TMJ

Temporo Mandibular
Joint
drg. L. Cinthia Hutomo, Sp. Ort
WHAT IS
TEMPOROMANDIBULAR
JOINT ?
WHY SHALL WE KNOW
TMJ ?

Pain and dysfunction in TMJ region

NOTHING IS MORE FUNDAMENTAL

TO TREATING PATIENTS THAN
KNOWING THE ANATOMY
TMJ

Masticatory Occlusion
Muscles of teeth

Temporomandibul
ar Articulatory
System
PECULIARITY OF TMJ

1. Giglimoarthrodial

2. Bilateral Diarthrosis

3. Articular surface covered by fibrocartilage

4. Start develop in about 11th week in utero

Components
M. Pterygoid Lateral

M. Pterygoid Medial
CRANIAL COMPONENT

- Fossa Mandibularis
Ossis Temporalis /
Glenoid Fossa
- Anteriorly Auricular
Eminence
- Posteriorly Post
Glenoid Tubercle
MANDIBULAR COMPONENT
Capitulum mandibula ( Condyle )

- Ovoid Condylar Process

- Articulating surface of the
mandible
- Convex in all directions
but wider latero-medially
( 15 20 mm than antero
posteriorly 8 10 mm )
TMJ CAPSULES

- Fibrous membrane
that surround the
joint
- Covers the joint for
all around
- Thin above the disc
and thick below it
LIGAMENTS
Major Ligaments
- Temporomandibular Ligament
- Capsular Ligament

Minor Ligaments
- Stylomandibular Ligament
- Sphenomandibular Ligament

Others Ligaments
- Oto mandibular Ligament
- Disco malleolar Ligament
- Mallelo mandibular Ligament
ARTICULAR DISC ( MENISCUS)
- Biconcave Fibrocartilaginous
- Disc devides the compartment into 2
- Upper formed by temporal bone and articular disc
( Menisco Temporal )
- Lower formed by mandibular and articular disc (
Menisco Mandibular )
- To accommodate a hinging action as well as a
gliding action between temporal and mandibular
articular bone
MOVEMENTS

- Rotational / hinge movement in first 20 25

mm of mouth opening
- Translational movement after that when the
mouth is excessively opened
Mouth Mouth
Closed Closed
Translatory Movement
- Superior part of the joint as the disc and
the condyle transverse anteriorly along the
inclines of the anterior tubercle to provide an
anterior and inferior movement of the mandible

Hinge Movement
- Inferior portion of the joint between the
head of the condyle and the lower surface of
the disc to permit opening of the mandible
1. Depression of Mandible
- Pterygoid Lateral
- Digastric muscle
- Geniohyoid
- Mylohyoid

2. Elevation of Mandible
- Temporalis
- Masseter
- Pterygoid Medial
3. Protrusion of Mandible
- Pterygoid Lateral
- Pterygoid Medial

4. Retrusion of Mandible
- Posterior fibre of Temporalis
VASKULARISASI
Branches of External Carotid Artery

- Superficial temporal Artery

- Deep Auricular Artery

- Anterior Tympanic Artery

- Ascending Pharyngeal Artery

- Maxillary Artery
INERVASI

Sensory
- Auriculo temporal Nerve
- Masseteric Nerve

Motor
- Mandibular division of mandibular
nerve
AGE CHANGES OF THE TMJ

- CONDYLE

- DISC

- SYNOVIAL FOLD

- BLOOD VESSELS AND NERVES

THIS AGES CHANGES LEAD TO

- Decrease in synovial fluid formation

- Impairment of motions due to decrease in
the disc and capsule extensibility
- Decrease resilient during mastication due
to chondroid changes into collagenous
elements
- Dysfungtion in older people
