1. NOTICE OF THE ANNUAL GENERAL MEMBERSHIP MEETING (“AGMM”). This set of
guidelines for online registration will be sent by the Property Management Office to the registered members of the Azure Urban Resort Residences Condominium Corporation together with the Notice of the AGMM on 22 November 2024, including the link for the registration.
• Member’s registration link with the option to designate a proxy both for individual and corporate unit owners ✓ Unit Owner Registration Link must be accomplished by the member who intends to attend the meeting ✓ Unit Owner with Proxy Registration Link must be accomplished by members who opt to be represented by his/her/its authorized representative during the meeting. - A Signed Proxy Form for individual unit owners OR a Notarized Secretary’s Certificate for corporate unit owners must be uploaded in the registration form. ✓ Members shall accomplish the form via online registration no later than November 15, 2024.
2. REGISTRATION. The online registration for those who will be attending the meeting will start at 1:00 PM (PST) on September 18, 2024, and end at 5:30 PM (PST) on November 15, 2024.
A. REGISTRATION FOR UNIT OWNERS WHO WILL BE ATTENDING THE MEETING
1. Copy the link provided below and paste it to your Google Chrome browser then click Enter OR SCAN the QR CODE. Note that the use of the Google Chrome browser is required to avoid incompatibility errors while accessing the online form.

Since we will use Google Forms for the registration, it will require you to log in using your Gmail Account. If you do not have a Gmail account, please create one for this purpose. 3. You will be directed to the Azure Urban Resort Residences Annual General Membership Meeting Registration Portal. Kindly fill out all the required information with an asterisk (*) to ensure hassle-free submission. Please take note of the Tower Legend in encoding your unit number/s. For multiple units, use a comma (,) in separating each tower & unit number.
4. Attach your valid government-issued ID with a photo and signature. Click on the Add file. Drag your file or select from your device then click the Upload button.
Note: The file size of the document is up to 10MB only. You may upload files in JPEG, PNG, or PDF format.
5. Select the type of attendance.
6. List down up to a maximum of three (3) questions you would like to be discussed during the meeting then click Submit.
7. Once submitted, you will be directed to a confirmation page informing you that the form has been submitted successfully.
Kindly take note that all submitted forms and attachments shall be subject to validation. B. REGISTRATION FOR UNIT OWNERS WITH ASSIGNED PROXY
