This transcript request form allows students to request official transcripts from Niagara College. Students must provide their name, student ID number if known, contact information, program details, and the name and email of the institution where they want the transcript sent. Transcripts can be requested now or after final grades are posted. Due to COVID, transcripts cannot be mailed or picked up on campus and must be sent electronically. The student signature authorizes Niagara College to send the official transcript to the requested institution.
