II. COMMUNITY PROFILE

1.DESCRIPTION OF THE COMMUNITY


The barangay Maddiangat is the
gateway of the municipality of Quezon,
Nueva Vizcaya. It is only 5 kilometers
away from the municipality of Solano
Nueva Vizcaya. This barangay is a
crisscross or several creeks which are
abundant of different kinds of marine
products, namely fishes. Folks here are
mainly composed of Ilocanos,
Pagasinense, Ifugao and Tagalog but
their dominant dialect used is Ilocano.
Their main source of income today is rice
field since they were provided water from
the Maddiangat Creek that could irrigate
90% of rice producers of the barangay.
The barangay was led by
barangay captain Danilo B. Cabading
and his 7 barangay councilors; Imelda B
Pinkihan, Ronnie L. Garcia, Rommel A.
Bisquera, Helen D Mahor, Roel B Mina,
Roger R. Tolentino. It has a total general
population 2,691 (source PSA) and total
number of households is 589, the total of
families are 695.
Their vision was “to be a prime
producer of Tilapia, Quarry Capital, a
progressive, well equipped citizenry,
God-fearing, enjoying quality Education,
complete health services, developed
agricultural facilities through a firm and
honest leadership” and mission is to
make “Leaders with sincere and
dedicated services, giving attention to
the demands and needs of the people,
seeking more resources improve
agricultural facilities and make use of our
natural resources for a more progressive
and competitive barangay.”
They offer various programs
such as Maternal Care, Child Care,
Adolescent Care and other health
services
Lastly, they also has two
amazing resorts which are the RVL
Resort located at Purok 5 and the
Kubkubo Resort wherein open for public
and tourist. They also has two beautiful
falls which are the Dayog and Mt.Pollal
that open to visit and experience the
welcoming view .
2.HISTORY OF THE BARANGAY
Maddiangat is the gateway of
the municipality of Quezon, Nueva
Vizcaya. It is located 5 kilometers from
the gateway to the municipality of Solano
Nueva Vizcaya and eastern part of
Magat.
Before, the early settlers who
came from Ilocos region named this
placed San Antonio. They suitably
named after patron St. Anthony because
of its geographical location and it was
under the municipality of Solano.
Maddiangat is a place
crisscrossed by several creeks, which is
abundant with different kinds of marine
product. The biggest of these creeks is
the Maddiangat creek. Because of the
abundance of fish, majority of the people
engaged in fishing at that time. The
place became popular, so therefore the
people agreed to name this place
Maddiangat after this famous creek on
the month of May 7 in the year nineteen
hundred and sixty-one (1961) under the
municipality of Solano.
Since then this barangay was
popularly called Maddiangat and was
confirmed by the pioneer municipal
officials of Quezon in the year 1961
Maddiangat folks was composed mostly
of Ilocanos, Tagalog Pangasinense,
Bicolanos, Igorots and Ifugao. The
dominant client dialect used is Ilocano.
At present Maddiangat creek was tapped
to irrigate rice fields since it’s the main
source of livelihood among its people.
This could irrigate 90% of the leading
rice producers of the barangay.
3.SPOT MAP
The barangay Maddiangat of
Quezon Nueva Vizcaya is only five
kilometers away from the gateway of
Solano Nueva Vizcaya. First to see when
arrived at the barangay is the wide and
fresh rice fields, then next is their clean
barangay hall with welcoming and kind
barangay officers. Near their hall is the
barangay Maddiangat EIementary
School.

The barangay maintains their healthy
green leafy vegetable garden in their
barangay hall to serve as their source of
healthy food whenever there is an
activity, meeting or any programs in their
barangay hall. They planted fresh
eggplants, okra, pechay and string
beans with trees of fruits around the
garden such as papaya, melon and
many more that makes the environment
refreshing.

III.CHN PROPER
A. Description
The family is living a very simple
life at barangay Maddiangat Quezon
Nueva Vizcaya. The couple were the
only ones in their house since their three
children has their own families. The
husband is a simple farmer while his wife
is housewife.
Their source of income was from
the rice field that they are taking care of,
every four months they receive four to
twelve sacks of rice as their share in
working in the field. Sometimes their
children lend them some amount but not
regularly due to their children who are in
college, so in order to keep everyday’s
need attended, the couple do extra
works like washing clothes of their
neighbors and drying corn in the
pavements.
Their house is already old and
rugged due to old age but will be
renovated in the coming next year.

Our intervention was to fix their comfort
room so that they will have a comfortable
and safe comfort room. The first day of
our intervention was to cement the floor
because it was crack and uneven and
addressing these promptly helps
maintain safety. Each of us help in doing
this because it was a heavy task
specially that we only have one boy on
our group. We were happy and satisfied
that we finished it because we know that
they can use it safely.
While the others are finishing the
cementing of the floor, three of our
groupmates cleaned the front yard of the
house as there are presence of dirts
scattered around and woods that are
disorganized. We fixed the woods and
arranged them under the wooden chair.
We also disinfect the tables and chairs
then arranged them properly.
The second day of our intervention was
to put a tent on their comfort room
because it was open and they have no
privacy while using it.

