The researcher is a student of the aforementioned school and is conducting a research survey on
“Influence of Teachers Attitude on Instructional Effectiveness” in partial fulfillment of his
B. Ed Social Sciences program.
Any information provided will be used solely for the purpose of the study. Your anonymity and
confidentiality are fully assured, therefore do not indicate your name or identity on this
questionnaire. This may take between 15 to 25 minutes of your time. For the questions, you are

could assist in policy formation
