    AFFIDAVIT OF DECLARATION

    I M/S.Sainath Estates PVT LTD,Representative Mr. Satyanarayana Rao Saini residence Hno:1-
    8-505/8, Prakash nagar, near Register Office, Begumpet, Hyderabad - 500016.do here by
    solemnly affirm and state that following on oath as under.

    1. That I am deponent herein and I am giving with free will concern and without any
    under inflorescence and coercion.
    2. That I am the owner of the vehicle no .AP01AD0001 Mercedes Benz GL 35 Model
    which is insured with your company vide Policy no.28991441 Valid from 17-09-2017
    to 16-09-2018 where as on 14-02-2018, the said vehicle involved in incidentally took
    by accident near Karimanagar, Telangana, the vehicle was damaged and here I put
    forth claim before you for under own damage category.
    3. Through this affidavit, I hereby state that there is no third party involvement in this
    accident and such I did not inform the same to the Police and no crime registered, as
    there is no intimation to the Police.

    4. I further declare that contrary to the above contents of affidavit, any things wents in
    wrong I will be liable for prosecution and if at all any third party involvement is in this
    accident, I am alone liable to pay the amount that is going to be awarded against me
    and the company in view of the suppression of material facts and the company is not
    at all liable to pay anything in any manner.
    5. I further agree that I will not raise any further claim against this accident either
    before you or before any court of law and if act against the contents of the affidavit,
    the company is at liberty to prosecute me for the violation of the contents of the
    affidavit in the court of law.

    The above facts are true and correct to the best of my knowledge, belief and
    information
