    Introduction

    The following review questionnaire will be used to evaluate the [insert project
    name] project, during our forthcoming debrief meeting.

    The aim of this meeting is to establish if the project met its aims and
    deliverables, and to discuss the effectiveness of the processes and procedures
    employed.

    Completing the Questionnaire

    The questionnaire is split into ‘group’ and ‘individual’ sections. The group sections
    are dedicated to specific phases/elements of the project and we will talk through
    these together, with a designated participant taking notes.

    Please take a few minutes to complete the ‘individual’ section (pg. 4–5) and bring
    to the debrief meeting. When answering the questions, please consider the
    following:

     Was the identification and management of risks satisfactory?


     Were estimations for the duration of the deliverables accurate?
     How satisfied are you with the project’s outputs/deliverables?
     Were the best processes/procedures used? How could they be improved?

     Are there any outstanding issues (e.g. training needs, activities still to be
    completed)?
    Please be as candid as you feel is appropriate. Constructive comments help us to
    improve our processes and procedures, and will benefit future projects.
