    AFFIDAVIT OF DISCREPANCY

    I, SILVESTRE MARIANO JR., of legal age, married, Filipino citizen, and


    a resident of #882 Romulo Blvd., San Vicente, Tarlac City, do hereby depose and
    say THAT:

    1. That my account ID: M000470900155 with Tarlac Electric Incorporated


    contains the name “SILVESTRE MARIANO” however, my true and
    actual name is actually “SILVESTRE MARIANO JR” as indicated in my
    Birth Certificate, Senior Citizen ID, and Employee ID which are hereto-
    attached to this Affidavit;

    2. That the name “SILVESTRE MARIANO” and “SILVESTRE


    MARIANO JR” refer to one and the same person – the herein affiant – with
    the latter name as my true and accurate name;

    3. That I am executing this affidavit to attest to the truth of the foregoing


    statements and fact.
