I am certified with NVQ level 6, NEBOSH IGC, IOSH managing Safely and OSHAS 36 hours, I started my
profession in 2005 with Julius Berger Nigeria PLC back in Nigerin – west road of 38km road with 15 over
head bridges and 10 under passes.
1, What are you company's HSE Objectives. How would you measure it: Protect personnel
from any health hazards that may be associated with their work, Provide accident-free
Environment to Marbu and sub contractors' employees? Maintain compliance with all the
applicable HSE Legal/Regulatory requirements in Doha, Improve HSE culture among Marbu
and sub contractors' employees.
2, How would you measure KPI: Is tracking or measuring of safety performance. Leading Indicator:
are program/HSE activities to prevent workplace injuries and illnesses. Number of training hours,
Inspection, Audit, Survey, toolbox talk, hazards identified, equipment breakdown, Employee Perception of
Management Commitment. Number of Reported Accidents & Incidents – lagging indicators measure
the occurrence and frequency of events that occurred in the past, such as the
number or rate of injuries, illnesses, lost workdays, fatalities and worker’s
compensation cost. The most difficult way to keep track of these numbers is to 'assemble' them
at the end of the year or reporting period after capturing them with paper-based forms, word
docs, spreadsheets and PDFs. The easiest way to track these safety KPI's over time is to use a
proper safety management system. Dashboards or automated charts. A good safety and
health program uses leading indicators to drive change and lagging indicators to
measure effectiveness.
How can you improve your safety culture: Adequate communication system in the workplace
in all changes in the site, communicate lesson learned, promote safety alert, Training of workers

First question covered cylinder storage and transportation: Cylinders will not be transported with
regulators, gauges and hoses attached, transported in an upright position and will not be dragged in
equipment beds or truck beds on their side & stored in a clearly identified, dry, well-ventilated storage
area that is not exposed to heat or the direct rays of the sun. both empty and full, in the upright position
and secure with an insulated chain or non-conductive belt. During storage, close the cylinder valves with
the protective caps in place. To close the cylinder valves, remove the regulator and replace the valve
protection cap and hand tight before moving a cylinder with appropriate trolley. Maintain register of all
hazardous substance, obtain MSDS, health risk assessment.
Cylinders lifted from one elevation to another will be lifted only in racks or containers designed for that
purpose. Cylinders will not be hoisted by the valve cap or by means of magnets or slings. Cylinders will
not be used as rollers; will be transport on appropriate trolley & kept free of oil and grease. Type of gas
clearly marked on cylinders and segregation of incompatible gases is required.
Cylinders will be secured in place during use and storage. Securing shall be around the body of the
cylinder, securing around the cylinder neck or cap will be prohibited. All hose, valve and regulator

assemblies will incorporate a flashback arrestor. Fuel gas and oxygen hoses will be of different colours
and will not be interchanged. Fuel gases will only be used for the purposes of cutting or heating. They will
never be used for pressure testing or ventilation.
Train people that use, handle or transport hazardous substances in the associated risks and control
measures. Provide appropriate spill containment and disposal Spill kit. A fire watchman will remain at the

Lifting gear:
A wire rope used in raising, lowering or suspension of a load, will not be used if it is kinked, significantly
rusted, the core is visible or the rope has visible broken wires, exceeding 5% of the total or in any length
equal to 10 times the diameter of the rope. All lifting gear will be subjected to a thorough examination by a
competent person from 3rd party at intervals not exceeding 6 months. Identity number and SWL to be
check. All riggers will be required to wear a blue construction safety helmet and a red high visibility vest
for the purpose of easy identification on the Worksite

Heat Disorders & Health Effects: Heat fatigue is a state of discomfort from prolonged heat exposure. It is
generally caused by fluid loss. Heat rash is “prickly” Prickly heat occurs on skin that is persistently wetted
by unevaporated sweat, and heat rash papules may become infected if they are not treated. Heat
cramps are usually caused by performing hard physical labor in a hot environment. Cramps can be
caused by both too much and too little salt. Cramps appear to be caused by the lack of water
replenishment. Heat collapse is a condition where the brain does not receive enough oxygen because blood pools
in the extremities, resulting in a loss of consciousness (fainting or syncope). Heat exhaustion is a condition with
symptoms of headache, nausea, vertigo, weakness, thirst, and giddiness. Heat stroke is a condition when the
body’s system of temperature regulation fails and body temperature rises to critical levels. Engineering Controls:

(21) What is AFR (Accident Frequency Rate) The frequency rate is the number of
occupational accidents (work stopped more than one day) arisen during a period of 12 months
by one million hours worked Number of reportable accidents in a specific period x 1,000,000 /
number of hours worked in that period. LTIFR = Number of lost time injuries in a reporting
period x 1,000,000) / (Total hours worked in the reporting period. the number of lost time injuries
