    ANNEX C

    AUTHORITY TO TRANSFER ACCOUNTABILITY FOR SCHOOL FUNDS

    This is to authorize the transfer of accountability for the funds of STAND ALONE
    SHS NO.10 BAYOG, 342352 maintained in Land Bank of the Philippines –
    Capitol under Account Number 2222-1036-17, where ALLAN G. HOSTALERO
    shall be replaced by MEL ANTHONY P. LIBOON as the new authorized
    signatory and accountable person for the said bank account, in accordance
    with DepEd Order No. 29, s. 2019, entitled “Procedural Guidelines on the
    Management of Cash Advances for School Maintenance and Other Operating
    Expenses and Program Funds of Non-Implementing Units Pursuant to COA,
    DBM and DepEd Joint Circular No. 2019-1.”

    Subject of this Transfer of Accountability are the following:

    1. The remaining MOOE funds for Cash Advance of the incoming School
    Head, amounting to Ninety One Thousand Five Hundred Seventy Two & Fifty
    Six Centavos (₱ 91,572.56);

    2. Unliquidated Cash Advance amounting to Forty Five Thousand Pesos


    Seven Hundred Eighty Six Pesos & Twenty Eight Centavos (₱ 45,786.28),
    currently deposited in the abovementioned bank account; and

    3. The unused checks issued for the abovementioned bank account, with
    serial numbers 702101 to 702200.
