Title: Senior Clinical Fellow in Obstetrics & Gynaecology

Location: Based at King’s Mill Hospital, Sutton-in-Ashfield

Accountable to: Medical Director

Reports to: Service Director

Medical Staff in Obstetrics & Gynaecology, Ward Staff, Divisional


Works with: Managers and other members of the Obstetrics & Gynaecology
Department.

 Assist in the management of patients on the ward.


 Supervision, teaching and training of junior staff and medical
students
Key tasks:
 Maintain the standards of the notes, investigations and GP
communications.
 To participate in the out of hours on-call rota.

We are looking for a Senior Clinical Fellow to work at middle grade level within our Obstetrics &
gynaecology department for 12 months from 1 August 2019 to cover maternity leave. If you are looking

you.

With over 3,300 babies born each year at King’s Mill Hospital, we are proud to have one of the highest
percentages of normal births in the country, below average caesarean birth rate of 23% and a home birth
rate above the national average.

The maternity unit at King’s Mill Hospital includes the pregnancy day care unit, antenatal clinic, maternity
ward (antenatal and postnatal), the Sherwood Birthing Unit and the neonatal unit. The Early Pregnancy
Assessment Unit (EPU) is situated within the King’s Treatment Centre. The Sherwood Women's Centre
at Newark Hospital provides comprehensive facilities for antenatal and postnatal care, including
ultrasound.

Revolutionary electronic maternity records, introduced in 2008, are unique to our Trust and provide a
secure system where each woman’s records can be instantly accessed by midwives and consultants.
This ensures the latest information is always to hand to provide the highest standard of care.
Your duties will include:

 To undertake general clinical care of in-patients and any related documentation. This work
involves the care of adult patients of all ages, including clerking of admissions. Your work will
be generally ward based as far as practicable.
 To provide ward based cover in the absence of other junior staff on the ward.
 To participate in Consultant ward rounds.
 To perform emergency duty in the wards as and offering advice as required.
 To undertake outpatient clinics each week.
 responsibility for immediate communications with the family doctor on the day of patient
discharge, and should ensure that discharge summaries and out-patient letters are sent to the
general practitioner as soon as possible.

 Full registration (with a licence to practise) with the General Medical Council (GMC)
 MBBS or equivalent medical qualification
 Successful completion of Part 1 MRCOG
 Have evidence of achievement of foundation competences from a UKFPO-affiliated foundation
programme or equivalent
 Evidence of achievement of ST1 and ST2 competences in this specialty by time of
appointment

Gynaecologists or postgraduate dean.

Located just outside of Mansfield town centre,

including a CaSH clinic, a multidisciplinary tier
two diabetes community clinic, an osteoporosis
service and wheelchair services.
Mansfield Community Hospital provides a vital
link between the acute wards at King’s Mill
Hospital and primary care.
What we do

In the last financial year there were 412,000 outpatient attendances, 102,000 people attended the King’s
Mill Emergency Department and more than 21,000 patients were seen and treated at the Minor Injuries

Each Division is headed by a Clinical Chair who works with the Divisional General Manager and Divisional Head of
Nursing to ensure the day-to-day provision of services. Each Division comprises a number of services headed by a
Service Director or Head of Service (usually a Consultant). Service Directors report to the Clinical Chair who in
turn reports to the Chief Operating Officer.

Divisions receive operational support from Finance, Human Resources, Strategic Planning and Information
representatives.
General Information

King’s Mill Hospital is situated close to the Derbyshire Peak District and Sherwood Forest and is within
easy reach of the major cities of Sheffield, Derby and Nottingham, the latter being only 14 miles away.

Road
The region has good road links and is well served by the M1 from London in the south and Leeds in the
north. Our hospitals are also within an hour’s travelling time of Derby, Chesterfield, Buxton, Lincoln,
Grantham, Sheffield, Doncaster, Nottingham, Leicester and Loughborough.

Rail
A rail service operates in the Mansfield and Ashfield area to Nottingham train station. The train stations
at Nottingham and Newark operate services to many locations across the UK. A main line service also
operates from Newark to London taking under 1 hour 10 minutes.

