The first time Merlin tried healing an arrow wound was such a failure that he spent months mastering that spell afterwards. By the time he found Gwen in the forest with an arrow in her leg, he knew the spell so well he could heal her without much effort.

The wound is not the problem.

The problem is that Arthur and Gwen were unconscious, while Gwaine is very much awake.
