that the material size and schedule is correct and approved as per specification and/or
approved alternative.
• Ensure that the pipes and fittings must conform to the requirements as outlined in NFPA 2001 in
strict accordance to systems approved plans.
• All pipe thread connections must be free of scale and burrs before installation.
• Check to ensure that all grooved coupling gaskets shall be lubricated as per manufacturer’s
specification.
• Pipe thread sealants as per approved shall be applied to the threads to ensure leak-tight seal.
• Ensure that the piping system should be securely supported with due allowance against agent
against thrust forces, thermal expansion and contraction.
• Pipe hangers shall be spaced at intervals not exceeding 4.5 meters.
• Rigid bracing shall be installed at each directional change, fitting, tee and nozzle.
• Check to ensure that the nozzle identification number is verified before installation and each nozzle
should be within 24mm to 300mm of their intended location on system plans.
• Check to ensure that piping should be blown clear to remove chips, mill scale or metal shaving
before the nozzle should be installed.
• Make certain that each container has been installed in the correct mounting position and shall be
located in clean, dry and relatively vibration-free area.
• Check to ensure that the correct container part number is identified in the name plate and correct
agent capacity should be verified as per location on the system plan.
• Make sure that the container brackets must be mounted securely to solid bearing surface that will
support the container load.
• All conduit and hardware, initiating and signaling devices, and other related electrical connections
shall be done as mentioned in the specification with the approved materials and manufacturer in
accordance with the minimum requirements in NFPA 72 and/or as per civil defense department
regulation.
• Verify that all auxiliary functions such as door closures, damper closures, air handling shutdown, etc.
Are connected as specified.
• On completion of the works, all elements of the installation shall be checked to ensure the
installation is following the specification and manufacturer instructions.
• All pipes and equipment installation works shall be monitored on an on-going basis to ensure
compliance with the quality requirements of the project.
• System shall be identified in compliance with the specifications, identifying all equipment, flow
direction and services.

8.5.2.6. Threaded Connections


• Confirm the valve size and location based on the shop drawings. Ensure that the selected valve is
screw type.
• Fabricate the pipe according to the required length and thread it accordingly.
• Place the valve between the pipes and make sure the threads are free from dust.
• Guide one of the pipes ends onto the valve’s thread, slowly run the pipe in clockwise direction.
• Hold the valve firmly by a spanner on the flat nearest joint and then rotate the pipe with a plumbing
wrench. Rotate the pipe until the end of the spindle operating joint. Repeat the above steps for
other pipes.

8.5.2.7 Erection of Pipes

• Complete the painting of pipes based on the approved submittal to be use for painting.
• Start the sample installation and get it approved.
• Start marking the routing of the piping as per the approved and coordinated drawings.
• Start the fabrication of pipes (grooving for sizes 2 ½” ø above and threading for 2” and below)
• Start drilling and fixing of supports as per the prescribed marking.
• Start hanging the pipes with proper hanger and supports.
• Join all the fabricated pipes and prepare it for pressure testing.
• Pressurize the piping as per testing procedure.
• After completing the pressure testing, start fixing the equipment’s and do the final touch-up of
painting

8.5.2.8 Installation of Clean Agent Containers

• Installation of clean agent containers shall be in accordance with the manufacturer’s installation
instructions.
• Ensure and verify the panel’s nameplate information match with the name and location as specified
in the shop drawing.
• Identify the location of the container.
• Containers should be located in clean, dry, and relatively vibration-free areas. Avoid aisle ways and
other high traffic areas where physical damage or tampering is more likely. Containers should never
be mounted where the container could potentially be splashed with, or submerged in any liquid.
• Container brackets must be mounted securely to solid load-bearing surfaces that will support the
container load.

7.6. Inspection & Testing Plan

 Before commencing the work, the whole site shall be inspected to insure that there are no
obstructions to the work.
 All inspection and Testing shall be inspected by qualified and approved engineer/Inspector as
per approved ITP.

7.7. Quality Control

 JEM has developed the implementation of all its activities, a quality manual including quality
policy, quality assurance procedures, forms, and tests report in order to ensure that all works
are in accordance with the required relevant standards, specification, and tests procedures.
 The quality assurance is based on quality planning for the operation and in process inspection
and testing.
 The use of correct tools/equipment in suitable environments is an integral part of the quality
assurance plan that will be implemented. Check lists are prepared for all operations.
 All field works and laboratory testing will be carried out by qualified staff and technicians and
under the supervision of qualified and experienced engineers.
 Each report is issued by the project engineer is checked by the branch-manager for the
accuracy and completion of all date and results and a final inspection form is issued. Time is
allocated for this checking.
 A quality manager is responsible for the implementation of the quality plan. The overall
supervision and direction of the work will be carried out by the project manager.