After implementing some safety


measures, our client’s comfort room
have undergone a significant
transformation, it is now much safer to
use without the risk of any injuries or any
underlying health conditions, also
providing a sense of privacy for the
family to use. Furthermore, the outdoor
of their house is now clean and woods
that were recently scattered around the
house are now organized, thorough
removal of plastic and pieces of
cardboard boxes, and proper disposal of
corn cobs, resulting in a much cleaner
and more visually appealing outdoor
environment for the family.
Abstract
The concepts of community nursing and
health together provide the foundation
for understanding community health. The
term community refers to the collection of
people who interacted with one another
and whose common interest of
characteristics from the basis for a sense
of unity and belonging.
(ANDERSON&MCFARLENE, 2004) the
term health is conceived of as the
effective functioning of self-care
resources which ensure the operations
and adequacy of self-care actions.

Community health nursing focuses on
improving health outcomes of
populations through holistic approach
that emphasizes prevention, education,
and access to care. Nurses in this field
engaged in a variety of roles including
health promotion, health prevention, and
advocacy working collaborately with
individuals, families, and communities to
address health disparities.
The view of community health is the
meeting of collective needs by identifying
problems and managing interactions
within the community itself and between
the community and the larger society.
Thus community health programs are
provided to help meet this needs.
We the student nurses had been
exposed to the community of the
barangay Maddiangat, Quezon Nueva
Vizcaya, an urban type of community
where people are engaged in a variety of
agricultural activities such as farming and
fishing, apart from its scenic beauty
which is rapidly gaining attention as
being famous with their water falls.
This study is a non-experimental
research specially a descriptive
normative survey which describes the
health practices and condition of the
Vergado family living in Brgy.
Maddiangat, Quezon Nueva Vizcaya.
The survey was conducted with the aid
of survey questionnaire composed of
both open-ended and closed-ended
questions. Self-observed data are
collected from each member of the group
for the purpose of describing the family
situation and other specific variables.
The questionnaire includes the personal
data of the respondents, the general
household data, environmental,
economic, educational, and medical
health data as well as some of the
personal health perceptions of the
members of the household.
The result of the data showed that the
family has poor competence in terms of
health maintenance. They have incorrect
health practice due to socio-economic
factors that seemed as a hindrance in
their quest for achieving optimum level of
wellness.
After all data has been collated, number
of problems were identified including the
foremost concern which is the poor
hygienic toilet facility, followed by
improper disposal of pig manure,
incomplete covid-19 pfizer vaccination
and hypertension.
We, student nurses, have come up with
recommendations and interventions to
better the problems. We provided
materials such as cement and a tarp for
covering, with cleaning materials used to
clean the entire surrounding of the
house. Series of health teaching were
conducted as well.
We, student nurses, aimed to provide
quality and efficient health promotion and
illness prevention services to this family
and were all possibly done though the
active participation of the members of the
family.

We help and coordinate to one another,
we’re always prepared, as for our CI, she
is kind and open to answer any
questions as she instructed us properly
and clearly.
Learn: I learned this day the importance
of community health nursing which was
to promote health and prevent illness
towards the community.it allows me to
understand how to address the needs of
individual members of a family, it allows
me also to learn and practice proper
relationship towards my fellow students
and families.
Hindrances: Bad weather due to the
typhoon Julian, muddy road that sticks
on our shoes and the strong wind that
blew our umbrellas and lastly the lesser
number of poor households who needs
intervention.
Recommendation: there should be no
late comers, should strictly follow time
schedule.

Observation:
Self: To myself, I learned to understand
more the cultural background of the
community. I enjoyed this duty because
we get to know the families in the
community.
Groupmates: To my groupmates, I
noticed that they are so cooperative and
we are doing our best to fully accomplish
our task which is to interview as much as
we can.
Clinical Instructor: To my clinical
instructor, I observe that she is kind and
provide us important information on what
were going to do.
Learning Insights: I learned that being
an active listener allowed me to better
understand their health concerns and
barriers to care. Also some of the
families are struggling with transportation
and financial matters which impacted
their ability to seek care.
Hindrance: The hindrance for me is the
bad weather due to typhoon Julian, the
strong winds because there’s a
possibility that a tree might fall and the
dog’s that barking on us everytime we go
to the houses.

Learning Insights: Our second day of
community duty, i learned how to collect
and gather data in the community and i
was able to improve more my
communication skills.
Hindrance: The hindrance of my
learnings is my foot are very tired in
walking long distance to go to different
houses to have interview and collect
more information.
Recommendation: I hope in the next
day I can be active and energetic every
time I will exposed again in the
community and I can manage my own
health so that it won’t affect my
learnings.
Comment/Suggestion: Our community
duty today is done well, and our clinical
instructor did a great job on guiding and
helping us on how to collecting and
gathering data. She is also very kind and
easy to approach which is a great help
on our learnings.
DAY 3

Objectives:
-To compile and write all the data and
information gathered from the family we

-To be able to choose a case for our
group case presentation
-To study our case and identify the

