1. OCCUPATIONAL HEALTH AND SAFETY PLAN

The Occupational Health and Safety Plan seeks to comply with current national regulations and
internal regulations established by the Entity, ensuring the necessary basic infrastructure conditions
that allow workers to have access to essential hygiene and medical services.
Furthermore, this Plan aims to improve the working conditions of its employees, making their work
safer and more efficient, reducing accidents, providing them with essential personal protective
equipment and training them in safety procedures and habits.
For the preparation of this plan, the established regulations in force have been taken into account,
considering that our Law 29783, Law on Safety and Health at Work, Law No. 30222 Law that
modifies Law No. 29783, Law on Safety and Health at Work 07/11/2014, Supreme Decree No. 005-
2012-TR, Regulations of Law No. 29783, Law on Safety and Health at Work. 04/25/2012 and
Supreme Decree No. 006-2014-TR, Amendment to the Regulations of Law No. 29783, Occupational
Health and Safety Law, approved by Supreme Decree No. 005-2012-TR 08/09/2014, which
establishes that the employer must guarantee in the workplace the means and conditions that protect
the life, health and well-being of workers and those who do not have an employment relationship and
provide services or are within the scope of the workplace.

1.1. GOALS

• To publicize the occupational health and safety policy for accident prevention and control
of risks.
• Encourage company personnel to carry out their activities safely through the use of
appropriate Personal Safety Equipment.
• Maintain a good level of occupational health of staff.

• Prepare staff so that in the event of an emergency the necessary measures are taken.
• Provide safe conditions for workers in all places where activities involving some risk to them
are being carried out.

1.2. STRATEGIC PLAN

To ensure the success of this Occupational Health and Safety Plan, the following activities will be
carried out:
• The media will be used to disseminate this plan.
• Adequate signage will be provided in the areas within which personal protective equipment
(PPE) must be used.
• Ongoing medical care for illnesses will be provided.
• Staff will be trained in important aspects of first aid and others.
1.3. DESCRIPTION OF THE RISK PREVENTION SYSTEM

The Risk Prevention System is an integral part of the company's general management system and
has been designed taking as reference the requirements of the Occupational Health and Safety Law
No. 29783. The general plan for this work has as its basic principle that the following documentation
scheme is maintained as mandatory compliance due to its importance:
• Current government legislation.
• Policies, standards and procedures
• Internal construction procedures.
• Information circulars.
• Field documents.

1.4. DEFINITIONS

Policy: Risk Prevention Policy


Standard: Official documents approved by the General Management of the Company that serve as
a parameter for the preparation of internal construction documents.
Lost Time Accident: Absence of the worker for more than one work day.

Minor injury: Minor injury that does not result in lost days and requires First Aid care without being
absent from the work day for a period of more than one work day.

Serious injury: Serious injury that causes lost days and leads to first aid care and hospitalization
with absence from the work day for an indefinite period.

Fatal Injury: Injury that causes death.

1.5. RESPONSIBILITIES OF EACH ACTOR IN THE PROCESS OF EXECUTION OF THE


WORK.

• . The Occupational Health and Safety Act 29783 is applicable to all economic and
service sectors; it covers all employers and workers under the labor regime of
private activity throughout the national territory, workers and
public sector officials.
• . The entity or Contractor in charge of carrying out the work will prioritize the supply of
the systems, equipment and materials needed to meet the general safety
requirements in all facilities and work areas, both interior and exterior, that comprise
the Project for the execution of the work.
• . In turn, the Contractor firms of the Work, in a unitary and independent manner, will
be responsible for additionally supplying the barriers, mesh, tapes and other
elements of physical protection, as well as the sufficient number of cones, warning
lights, signs and other safety signals, essential to identify and delimit the risks in their
respective assigned work areas.