The catchment area for Sherwood Forest Hospitals NHS Foundation Trust comprises the central and
northern areas of Nottinghamshire. The main towns in the area are Mansfield and Sutton-in-Ashfield in
the south west, Worksop in the North West, East Retford in the north-east and Newark and Southwell in
the south-east. Textiles, light engineering and, to a diminishing extent, mining, are the industries in the
Mansfield and Ashfield district, with the remainder of the area being predominantly rural. The resident
population served by the Trust amounts to approximately 420,000 and the majority look for the provision
of acute hospital services to King’s Mill Hospital, in Sutton in Ashfield.

around the district and links into Derbyshire Dales. The area is also rich in sporting heritage, being the
home of cricketing legend Harold Larwood.

Education and Conference Centre

There is an active purpose built postgraduate education facility and separate reference library at King’s
Mill Hospital. Facilities include high technology equipment, lecture theatre and several seminar rooms.

There is also an excellent Clinical Illustration Department which provides Clinical and Non-clinical
photography/illustration, photography of Pathology and Laboratory specimens and desktop publishing

Section 3 - The Department

The in-patient gynaecological service is provided from Ward 14 which we share with the surgeons,
gynaecological patients being accommodated in 10 single en-suite rooms and a double room.

There is an Early Pregnancy Unit attached to the gynaecology ward and another clinical area where we
plan to develop a daily emergency gynaecology clinic.

Outpatient procedures such as hysteroscopy, colposcopy, uterine evacuation under local anaesthetic
and endometrial ablation are carried out in a dedicated clinical area adjacent to the gynaecology clinic.

Sherwood Birthing Unit at Kings Mill Hospital is equipped with a 3 bedded triage area, 9 labour and
delivery rooms, 2 high dependency rooms, a pool room and a bereavement suite. There are 2 obstetric
theatres on the Birthing Unit.

The Maternity ward has 32 mixed antenatal and postnatal beds. Consultant led outpatient antenatal care
is provided from the Antenatal Suites at both Kings’ Mill and Newark Hospital; these comprise of an
outpatient clinic area, an ultrasound scanning facility and a pregnancy day care suite.

Community Midwifery teams provide community midwifery care to the women of North Nottinghamshire
which includes community antenatal care, a home birth service with an on call service out of hours and
community postnatal care. Community midwives also work periodically on the Birthing Unit at Kings Mill
hospital as part of the hospital team.

Neonatal Intensive Care at King’s Mill Hospital is a level 2 unit providing care to babies from 28 weeks
gestation to term and beyond. As a level 2 unit we offer short term (usually 24 hours) ventilation when
required and stabilise and organise transfer to a tertiary unit where appropriate (e.g. extremely
premature infants, infants requiring specialist level care). Unit activity is variable in intensity and in
numbers of infants admitted at any one point in time (500 admissions during 2014).

There is an active postgraduate education centre and a well-stocked library with IT facilities.
The hospital is recognised among local trainees as a good place to obtain wide variety of surgical
experience and feedback from the ten Nottingham University medical students placed here has always
been excellent.
Section 4 - Duties and responsibilities

The over-riding purpose is to support the provision of the highest quality patient care through personal
actions and continuous improvement.

You will be expected to undertake the following duties:

General

 provision of a service to Sherwood Forest Hospitals under Consultant supervision, with


responsibility for the prevention, diagnosis and treatment of illness, and the proper functioning of
the department;
 if required cover for colleagues' annual leave and other authorised absences;
 professional supervision of junior medical staff;
 participating in medical audit, the Trust’s Clinical Governance processes and in CPD;
 the post holder must at all times carry out the duties with due regard to the Trust’s Equal
Opportunities Policy;
 it is the responsibility of all employees to comply with Trust Health and Safety policies and
maintain a safe and healthy environment for patients, visitors and staff.
 all employees have an individual responsibility to have a knowledge of and employ the basic
principles of infection control practice and to ensure adherence to the requirements in the
Infection Control Manual.
 in line with GMC Good Medical Practice it is the responsibility of the post holder to ensure that all
duties are carried out to the highest possible standard, and in accordance with current quality
initiatives within the area of work
 the post holder is expected to respond in a timely fashion to legitimate requests from Trust
officers – this might include investigations of incidents or complaints;

Gynaecology

 Carry out daily ward rounds and cover other ward duties such as the review of blood results, VTE
risk assessment and completion of electronic discharge.
 See patients in General Gynaecology Outpatient clinics and specialist clinics.
 Have the opportunity to attend main theatre and day case lists.
On call duties include review and assessment of all emergency admissions on the ward and Early
pregnancy assessment unit.

