ABC Digital Registration &
Entitlement Allocation Form
ABC Digital provides online banking services such as manage accounts, view account transactions, view or print transaction details, make funds transfer,
manage payees, view status of loans and trade finance accounts and request statements. This form must be completed in order to grant the company and
its designated users access to ABC Digital. The completed form is to be mailed to the following address:
BANK ABC New York Branch, 140 E 45th Street, 38th Floor, New York, NY 10017, U.S.A.
Once the request is approved by Bank ABC, an email will be sent to the user(s). Each user must follow the steps explained in the email to activate access
to ABC Digital.
Please complete the form in BLOCK CAPITAL LETTERS and tick where appropriate.

has sufficient authority and is authorized to use ABC Digital service at any time and from time to time for and on behalf of the Company in relation to the
designated accounts.
