Dear Ms. ____________,

After conducting an audit of your collections, it has come to our attention that you
have unremitted cash collections in the total amount of _______________. Your action is
a violation of the company policy, allegedly construed as misappropriation of company
funds. (TO BE EDITED – PLEASE INSERT DETAILS OF WRONG COMMITTED)

Please submit your written explanation to the disciplinary matter being charged
against you within FIVE (5) DAYS from receipt of this Notice.

Failure on your part to submit a written explanation within the prescribed period
shall be deemed as waiver of your opportunity to be heard, and constrain us to decide the
matter based on the available information and evidence on hand.

Also, you are directed to personally appear for a hearing to be held on


____________, 2020 at _____ AM/PM at _________________________ as this will
give you additional opportunity to verbally explain your side and present your witness
and other evidence regarding said disciplinary matter.

Furthermore, you have been placed under a 15-DAY PREVENTIVE


SUSPENSION WITHOUT PAY starting on January ___, 2020 until ____________,
2020, unless earlier withdrawn by written notice. Note that preventive suspension is NOT
A PENALTY. The company resorted to preventive suspension because imminent and
serious threat to the property of your employer as may be inferred from the nature of the
charge against you.

In the event that after due investigation the imposition of disciplinary action(s) is
warranted, same shall be known to you.
