DATE: NOVEMBER 23, 2023
1. On or about November 23, 2023 around 9:00 am in the morning a car

incident transpired in front of Sotaridona resident in Tupi Poblacion South
Cotabato. The victim named Juan Dela Cruz 25 years old married and the
resident of Polomolok South Cotabato.
2. The investigation related to the car of the victim. When unidentified car
suddenly crushed on the victim car with unknown plate. Then the running
suspect was not identified on the scene.
3. The victim Juan Dela Cruz who sustained a multiple fractured bone. And
rushed to the nearest hospital but declared dead on arrival of DOH
4. The motive on the incident still unknown.

1. REFERENCE:
A. Incident record form Blotter Entry No. 001 with the subject conduct
dated November 28, 2023
B. Memorandum from TMPS with the subject spot report Re: car incident
dated November 28, 2023.
2. In connection with the above reference. Please be informed the station of

Tupi Municipal Police Station requested the patrol man to locate the car on the
incident. And coordinate to the nearest owner of the CCTV to check the
possible footage of the crime scene. And copy the footage to send the crime
investigation.
3. Possible motive of the incident.

SUBJECT: FINAL REPORT RE: CAR INCIDENT
1. Reference:
A. Incident record form with the SPOT REPORT AND BLOTTER ENTRY
001 Dated November 28, 2023.
B. Spot report received on email 3:00PM of December 28, 2023.
2. On or about November 28, 2023 around 7:00AM a car incident, transpired

in front of Sotaridona resident of Poblacion Tupi South Cotabato. Wherein the
victim named Juan Dela Cruz 25 years old of age married resident of
Polomolok South Cotabato. Was crushed by unidentified car and the running
suspect named Jeff Morales 30 years old of age smarried at the resident of
Tupi South Cotabato. At the result the victim sustained of multiple fractured
bones and rushed to the nearest hospital for medical treatment but declared
dead on arrival of DOH therefor the suspect was arrested on their house
during the operation and proven by guilty that his car was the one who crushed
the victim.
3. The motive of the incident was revenge for his wife.
4. Case is solved.

3. Suspect: Jeff Morales 30 years old of age married and the resident of
Polomolok South Cotabato.
STATUS: ARRESTED
4. On the for mentioned date, time, place elements of this station led by PO1
JHON KEVIN BACULBAS, this station able to arrest by virtue of warrant arrest
(WOA) issued by the judge of the court.
5. Remarks
To be presented to the court of origin.

1. Reference:
A. Standard Blotter Entry 001 and
B. Standard Operating Procedure
2. Above the reference pertains to an alleged of drug raid, transpired around

9:00AM on November 28, 2023. in Sotaridona Resident of Poblacion Tupi
South Cotabato. Wherein Juan dela cruz 25 years old of age married resident
of Polomolok South Cotabato. Buy the drugs on Jeff Morales 30 years old of
age married resident of Tupi South Cotabato.
3. Investigation that was conducted by his office that according to Lufel Peligro
27 years old neighbor of Juan dela cruz. During to his interview at the station.
While he was walking outside he saw jeff morales was selling the drugs to
Juan dela cruz inside the house of juan dela cruz.
4. Based on the affidavit of the witness at the responding personnel led by

PO1 JHON KEVIN BACULBAS from Tupi Municipal Police Station with the
Chief of police Edwin Parreno immediately respond and arrest the suspect
when the witness reported.
5. Other development regarding the incident shall be submitted as soon as

possible.
