The barangay maintains their healthy
green leafy vegetable garden in their
barangay hall to serve as their source of
healthy food whenever there is an
activity, meeting or any programs in their
barangay hall. They planted fresh
eggplants, okra, pechay and string
beans with trees of fruits around the
garden such as papaya, melon and
many more that makes the environment
refreshing.

III.CHN PROPER
A. Description
The family is living a very simple
life at barangay Maddiangat Quezon
Nueva Vizcaya. The couple were the
only ones in their house since their three
children has their own families. The
husband is a simple farmer while his wife
is housewife.
Their source of income was from
the rice field that they are taking care of,
every four months they receive four to
twelve sacks of rice as their share in
working in the field. Sometimes their
children lend them some amount but not
regularly due to their children who are in
college, so in order to keep everyday’s
need attended, the couple do extra
works like washing clothes of their
neighbors and drying corn in the
pavements.
Their house is already old and
rugged due to old age but will be
renovated in the coming next year.

Objectives: To help individuals develop

Observation:
Self: To myself, I learned to understand
more the cultural background of the
community. I enjoyed this duty because
we get to know the families in the
community.
Groupmates: To my groupmates, I
noticed that they are so cooperative and
we are doing our best to fully accomplish
our task which is to interview as much as
we can.
Clinical Instructor: To my clinical
instructor, I observe that she is kind and
provide us important information on what
were going to do.
Learning Insights: I learned that being
an active listener allowed me to better
understand their health concerns and
barriers to care. Also some of the
families are struggling with transportation
and financial matters which impacted
their ability to seek care.
Hindrance: The hindrance for me is the
bad weather due to typhoon Julian, the
strong winds because there’s a
possibility that a tree might fall and the
dog’s that barking on us everytime we go
to the houses.
Recommendation: More clinical
instructors.
Comments/Suggestions: Overall, I can
say that this duty reinforced the
importance of holistic care and
community involvement in promoting
health and well-being.

To grasp and understand the importance

With self-observation, I realized that
communicating with people helped me
improve my communication skills.
With my group mates, we grew closer
each time that we’re on duty; our
teamwork has helped us.
And lastly, with Ci observation, I did
observe that our Ci would teach us
before we went to actual houses and
taught us what we should do to have a
better outcome of the house visits.

I did learn that having social status does
help you a lot. Also, I learned how these

•Objectives:
are still the same to hone my skills and
knowledge during the community
service.
To look for families who are really in
need of help for us to help them and
have a better understanding of their
lives. I have noticed that a lot of them are
sick or living in poor conditions, such as
the epeleptic px that we have seen
during the occasional survey and
community profiling.
To understand the importance of why we
do such things, and also to educate the
family of the community about healthy
practices to make the right choices when
it comes to their health.
•Observation:
I’m becoming less shy when talking with
other people and becoming more
courageous, as being shy wouldn’t do
me any good.
With my group mates, we would help
each other as we go along the way, as
building effective communication and
teamwork will help us go through
everything.
•My learning insights:
would be that building trust will always
be important, and at the same time,
being respectful is on par with it. We
have to respect people’s choices and
people in general to gain their trust to
promote much better health interventions
and interactions.

would be that we would have to go and
walk to much more secluded places to
have a family to talk to, as a lot of the
houses that we go to would’ve been

For myself, I have become a lot more
confident with communicating with
others.

closer as we initiated more

Is that engaging directly with the family in
their home environments allows our
group to gain a deeper understanding of
the living conditions and family dynamics
of the family that we have selected. By
understanding all of these, it can

Myself is that I wasn’t thinking of getting
dirty like I usually think about as I hate
getting dirty. I was thinking about how to
really help our clients in making their
front yard clean and away from any
injuries.
With my groupmates we were divided
into cleaning the front yard and helping
with the renovation of the comfort room,
but we all have one thing in mind and
that is to help our clients make their lives
a little bit better.

I’ve learned that putting our minds
together will make a better outcome.

With myself, I got a little sad bidding
goodbye as it is our last meeting.
With my groupmates, they were also a

I learned that patience is very important,

Recommendation:
Assign the students to a designated area
to make the work easier. In this way,

that the students will not have to spend a

-Self observation: Honestly, I cannot
focus on listening to our Clinical
Instructor because the surroundings
were quite noisy. This resulted in me not
understanding much of what is being
taught by the Clinical Instructor.