• . The entity or Contractor in charge of carrying out the work will provide all of its
construction personnel with the required work clothing and personal protective
equipment, in the required quality and quantities. Workers are required to use the
work clothing and personal protective equipment they receive correctly and
permanently, under penalty of disciplinary action.
• . The entity or Contractor in charge of carrying out the work will maintain adequate
supplies of equipment, tools and other work and safety implements on site, in
optimal conditions for use, including sufficient quantities of appropriate and identified
containers to collect garbage and debris from the work.
• . In order to be able to join construction work, it will be an essential requirement that
all workers, without exception, be up to date with their registration in the
Complementary Insurance for Risky Work, and also have recent Medical
Recognition Certificates, which prove their aptitude for work. Each Contractor firm
shall promptly submit to the Administration of the entity the documentation of its
personnel, which validates such requirements.
• . All Supervisors of the entity or Contractor in charge of carrying out the work and its
Contractors must strictly comply with the contents of the internal Safety Standards
and Procedures, established by the Company through the Project Safety
Management, with the exclusive purpose of reducing to a minimum or completely
eliminating the risks to the safety of the works. Such regulations shall refer to at least
the following aspects:
▪ Operation of Heavy Machinery and Construction Mechanical Equipment.
▪ Safe Use of Dump Trucks and Larger Vehicles.
▪ Road Safety, inside and outside the Construction Facilities.
▪ Safety in manual and equipment excavation work
▪ Safe Use of Ladders, Scaffolding and Work Platforms.
▪ Safe Use of Lifts and Winches.
▪ Safety in Pressure Concreting Operations.
▪ Rescue Operations and Basic First Aid.

RESIDENT ENGINEER

• Develop, with the assistance of the prevention specialist, the risk analysis of all the work
entrusted to you and present it to the site manager for approval, before the start of the work.
• Plan the development of the works in a timely manner, in coordination with prevention, in
order to
ensure that the preventive measures and controls established in the work procedures and
risk prevention and environmental management directives are implemented before the start
of activities.
• Coordinate with the construction manager, the entry of new workers, both for hiring
directly and through subcontractors, in order to guarantee the formal contracting process in
compliance with current legal provisions and client provisions, as applicable.
• Coordinate with the boss of equipment, he

ASSISTANTS, FOREMEN.

• Verify that the workers under your charge have received the "Induction Talk" and signed the
"ATS",

• Develop the ATS before the start of each new activity and when there are variations in the
initial conditions of the activity. Record evidence of compliance.
• Inform the workers under your charge about the dangers and environmental aspects

• Instruct your staff on the correct use and maintenance of the personal protective equipment
(PPE) and collective protection systems (CPS) required for the development of assigned
work and promptly request the replacement of those that are damaged. Record evidence of
compliance.
• Permanently use the personal protective equipment (PPE) required for the development of
the works and require their staff to use them correctly and in a mandatory manner.
• Give the "five-minute talk" to all your staff every day before the start of the day, using the ATS
as a reference. Record evidence of compliance.
• Ensure order, cleanliness and preservation of the environment in your work area.
• Maintain a state of permanent observation in your work front, supervising with
preventative mentality the development of tasks assigned to their staff and immediately
correcting any substandard acts and conditions that may arise. In high-risk cases, the

• Arrange the placement, if environmental conditions require it, of signage and
necessary collective protections, before leaving the work front.
• Immediately report any incident or accident that occurs on your work site to the Site Manager
and the Prevention Officer and provide truthful information about what happened during the
corresponding investigation process.
• Participate in the training program and the inspection program, as an instructor and
inspector respectively. Such participation will be recorded in the corresponding forms and
will be evaluated based on the performance tables of the Chain of Command.

SAFETY PREVENTIONIST

• Assist the construction resident in the preparation and implementation of the Risk Prevention
and Environmental Management Plan for the work, in accordance with the guidelines of the
Risk Management and Environmental Management System.
• Likewise, manage it and report the results of its implementation to the Construction Manager
and to the
Department of Risk Prevention and Environmental Management, simultaneously.
• Assist the Line of Command in the development of Risk Analysis, and the attendees and
foremen in the preparation of the ATS and completion of work permits.
• Keep in state of observation permanent supervising with mindset preventive
he
development of the work and immediately correcting, to the extent possible, any sub-
standard acts and conditions that may arise. In cases of high risk, operations must be
stopped until the dangerous situation is eliminated.
• Additionally, it has operational functions concerning the risk analysis of the different
activities carried out on site, participates in work planning meetings and coordinates with the
technical area the incorporation of preventive measures in specific work procedures, trains
construction personnel regarding compliance with risk prevention standards and supervises
the development of operations.

• The Head of Risk Prevention for the project reports simultaneously to the Project Manager
and Engineer. Resident and Field Engineer.

RISK PREVENTION COMMITTEE

Pursuant to Art. 18 of the WORK SAFETY AND HEALTH REGULATIONS, companies with 25 or
more workers must establish a Work Safety and Health Committee, which will be formed in equal
numbers, that is, with an equal number of representatives from the employer and the employee
sides.

