Instructions for use of Excel Spreadsheet to log internship hours:
The Excel Spreadsheet is designed to both allow logging of your internship hours AND do an
automatic summary of those hours.
General Information:
Do NOT change the width or height of any column or row. If any change automatically or
some content scrolls out of view that is fine.
Do a log entry for each day even if you do many things with more than one person or at
more than one site that day
If you do small amounts of Internship work on multiple days of the week you can put
them down for a single date. Say you did this in the note section.
Save these files with your last name first and change the date at the end of the file name
with each submission:
o JonesMaryInternshipLogSp15_02_01.xlsx (or .xlx)
o JonesMaryInternshipLogSp15_03_01.xlsx (or .xlx) etc.
Reflecting which preceptor you were working with.
This excel log is set up to capture information on a single MAIN preceptor at up to two sites
and some information on any secondary preceptors. To enter this data:
Put the names of your main preceptors above and on the spreadsheet as P A and P B.
o If you only work at one site, do not use any of the PB entry boxes.
At site A you can also indicate time worked with up to two other persons at that site.
o Do this by entering the names of these persons as P A2 or P A3 above and on the
spreadsheet.
o If you work with more than two additional people, enter hours with them as if
done with your main preceptor but put the persons name in the notes section.
At site B you can also indicate time worked with up to one other person at that site.
o Do this by entering the names of that person as P B2 above and on the
spreadsheet.
Hours spent with these additional preceptors will be credited (in the total area) only to the
main preceptor at that site since that person is officially responsible for your overall
Internship experience.
If you work with additional people at any site, enter hours with them as if done with your
main preceptor at that site. Provide their name, title, what they did in the notes section. If
they are at an agency not one of your sites say its name. Bold these details so can be
easily seen.
See below for more on entry of hours for each preceptor.
Recording your actual Internship activities:
To get a good picture of how many hours you spend in simulation, online teaching,
clinical teaching, class teaching, these will be separately calculated.
Put the number of hours you are actively engaged in simulation, online teaching, clinical
teaching, and class teaching each day on the left side of the spreadsheet in their appropriate
column. This section is in blue. This would include teaching in the classroom, working on online
activities (development of module, reading/ scoring discussions), directly working on simulation
(prep or the event) and in clinical setting including time spent doing assignments/conferences.
o Preparation time (e.g. writing a lecture) should not be included in these entries.

You will also credit these hours for the appropriate preceptor as described above
and below. (Yes, they will be entered twice as a result but the spreadsheet is set up
to only count them once.)

Recording the number of internship hours with each preceptor:
Enter the number of hours with each preceptor in their column.
Entering details about your Internship activities
Do ONE entry per day even if you work with more than one person or at more than one site.
1. Enter the date
2. Enter the number of hours you spent doing simulation, online teaching, clinical teaching
and formal classroom teaching in their columns (they are blue).
3. Enter the number of hours you spent with your preceptor(s).
4. If more preceptors than there is room for, include the hours spent with them with the
hours for your main preceptor and put their name in the Notes area.
5. If some of that work is done off site. (For example if you are working at home on a
lecture or are responding to discussion.)
6. Enter which of your personal objectives your activities helped meet
7. Enter which of the course objectives you activities helped meet
8. Put in brief notes about what you did that day. (Brief means brief. More detailed review
can be in your reflection posts.)