Observations:
> Self Observation- I help and
cooperate with my groupmates in doing
our family case study, and i am doing my
best in every activities.
> Groupmates Observation- I observe
my groupmates are all participating in
doing our case study to prepare for the
case presentation.
> Clinical Instructor Observation- Our
clinical instructor teach us in doing the
case study and explain what to write in
every column of the format in family case
study.
Learning Insights: On this day i learned
that complete information of every
member of the family is needed and
important in writing the 1st level
assessment for the family case study. I
also learned that it is not easy to collect
data and identify the problem.
Hindrance: The hindrance is i cannot
understand properly about the family
case study and don’t know what to write
in the format.
Recommendation: I hope I can
understand clearly especially in doing
the 1st level assessment in family case
study.
Comment/Suggestion: Explain and
discussed the lessons or topic clearly
about the procedure on how to do the 1st
and 2nd level assessment.

Observations:
> Self Observation- I felt very tired and
sleepy, but i am trying my best to help
and participate with my groupmates.
> Groupmates Observation- I saw that
my groupmates are also tired walking
and having some complaints while
walking to visit the family.
> Clinical Instructor Observation-
Ma’am Julita Rodriguez guided us during
our community duty and she help us in
making our family case study.
Learning Insights: In the fourth day our
community duty, i learned to be patient
even i am tired walking, and waiting
other students who are late.
Hindrance: The hindrance in my
learning is the bad smell of canal coming
from their neighbors near the house of
the family.
Comment/Suggestion:
-Lack of clinical instructors during our
community duties and even in the
hospital.
-Lack of transportation for Nursing
students going to their duties in far
places.

> Self Observation- My confidence is
not enough in talking or interviewing the
family.
> Groupmates Observation- My
groupmates are all good and very much
cooperative, and they are happy to be
with in our community duty.
> Clinical Instructor Observation- Our
clinical instructor ma’am Julita Rodriguez
guided us and also come with us in
visiting the family in their house.
Learning Insights:
-This day i learned that helping those in
need makes me happy and it feels good,
and i learned a lot from the family the
importance of loving and taking care to
each other, they are also kind and
generous to us
Hindrance: The hindrance in our
learnings is the hot weather that might
affect our duties and learnings in the
community setting.
Recommendation: My recommendation
is to learn not to complaint and just enjoy
the opportunity and experience in the
community.
Comment/Suggestion: The weather is
very hot especially when we are inside
the jeep, and waiting for our clinical
instructors because they are talking to
long with each other.

Observations:
> Self Observation- I was able to do the

CI gave the test paper for Quiz 1 and 2
about home visit and bag technique.
Learning Insights: I learned on how to
do the 2nd level assessment in family
case study, and also i am very happy
because i got a passing score in the quiz
about home visit and bag technique.
Hindrance: The hindrance in our
learning is the late announcements on
what we will do and also no updates to
inform us students about our community
duties.
Recommendation: My recommendation
is to tell or announce early for us
students to be updated and to know what
we will do in our case study.
Comment/Suggestion: Explain clearly
the steps of 1st and 2nd level
assessment and teach us students on
how to do the Nursing care plan (NCP).

Observations:
> Self Observation- I helped in cleaning
the backyard of their house and arrange
the scattered woods.

groupmates are all active and we are
helping each other in doing the

clinical instructor guide and help us in
doing the intervention with the family for
our case study.
Learning Insights: I learned that
cleanliness and sanitation is the best
way to prevent illnesses that's why it is
very important to clean our environment
everyday and always maintain

Hindrance: The hindrance is the bad
smell of canal of their neighbor near the
house of the family.Recommendation:
Having a clean surrounding with fresh air
and no foul smell.

Observations:
> Self Observation- I was able to do
and finish my 1st and 2nd level
assessment and the Nursing care plan.
> Groupmates Observation- They are
all participated in the group case study,
and doing their best in community duties.

Learning Insights: In the eight day of
community duty, i was able to learn the
importance of good
communication,friendship, and teamwork
in doing all the activities.
Hindrance: The hindrance in our
learning is the unpleasant smell of the
canal in their neighbor.
Recommendation:
-To educate their neighbor to practice
cleanliness and sanitation especially
they are making food like wrapper and
miki to sell.
Comment/Suggestion: I realized that it
is not easy to do the implementation and
intervention in community setting, and i
saw the different situations of the people
in that community and hear their stories
and backgrounds.

Observations:
> Self Observation- I was able to
accomplish all the requirements and
activities in Community Health Nursing
(CHN).
> Groupmates Observation- My
groupmates help together to accomplish
our case study.
> Clinical Instructor Observation- Our
clinical instructor is very kind, and she
also teach us in doing our family case
study.
Learning Insights: On the last day of
our community duty, i learned a lot of
things like how to communicate to
different individuals in the community
and to gain their trust. I also realized the
importance of volunteering and helping
those family that needs help because it
feels good to help others.
Hindrance: Our room is too far for us
Nursing students when we are having
class or duties, and also the fare is very
expensive going back and forth.
Comment/Suggestion: I enjoyed my
community duties and have new
experience and adventure. Thankful to
our clinical instructor ma’am Julita
