INTRODUCTION

NBCP 3812 Clinical Practice Rehabilitation Geriatric and Palliative Care is an Elective
Practicum focusing on clinical Rehabilitation Geriatric and Palliative Care that is
implemented in the student’s own ward or area of practice. You will continue to work in
your organization and your official working hours will be considered as practicum hours to
fulfill the required 2 credits. You have to record your attendance and provide evidence of
presence at the work place for you to gather information and data that supports your writing
of Reflective Journal. Your learning will take place as you work because you need to apply
the theory that you have learned into your work tasks. Thus, you may find it beneficial to
revisit and review the learning kit NBMH 3812 Practice Rehabilitation Geriatric and
Palliative Care to guide you in writing your Reflective Journal.

Additionally, you are encouraged to apply all the knowledge and skills gained from your
current learning, particularly relating to NBMH 3812 Practice Rehabilitation Geriatric and
Palliative Care

SYNOPSIS

This is the elective practicum component and prepares the students to integrate, organize
and apply knowledge and skills related to the rehabilitation geriatric and palliative care in
delivery of holistic care to patients in their workplace. The Reflective Journal expected at the
end of the placement is evidenced of your progressive application of knowledge achieved in
communication, safe care and critical thinking.

COURSE OBJECTIVES

At the end of this course students will be able to apply their knowledge and skills in upholding
the quality of care in rehabilitation geriatric and palliative of Assistant Medical Officers
profession at their workplace.

LEARNING OUTCOMES

At the end of this course, students will be able to:

1. Analyse evidence-based knowledge, use patient care technologies, and practice


reflective analysis in planning, implementing, and evaluating outcomes of care (C4,
PLO 2)

2. Document patient record systematically and manage time effectively for assessment,
treatment and documentation (P3, PLO 10)

Demonstrate a caring attitude, teamwork, and professionalism and effectively


communicate with patients, carers, and colleagues. (A3, PLO 5)

CLINICAL PLACEMENT / PRACTICUM HOURS

 You need to be SELF-DIRECTED in implementing your learning activities.

 There is NO Local Preceptor to supervise and guide you.

 This practicum takes place in your respective area of practice, and your official working
hours are considered as the practicum hour.

 You must carry on with your normal tasks and responsibilities at your respective working
place.

 In order to achieve the objectives of this clinical component, you are expected to apply
or practice all skills that you have learned from the specific practicum wherever possible.

 You should be able to organized your activities and perform your roles effectively in
areas relating to patient management, assessment, communication, and clinical decision
making.

 By the end of the day, you MUST record your attendance as an evidence for your clinical
experience in general practicum. Make sure that you have achieved a minimum of 80
hours. Please refer to Table 1.

3812 Clinical Practice Rehabilitation Geriatric and Palliative Care. Please refer to the
detail information and instructions in the respective sections.

1. REFLECTIVE JOURNAL – 50%

A reflective journal often called a learning journal is a steadily growing document that
you (student) personally write, to record your learning experiences and the progress of
your learning. It is a space where students can record and reflect upon their
observations and responses to situations, which can then be used to explore and
analyze ways of thinking.

The ability to reflect on one’s learning is a fundamental and vital skill for lifelong
learning. Therefore, developing students’ capacity to engage in reflective practices has
been recognized as an essential goal in higher education and especially for preparing
students effectively for their personal and professional development. (Alt &
Raichel,2021)

It is designed to help you think deeply about your learning, especially on issues such
as: your progress in learning, the difficulties you encountered in the process of
learning, the strategies you have taken to get around those difficulties, and your
evaluation of your own performance.

Therefore, the task of submitting reflective journal writing is given to students because
of the established link between reflection and deeper learning. As well as facilitating
learning, the intention of the task of reflective journal writing is for students to acquire a
habit of reflection, achieve high levels of self-awareness and grow in the profession of
nursing.

Requirement:

You are required to write a Reflective Journal to reflect and record your learning
experiences during the tenure of this practicum based on your area of practice as
stipulated in the suggested learning activities that you think significant to your learning
and professional development. (You are required to select any of your learning

experience at your own clinical area related to your current clinical


specialisation).
Reflective Journal Writing Guidelines
The following may provide you with a framework upon which to build each journal
entry.  You are not required to set up your journal entries this way; however, it may
be helpful if you are feeling a little tangled or stumped

Item 1: Title:
"What are the most significant contemporary issues in rehabilitation, geriatric,
and palliative care, and how can they be addressed to improve the quality of
care for older adults with complex medical needs in your community?”

Item 2: Introduction
Brief introduction: 3 – 4 paragraphs that introduce the particular situation from
your clinical experience that you will be writing about.  The introduction should
set the context for the reflective entry.  The introduction must also include
other key concepts that will be discussed in the entry.

Item 3: Trigger Event


The trigger event is a positive or negative impression evoked by physiological
and psychological safety, behaviours, ideas related to health care
management, that intrigue you. This section should be 3 - 4 sentences that tell
the reader why you have chosen to write about this particular situation or
about an incident from your clinical experience.

Item 4: Appraisal
This section offers you the opportunity to identify and clarify a concern and
engage in self-examination.  Perhaps begin with those who face a similar
contradiction.

This section needs to be 1 – 2 paragraphs in length.  In this section, you return
to the situation and ask: “What is generally known about this issue?”, “What
does the literature say about it?”  There is a requirement to use the literature
(scholarly writing in textbooks and professional journals) to support your
thoughts and ideas.

Item 5: Exploration
This section provides you with the opportunity to take the time and begin
searching for new ways of explaining discrepancies in your understanding or
new ways to live with them.  Test options and search for new meanings –
develop alternative perspectives: come to a way of thinking and acting that
you feel make sense of a situation – it is a transition stage.

Leaving behind a familiar but inappropriate assumption can be a wrenching


experience.  A common tendency is to hand onto the assumption or
behaviour. Try to modify it to fit the situation more closely.

Include 2 - 3 paragraphs describing how this process has affected your


understanding, your sense-making of what you wrote in the initial paragraphs.
Are there other ways to look at the situation?  What did you read in the
literature that led you to think differently about the experience?

Item 6: Integration
This section should be 1 – 2 paragraphs in length.  You may begin by asking
yourself, “Have I learned something that I think will inform / further my
professional development? Something I had not thought of before?”  Line what
you knew before to what you know now and to how this might affect your
thinking and/or your responses (language and/or actions) in the future

Item 7: Conclusion
The conclusion should be concisely and precisely written that builds new
knowledge and draws out personal learning and professional development.

Should be in the APA Version 7.0 format

2. CLINICAL ACTIVITIES - 40%

This is SELF-DIRECTED learning. Hence, you are required to plan your learning
activities to fulfill the objectives of your general practicum.

Below is a list of suggested Learning Activities which you should perform at least
FIVE of them and document / summarize as per format provided. The summary of
each activity should be written in not less than one page (approximately 200 words;
with 1.5 spacing); and MUST be verified by your Superior.

You may add any other relevant learning activities that positively ameliorate your
learning and professional development.

2.1 Rehabilitation Geriatric and Palliative Care Management:

Perform clinical care in rehabilitation geriatric or palliative care in your setting.

2.2 Communication Skills:

Apply effective communication skills with patient, team members and family using
standard practices.

2.3 Team Building / Collaboration:

Participate with team members and/or collaborate with other healthcare


professional in delivery of care / services.

2.4 Psychosocial Care:

Deliver independently and/or participate with team members in provision of


psychosocial care for patients and healthcare professional.

in geriatric and palliative care.

2.6 Clinical Decision Making:

Play a substantial role in clinical decision making to achieve positive safety and
health outcomes.
