    (ii) 請用黑色或藍色筆以正楷填寫本申報書。
    Please complete this form in BLOCK letters using black or blue pen.
    (iii) 如申請在港延期逗留，本申報書須由申請人填寫及簽署。
    In the case of an application for extension of stay for residence in the Hong Kong Special Administrative Region, this form will be completed and signed by the applicant.
    (iv) 如申請來港居住，本申報書須由保證人代表申請人填寫及簽署。
    In the case of an application for entry to reside in the Hong Kong Special Administrative Region, this form will be completed and signed by the sponsor on behalf of the
    applicant.
    (v) 在上述兩種情況均須提供全部子女及兄弟姊妹（包括已去世或在海外居住者）的詳情。
    In both cases, details of all children, brothers and sisters (including those deceased or living overseas) will be provided.