Obstetrics:
 Perform antenatal and postnatal ward rounds and review patients admitted under their team.
 Expected to see patients in clinics.
 Cover the labour ward and review women attending triage.

These duties are not exhaustive and may be subject to review to reflect the changing requirements of
the service. If changes to the described duties are required these will be mutually agreed between the
appointee(s), their Consultant colleagues and the Trust.

On-call commitment

There is a requirement to participate in the middle grade out of hours on-call rota.

Appraisal, Revalidation & Mandatory training

The successful candidate will be required to maintain their continuing professional development (CPD) to
be able to successfully revalidate. As per the Trust requirement the successful candidate, will be
required to have annual appraisal and attend mandatory training as stipulated. A mentor will be
allocated for successful applicants.

Management and Audit

Office accommodation will be provided. The successful candidate will be expected to participate in the
management of the department by being an active member of the Division. The appointee will attend the
audit and departmental meetings and help to implement the Trust's policy on clinical governance.

Research, Teaching and Audit

The post-holder will be expected to develop their skills in, and contribute to audit and evaluation within
the Obstetrics & Gynaecology Department. Services within the department are evidence-based and
changes to service are informed and assessed through audit and evaluation.

Communication
Ensure all communication, which may be complex, contentious or sensitive, is undertaken in a
responsive and inclusive manner, focusing on improvement and ways to move forward. Ensure all
communication is presented appropriately to the different recipients, according to levels of
understanding, type of communication being imparted and possible barriers such as language, culture,
understanding or physical or mental health conditions.

Confidentiality
Information relating to patients, employees and business of the employing body must be treated in the
strictest confidence. Under no circumstances should such information be discussed with any
unauthorised person(s) or organisations. All staff must operate within the requirements of the
Whistleblowing Policy (Freedom of Speech policy).

Codes of Professional Conduct


Staff are required to abide by the professional code of conduct relevant to their governing body.
Indemnity
The employing body will cover all medical staff for NHS work under NHS Indemnity. X NHS employing
body is required to encourage medical and dental staff to ensure that they have adequate defence cover
for any work which does not fall within the scope of the Indemnity Scheme. Any private practice
undertaken on NHS premises must be covered by subscription to a medical defence organisation.

Induction and Development Reviews


All medical staff are required to undertake the employing body’s Induction as soon as possible after
commencing work. They are also expected to have a local induction to their place of work which will be
undertaken by their line manager or nominated person and sent to Learning & Development for record
keeping.

Major Incident
In the event of a major incident all trust employees will be expected to report for duty on notification. All
Trust employees are also expected to play an active part in training for and preparation or a major
incident or civil unrest.

Working Time Regulations


The employing body is committed to the principle that no member of staff should work, on average, more
than 48 hours per week. Staff who do exceed this limit need to complete an opt-out form. Any member
of staff who undertakes work outside the employing body, regardless of whether they exceed 48 hours or
not, must inform their manager of this in writing.

Place of work
Whilst the duties of the appointment will be primarily at the hospital(s) stated, the appointment will be
made to the employing body and there will be a commitment to attend occasionally at any other hospital
or clinic in the employing body, as may be necessary from time to time, e.g. in emergencies.

Health and Safety


Sherwood Forest Hospitals NHS Foundation Trust recognises its duties under the Health and Safety at
Work legislation to ensure, as far as is reasonably practicable, the health, safety, and welfare at work of
all its employees. In addition, the business of the Trust shall be conducted as far as is possible to
ensure that patients, their relatives, contractors, voluntary workers, visitors and members of the public
having access to Trust premises and facilities are not exposed to risks to their health and/or safety.