-CI observation: She guides us and
explained to us what should we do.
Learning insights:
Today’s activity, which is making a first
level assessment with the data we have
gathered, helps me improve my critical
thinking skills. Although I was confused
with what our CI have discussed, I was
thankful that she guides us and helps us
in doing our activity.
Hindrance:
Too many students in one room.

Recommendation:
Each Clinical Instructor should gather
their students in one room to avoid being

The information we have gathered today
is a great help for us to have ideas and

If possible, allow students to wear light
clothing to be comfortable in hot weather.

> Self Observation- I am glad and
excited when i saw the barangay of
Maddiangat even it is far. It was a great
experience and adventure to go to
community and meet different people.

my groupmates because they are my
classmates and also my groupmates in
hospital duty.
> Clinical Instructor Observation- Our
clinical instructor was Mrs. Julita
Rodriguez, she is the one who assigned
to guide us during our community duty
and she discussed how to do the ocular

Learning Insights: Being in the
community duty is new for me because it
was my first time. This day i learned the
importance of community health nursing
and experienced to be an eye opener in
understanding the needs of every
member of a family and how to address
it. I also learned that communication is
very important in gathering
data/information in the community.
Hindrance: It is difficult to interview the
family in the community because their
houses are too far and some houses we
passed by are closed, and also the bad
weather while we are walking to
interview every member of the family.
Recommendation: My recommendation
is to become more confident in
communicating to different people in the
community so that it won’t affect my
learnings and duties in the community.

Learning Insights: Our second day of
community duty, i learned how to collect

walking long distance to go to different
houses to have interview and collect
more information.
Recommendation: I hope in the next
day I can be active and energetic every
time I will exposed again in the

Learning Insights: On this day i learned
that complete information of every
member of the family is needed and
important in writing the 1st level
assessment for the family case study. I
also learned that it is not easy to collect
data and identify the problem.
Hindrance: The hindrance is i cannot
understand properly about the family
case study and don’t know what to write
in the format.
Recommendation: I hope I can
understand clearly especially in doing

sleepy, but i am trying my best to help

our community duty and she help us in
making our family case study.
Learning Insights: In the fourth day our
community duty, i learned to be patient
even i am tired walking, and waiting
other students who are late.
Hindrance: The hindrance in my
learning is the bad smell of canal coming
from their neighbors near the house of
the family.

hospital.

-This day i learned that helping those in
need makes me happy and it feels good,
and i learned a lot from the family the

learnings is the hot weather that might

is to learn not to complaint and just enjoy
the opportunity and experience in the

learning is the late announcements on
what we will do and also no updates to
inform us students about our community
duties.

is to tell or announce early for us
students to be updated and to know what
we will do in our case study.

> Self Observation- I helped in cleaning
the backyard of their house and arrange

Learning Insights: I learned that
cleanliness and sanitation is the best
way to prevent illnesses that's why it is

Learning Insights: In the eight day of
community duty, i was able to learn the
importance of good
communication,friendship, and teamwork
in doing all the activities.
Hindrance: The hindrance in our
learning is the unpleasant smell of the
canal in their neighbor.
Recommendation:
-To educate their neighbor to practice
cleanliness and sanitation especially
they are making food like wrapper and
miki to sell.
Comment/Suggestion: I realized that it
is not easy to do the implementation and
intervention in community setting, and i
saw the different situations of the people
in that community and hear their stories
and backgrounds.

Observations:
> Self Observation- I was able to
accomplish all the requirements and
activities in Community Health Nursing
(CHN).

our case study.

clinical instructor is very kind, and she
also teach us in doing our family case
study.
Learning Insights: On the last day of
our community duty, i learned a lot of
things like how to communicate to
different individuals in the community
and to gain their trust. I also realized the
importance of volunteering and helping
those family that needs help because it
feels good to help others.
Hindrance: Our room is too far for us
Nursing students when we are having
class or duties, and also the fare is very
expensive going back and forth.
Comment/Suggestion: I enjoyed my
community duties and have new
experience and adventure. Thankful to

What I have learned is that we need to

I experience that I still have a lot of