Its functions are:


• Promote compliance with risk prevention provisions.
• Analyze and give an opinion on the company's Health and Safety Regulations.
• Carry out the general inspection of facilities and equipment at the work centers,
recommending the
adoption of the necessary preventive measures.
• To know the results of the investigations carried out by specialized organizations, on the
work accidents and occupational diseases that occur in the company.
• Prepare statistics on accidents and occupational diseases presented and the controls
taken to prevent further cases.
• Conduct monthly sessions on an ordinary basis and extraordinary sessions when there are
accidents
serious or when circumstances require it.
• Cooperate and carry out risk prevention campaigns and ensure that all workers
receive adequate training in this area.
• Establish training and capacity building programs at all hierarchical levels in techniques of
preventive control.

1.5.1. MAIN SECURITY REQUIREMENTS

1) WORK CLOTHES AND PERSONAL PROTECTIVE EQUIPMENT


1. All construction personnel will wear work-type work clothing made from
100% cotton drill fabric, including a protective vest with standard reflective tape
attached, and with the company identification placed in a visible place.
2. All construction personnel will use hearing protectors (earmuffs, earplugs) to
attenuate noise.

3. All construction personnel will use anti-cut gloves (leather, nitrile, vinyl, etc.) to
avoid

2) ORDER AND CLEANLINESS


1. Workers in general, and Supervisors in particular, will assign high importance to the
tasks of order and cleanliness of the work areas, before, during and after the
completion of any task.

3) BARRIERS AND SAFETY WARNINGS


1. Yellow plastic safety tapes will be used to indicate any latent dangers. Areas of
imminent danger will be kept fenced off with red plastic tape, prohibiting all unwanted
access. Only duly authorized personnel may approach these risky areas.
4) INDUCTION PROCESS.
1. It will be an essential requirement for all personnel to begin working on the site, that
they receive a prior induction talk by the Safety Supervisor or Contracting Company
lasting no less than 02 hours, aimed at providing orientation to workers on basic
aspects.
2. The topics of the Induction course must have the following structure: The Company
and accident prevention, Management policy in prevention of Work Accident Risks,
Occupational Diseases and environmental protection. Definitions (unsafe acts and
conditions, accidents, emergencies, incidents). Accident and incident investigation.
Emergency plan. Prevention activities. Construction Documentation. Job Safety
Analysis (JSA), PPE Order and cleanliness. Hand Tools. Tool reviews. Policies on
alcohol misuse. Rules for coexistence at construction sites and camps.

5) 5' SAFETY TALKS


Every day before starting work, the Supervisors and Foremen, with the ongoing advice
of their Safety staff, will gather the workers into groups to read and discuss the 5-
minute Safety Talk. Supervisors and Foremen will take advantage of these occasions
to provide specific instructions to refresh workers on additional precautionary measures
that should not be neglected during the day's work.
The topic of the safety talk will be:

of gathering people together and saying: “Comrades, there are some accidents that are
bothering us. They should be more cautious. Keep in mind that security must be
ensured. Don't forget it. See you later". Nor should we start rambling on about a
subject, hesitating and stammering, speaking in generalities and the importance of not
having accidents. No
tire your listeners with long speeches, which at the end leave them less informed than
before.

Meetings like this are a failure because no one participates except you, resulting in no
one contributing to problem planning or suggestions to help solve existing problems.
They are a failure because you are not prepared to teach nor are people prepared to
learn. There is also a simple technique for this:

You'll probably have to get people's attention and encourage questions or


suggestions at first to get things going, but with practice they'll get interested. By then

PREPARING THE MEETING FOR THE SAFETY TALK

1. Schedule the meeting at least a week in advance, so that you have a chance

present the talk in a compelling way, without reading it, in your own words.

2. Check the meeting directly in the workshop. Seating space is not absolutely
necessary, because meetings are short, but arrange them so that everyone can
easily see and hear you.

3. Gather in advance all the materials you intend to use at the meeting (posters,
handouts, cards, etc.) When possible, use existing equipment to illustrate your
points. For example, broken hammer handles, damaged tools to demonstrate
how they can cause accidents, fire extinguishers, personal protective equipment
to demonstrate their proper use.

