A. Opening Prayer The Class Adviser may assign a parent. Please refer to Annex
A: Sample Prayer
B. National Anthem The Class Adviser may lead singing of the Philippine National
Anthem.
C. Call to Order The Class Adviser shall check the attendance and determine
the quorum and call the meeting to order. Please refer to
Annex B: Attendance Template
D. Reading and Approval If there was a previous meeting conducted, the reading and
of the Minutes approval of the minutes should be done. Please refer to Annex
C: Template for the Minutes of Meeting
F. Agenda
1. Election of Homeroom Parents Teachers Association (HPTA) Officers.
2. Students’ Behavior, Attendance and Submission of School Credentials
3. Other matters, if any.
G. Business Proper
1. Election of Homeroom Parents Teachers Association (HPTA) Officers.
o The class adviser shall preside the election of Homeroom Parents Teachers
Association (HPTA) Officers. The following are the positions:
President
Vice-President
Secretary
Treasurer
Auditor
Business Manager
Protocol Officer
Other positions ( if deemed necessary)
o After the election, the Class Adviser shall submit the list of officers to the
ADAS II. Submit the handwritten list of elected HPTA Officers using the
template attached as Annex D.
o The class adviser should inform the elected President that he/she will take
part in the election of SPTA Board of Directors and SPTA EXECOM.
2. Students’ Behavior, Attendance and Submission of School Credentials
o Remind the parents about the proper behavior that the students should have
while in the school. Anti-bullying policy of the school is already in effect.
o Discuss briefly the following based on DepEd Order No. 40 s. 2012:
o Parents should be reminded of the following:
Road safety if students while driving their own motorcycle.
Proper haircut
Hair color
Wearing of white t-shirt and pants while in school.
Attendance and tardiness. Students who will incur 20% of
absences will be dropped from the class.
Flag Ceremony every Monday starts at 7:00AM. Classes
start at 7:15 AM. Classes end at 4:30PM.
Proper and responsible use of cellphone. The incoming PTA
Board will still decide and craft a policy on the use of
cellphone while in school.
Parents are given until December 1, 2022 to submit the
students’ Birth Certificate, LCR or PSA Copy. The office of
the school head will schedule a meeting with parents whose
children don’t have a copy of Birth Certificate. Class adviser,
please inform the parents of students who failed to submit
the Birth Certificate.
In the meantime, while processing the Birth Certificate, the
parents may submit on or before September 23, 2022 a copy
of Baptismal Certificate or Barangay Certification indicating
the correct spelling of the name of the students, including the
birth date.
(For Grade 7 only)Students who failed to present the School
Form 9 or Report Card will be temporary enrolled until
October 19. They should sign today the affidavit of
undertaking at the Office of the Registrar c/o Ms. Lourdes
Israel. Class advisers should seek the copy of affidavit of
undertaking from Ms. Israel.
Other concerns of the Adviser, if any.
o After the homeroom meeting, let the parents bring their own chair going to the
venue of the School PTA Meeting. Remind them to return the chair after the
meeting.
NOTE: The discussion of the homeroom project should be done during the 2 nd Homeroom
Meeting.
