This is the thought record printable that I personally keep in my planner. I never liked the ones that typically come in CBT books and the like, with the very long narrow columns; they're so difficult to write in! I'm a journaler, so I was always scrunching tons of tiny writing into those awkward columns. I designed this worksheet with horizontal sections to give me plenty of room to write on each prompt.