HOW TO CONDUCT THIS MEETING

1. Hold a safety talk meeting every week.


2. Limit each talk to five minutes (if possible).
3. Start the meeting by congratulating your employees on some recent good
work or by giving them some constructive criticism in a friendly tone.
4. Give the talk in your own words, with your own examples. For each talk, bring
your notes or keep this booklet handy for easy reference, but use it only to
remember or outline what you need to say in your own language.
5. Get people involved. The purpose of these talks is to get workers thinking
about safety issues. One of the best ways to achieve this is to turn the talk into a
discussion. Have them name and point out the risks and the remedies that can
be put in place. Encourage them to make suggestions to improve the safety
conditions in their department.

OTHER MATTERS THAT CAN BE DISCUSSED AT THE MEETING


Review any injuries that have occurred during the past week.

6) JOB SAFETY ANALYSIS (AST)


1. Both the Contractor staff will obligatorily use the documents called
Job Safety Analysis (JSA), which will be carried out daily by the workers at the
workplace itself, as a prerequisite for starting any task on the Site.
7) WORK PERMITS
1. The Supervisors directly responsible for the works, as well as for the Contractor firms,
must coordinate with the Head or Supervisor of Safety of the Work, the permits
required to carry out those works or maneuvers that, due to their nature or special
characteristics, require the adoption of exceptional precautions to achieve safe
execution.
2. For example, prior permits will be required to carry out any demolition, excavation,
height, hot work, confined space work, and other clearly defined tasks in this regard.

8) SAFETY INSPECTIONS
1. The Supervisors and Resident of the Construction Site and of the Contractor firms will
carry out daily and weekly inspections of their workplaces, submitting in writing to the
Head of Safety of the Construction Site the results of said inspections, indicating the
substandard conditions and acts observed and corrected.

9) FIRE PREVENTION
1. To prevent potential fire risks at the site, constant attention will be given to the
following aspects:
• Permanent development of Order and Cleanliness tasks in all phases of work
and in all work areas.
• Proper use of Hot Work Permits and Procedures.
• Provision of Safe Procedures for identifying, storing and handling
combustible substances, lubricants and flammable materials.
• Knowledge of the Emergency Response Plan and the proper use of fire
extinguishing equipment and systems in the facilities.

As part of the construction planning, all activities that will be carried out during the development of the
activities are evaluated, identifying the dangers associated with each of them and assessing them through a

Hazard: A source or situation with the potential to cause harm in terms of injury to people or illness, damage to
property, damage to the environment, or a combination of these.

2. EMERGENCY RESPONSE

• Create exit stairs that help with quick evacuation in case of emergencies.
• Keep the ground stabilized with the help of water and cement by watering constantly.

In the Circulation of Personnel

• In excavations and trenches with a depth greater than 1.20 m, stairs, ramps, steps or other
systems that guarantee easy and safe entry and exit of personnel from the work will be used.
• If ladders are used, they must protrude from the ground surface by at least 1.00 m and must be
secured to prevent slipping. These stairs should not be more than 25 m apart.
• If the width of the trench at ground level is between 0.70 and 1.20 m, solid walkways at least
0.90 m wide must be placed.
• If this width is greater than 1.20 m, the walkways must have handrails and sufficient support on
the ground to prevent the walkway from moving.

• Walkways shall not be spaced more than 20 m apart to prevent personnel from jumping over the
trenches.

On the Circulation of Vehicles and Equipment in the Work Area.

• The transit of vehicles of any size will be at a minimum horizontal distance from the edge of the
excavation equal to 1.5 times the depth of the excavation.
• If any heavy machinery (shovels, backhoes, trucks, generators, etc.) needs to be temporarily
installed near the edge of an excavation, it will be installed at a distance no less than 1.5 times
the depth of the excavation.
• The adjacent sectors of mobile, stationary or semi-stationary equipment must be marked and
also fenced, placing tapes or berms with a minimum height of 1.00 m to limit the distance of the
equipment from the excavation or trench.
• Lookouts will be placed to monitor vehicle movement, especially at the entrances to the
excavations.
• All personnel involved in excavation work near vehicular traffic will use
reflective vests.
For mobile equipment operators

• Operators are required to check vehicles daily.


• The check-up will consist of inspecting brakes, steering, backup alarms, emergency equipment,
tires, lights, gearbox, accessories, among the most important ones.

To Reduce Noise Generation

• Lubricate noisy machine parts and equipment.


• Make sure the machine is properly mounted.
• Make sure the machine is balanced.
• Replace worn parts.
