    AFFIDAVIT OF DISCREPANCY
    I, FEDERICO H. JACUSALEM, of legal age, Filipino, residing at No. 045
    Prk. 7 St. Joseph Village, Baguio City, after having been duly sworn to in
    accordance with law, hereby depose and state:
    1 That I am the father of PIO VALDEZ JACOSALEM who was born on May
    5, 1971 in Sta. Cruz, Ilocos Sur;
    2 That it appears that my sons family name JACOSALEM as entered and
    registered with the Local Civil Registrar of Sta. Cruz, Ilocos Sur and
    confirmed by the National Statistics Office was erroneous as it was
    misspelled;
    3. That his family name should be spelled as JACUSALEM instead of
    JACOSALEM;
    3 That, however, the names PIO VALDEZ JACOSALEM and PIO VALDEZ
    JACUSALEM refer to one and only my son as above-referred to;
    4 That the erroneous entry and registration of my sons family name with
    the Local Civil Registrar of Sta.Cruz, Ilocos Sur and with the National
    Statistics office was due to inadvertence, and did not arise from any
    malevolence or sinister motive to hide his identity or to defraud any
    third party;
    5. That I am executing this Affidavit to attest to the truthfulness of the
    foregoing and to reconcile the discrepancy of my sons family name in
    any and of all his records and for all legal intents and purposes.
    IN WITNESS WHEREOF, I have hereunto set my hand this 26 th day of
    September, 2011, in the City of Baguio, Philippines.
