1. MONITORING AND EVALUATION OFFICER


Duty station: Moshi Kilimanjaro
Report to: Executive Director.

Functional responsibilities.
Monitoring and Evaluation (M&E) Officer will be responsible for the following duties and responsibilities:
Monitoring and evaluation.

 Support the development of a comprehensive M&E and reporting system that will enable the collection,
documentation, tracking, and reporting of data related to all results levels of the project: outcome, output,
and activity.

 Develop and/or revise the project M&E tools as well as data collection procedures (e.g. project performance
tracking and monitoring, indicators, data flow chart, and M&E guides).

 Support and guide the development of feasible (specific, observable, measurable, qualitative and
quantitative,) indicators, targets, data collection and reporting framework, and performance monitoring
plans for all project components.

 Support in compiling baseline data for each project component and for all project indicators.

 Systematize relevant quality dimensions into the project& M& E framework as well as activities.

 Cascade M&E framework to all result levels of the project ensuring coherence and integration of indicators
across all components of the project & results framework.

 Support the executive Director in reviewing work plans, reports, and deliverables produced in the lifespan
of the project.
Data management and reporting

 Support the development of an automated and interoperable information management system and
dashboard to enable the translation of raw data into an interactive visual platform to track real-time progress
and inform decision making.

 Regularly collect data on targets of each indicator to measure achievement against performance indicators
at all levels.

 Ensure validity, reliability, precision, integrity, and timeliness of information through quality checks and
implementation of corrective actions to improve data quality and accountability of project team including
technical assistants.

 Closely work with the program counterparts to develop an efficient and centralized database for compiling,
analyzing and reporting.

 Maintain and update the M&E/MIS platform to ensure that M&E data are readily available.

 Compile and analyze data and submit to ED monthly, quarterly, and annual reports as well as ad hoc
updates which provide comprehensive insight to the progress, constraints, gaps, and lessons of the project
both to internal and external stakeholders.

 Support the ED in preparing briefing materials on project performance and status, as required.

 Support compliance with reporting requirements, reviewing relevant project documents, work plans, M&E
frameworks and other relevant documents and providing input as needed.

Other

 Work in close collaboration with Health cum Clinical Officer and Advocacy Officer to ensure that project &
M&E framework is implemented and practiced according to organization standards.

 Other duties that may be assigned by the Executive Director within the scope of the role.

Education/Experience/Language requirements
A. Education

 Bachelor Degree in Public Health, Statistics, Health, M&E, Project Management, Community Economic
Development, Development Studies or other relevant disciplines.
B. Work Experience

 At least two (2) years’ work experience in the design, development and/or implementation of M&E systems,
in the health, social development sector.

 Experience in conducting baseline, Evaluation, reporting, and knowledge management is required.

 Experience in setting up and maintaining information and data management platforms is added advantage.

 Quantitative and qualitative data analysis skills.

 Experience using statistical packages such as SPSS, STATA is an added advantage.


C. Language

 Fluency in spoken, written and oral English is required.

2. Job Tittle: MEDICAL SOCIAL WORKER

Duty station: Moshi Kilimanjaro

Reports to: Health and Training Officer.

Duties and Responsibilities.

A Medical Social Worker at Faraja Forward will be responsible for the following duties;

 Conduct assessment and evaluating client’s social conditions to determine service required.
 Conduct complete case management of each child (client) case. Conduct monitoring and follow ups for
proper and quality service delivery.
 Ensure proper admission procedures of children into database and update accordingly.
 Guiding patients and families in understanding the illnesses, conditions, and treatment options
 Performing psychosocial interventions with individual children and families.
 Coordinate referral of client cases that might need specialized services to partner health care providers to
ensure continuity of care for children and their families.
 Educating families and community on issues of physical, mental, and emotional health
 Assisting patients with medications, medical devices, transportation, and advance directives.
 Support the Health and Training Officer in delivering health trainings to families of children with special
needs.
 Any other duties as may be assigned by supervisor.

Education/Experience/Language requirements

 Degree or advanced diploma in social work, Diakonia or its equivalent.


 At least 2years experience in clinical social work practice
 Passionate in working with children with special needs.
 Practical experience in community outreach work is required
 Knowledge in casement and psychosocial support and counselling is required.
 Good command in English and Kiswahili language.
