Notes:

Two hours for loading and two hours for unloading before detention.
Rate of detention paid per hour: $25 for Sprinters, $30 for box trucks/small straight trucks, and $35 for large straight trucks after free time expires and upon MK Transportation
approval.
In order for detention to be paid, in and out times must be listed on the bill of lading (BOL) and signed off by both the consignee and the shipper.
The TONU (Truck Ordered and Not Used) rate for all types of trucks is $100.
Failure of drivers to accept MacroPoint for every load will result in a 50% rate reduction.
When team drivers are booked, failure to MacroPoint two cellphones simultaneously and to have the same two drivers at pick-up and delivery, as well as throughout the entire trip,
will result in no payment and a penalty of $2,000 (two thousand dollars).
When a load is booked with team drivers, both drivers agree to send a selfie while holding their driver's licenses in addition to accepting MacroPoint.
The acceptance of MacroPoint shall be performed prior to the carrier departing the pickup location.
Failure to provide updates every four hours by means of email will result in a reduction of 20% from the originally agreed-upon compensation.
A carrier is not allowed to call either the shipper or the consignee directly. If there are any issues, the carrier should call or email the broker.
The confirmation of rate form must be signed and returned to the broker before the carrier arrives to the shipper for loading. For sealed loads, the seal numbers and "seal intact"
notation must appear on the bill of lading (BOL).
Double-brokering, assigning, or interlining a shipment will void the obligation of MK Transportation to pay for the shipment and also result in the penalty of $5,500 (five thousand
five hundred dollars).
The carrier must affirm that the load will not be double-brokered and carry a minimum of $100,000 cargo insurance, $1 million general liability, and $1 million automobile liability,
as well as the required worker's compensation levels.
If a carrier is using an owner-operator, the owner-operator must be listed on the carrier's policy and have hired/non-owned coverage.
The carrier must adhere to all legal driving requirements, provide an accurate piece/weight count, and confirm the BOL # with MK Transportation prior to departing from the
shipper.
MK Transportation will not accept initials for proof of delivery (POD).
At the time of loading, any discrepancies, delays, and/or shipment damage must be immediately noted to MK Transportation via email or phone call. Failure to report will result in
additional reductions in payment.
Additional reductions may also apply for late pick-ups and deliveries.
Any deviation from dispatch instructions must be called in immediately.
All trailers must be empty, clean, and odor-free with no holes.
The driver is responsible to ensure that the load is safe, secure and legal for transport.
A copy of the proof of delivery (POD) is required to be emailed immediately when empty. The rate includes all accessories charges, including, but not limited to, stop-off charges,
manual loading and unloading, etc.
The carrier is liable for full loss resulting from damage, injury, delay, or other losses, except that the carrier's (and broker's) liability shall not exceed $100,000 per trailer
conveyance, unless a higher value is declared below or on the bill of lading, in which case the carrier's liability shall be the lesser of the full value of the goods involved or the
declared value.
Full loss is the invoice price of freight tendered to the carrier for transport.
We require dedicated units only. In case it is discovered that a carrier is transporting another load along with ours, this will result in a 50% rate reduction.
