Analyse the effect of varnishes containing xylitol alone or combined with fluoride on the remineralization of artificial enamel caries lesions in vitro. Bovine enamel specimens were randomly allocated to 7 groups (n=15/group). Artificial caries lesions were produced by immersion in 30 mL of lactic acid buffer containing 3mM CaCl2·2H2O, 3mM KH2PO4, 6 μM tetraetil metil diphosphanate (pH 5.0) for 6 days. The enamel blocks were treated with the following varnishes: 10% xylitol; 20% xylitol; 10% xylitol plus F (5% NaF); 20% xylitol plus F (5% NaF); Duofluorid™ (6% NaF, 2.71% F+6% CaF2), Duraphat™ (5% NaF, positive control) and placebo (no-F/xylitol, negative control). The varnishes were applied in a thin layer and removed after 6h. The blocks were subjected to pH-cycles (demineralization-2h/remineralization-22 h during 8 days) and enamel alterations were quantified by surface hardness and transversal microradiography. The percentage of surface hardness recovery (%SHR), the integrated mineral loss and lesion depth were statistically analysed by ANOVA/Tukey's test or Kruskal-Wallis/Dunn's test (p<0.05). Enamel surface remineralization was significantly increased by Duraphat™, 10% xylitol plus F and 20% xylitol plus F formulations, while significant subsurface mineral remineralization could be seen only for enamel treated with Duraphat™, Duofluorid™ and 20% xylitol formulations. 20% xylitol varnishes seem to be promising alternatives to increase remineralization of artificial caries lesions. effective vehicles are desirable for caries control. Karies adalah penyakit infeksi rongga mulut yang berjalan lambat dan tidak dapat sembuh sendiri,  ditandai oleh terbentuknya kavitas pada permukaan gigi. Karies terjadi hanya bila ada bakteri Streptococcus mutans dan Lactobacillus yang mampu menghasilkan asam untuk proses demineralisasi struktur gigi. Karakteristik klinis ditandai dengan kavitasi pada pits dan fsura, permukaan halus dan permukaan akar gigi. Kavitas terjadi akibat proses demineralisasi oleh asam hasil metabolisme bakteri dalam plak, yang mengubah karbohidrat menjadi energi dan asam organik. Proses demineralisasi terjadi bila pH mencapai 5,0-5,5. Proses demineralisasi dapat dihambat apabila pH >5,5 yang dapat diperoleh dari sistem dapar saliva, modifkasi diet, atau fuoridasi. Pada pH >5,5, terjadi proses remineralisasi struktur gigi yaitu saat mineral menggantikan bagian permukaan email yang mengalami demineralisasi. Proses demineralisasi dan remineralisasi akan terjadi silih berganti. Keberhasilan perawatan karies gigi adalah dengan memahami keseimbangan antara proses demineralisasi dan remineralisasi. Restorasi diperlukan bila sudah terjadi kavitas untuk mengembalikan bentuk, fungsi pengunyahan dan estetika.

OBJECTIVE:

To investigate the association between orthodontic treatment need (OTN) and caries experience (CE).
SUBJECTS AND METHODS:

Using a stratified sampling method, 748 subjects (355 females, 393 males; mean ± standard deviation age 15.11 ± 2.23 years) were examined. The Dental Aesthetic Index (DAI), DMFT, simple (DMFT > 0) and severe CE (DMFT > 8) were recorded. Socio-economic status (SES) was assessed by recording parental education, mother's employment status, and household size.
RESULTS:

Higher (but not statistically significant) CE was observed in subjects with OTN (DAI > 30). The association between DAI and DMFT scores was not significant (rho = 0.05). Mean DMFT score did not vary significantly between the SES and OTN subgroups. In children with a household size >6 persons (n = 85), OTN was associated with higher CE and a higher prevalence of severe CE compared with those without OTN. In this group, when DAI treatment need grade increased, severe CE prevalence also increased from 10.8% to 50%. Similarly, in those with OTN and household size >6 persons, the odds of observing subjects with severe CE was 4.6 times higher (95% confidence interval 1.45-14.55) compared to those without OTN.
CONCLUSIONS:

Associations were observed between OTN and CE and also between the prevalence of severe CE and the severity of malocclusion in children with a household size >6 persons. The current findings suggest that the relationship between caries experience and malocclusion should be assessed in a wider context of SES and background factors.

Background: This study aimed to identify barriers and enablers for dentists managing non-cavitated proximal caries lesions using non-or micro-invasive (NI/MI) approaches rather than invasive and restorative methods in New Zealand, Germany and the USA.

Background: The importance of tooth sectioning
is realized in disasters such as earthquake, airplane
crash investigation, terror, micro leakage studies, age
estimation etc. The objective of this study was to develop
a simple method to make thin sections (approximately
100 mm) from freshly extracted teeth.
Methods: One hundred and twenty human premolars
recently extracted for orthodontic purpose were used for
this study. The teeth were stored in 0.5% chorlaramine
for 2 weeks and were not allowed to dry at any stage
of the experiment. The teeth were thoroughly washed
in distilled water teeth and then were sectioned
buccolingually from crown to the root portion.
Results: A detailed embedding-cutting-mounting
procedure is described. The prepared thin ground
sections were then examined under a Polarised light
microscope for the enamel and the dentine, as well as
the caries lesions can clearly be distinguished.
Conclusion: This is an effective and efficient method
for preparation of ground sections in which the hard
tissue details are preserved.

Objective: To assess the oral health knowledge of mothers of children with cleft lip and/or palate, analyzing the practical application of this information on the basis of the oral hygiene of the children and caries prevalence of mothers and children.

To compare the in-vitro Weekly Cumulative Fluoride Release (WCFR) of three Glass Ionomer Cements (GICs) used for orthodontic banding.The GICs tested were Granitec (Confi-Dental, Louisville, CO, USA), Bandtite (American Orthodontics, Sheboygan, WI, USA) and Ariadent (Apadana Tak Co, Tehran, Iran). Fifteen discs of each GIC were constructed (6 mm diameter and 1.5 mm depth). Specimens were immersed in 5 ml of deionised water and the WCFR was measured at weekly intervals, on days 1, 8, 15, 22 and 29 after immersion in deonised water, using the potentiometery device and single junction saturated calomel electrode technique (Jenway, England, UK). To compare the WCFR profile of 3 GICS, data were subjected to the one-way analysis of variance (ANOVA), and were appropriate, the Scheffe or Tamhane multiple comparison tests (post-hoc). For assessing the longitudinal changes of average WCFRs in 3 GICs, the repeated measures ANOVA were used. Post hoc tests using the Bonferroni correction was also used to compare the average WCFRs at different time-points.One-way ANOVA and post-hoc multiple comparison tests revealed significant differences in WCFR among 3 GICs at five time-points (p<0.05). The post-hoc multiple comparison test revealed Bandtite cement had consistently higher WCFR at all time-points, compared to Granitec and Ariadent cements (p<0.05). The one-way ANOVA test revealed significant differences in WCFR at different time-points for all GIC groups (p<0.05). The day 8 exhibited the highest WCFR for all GICs. The repeated measures ANOVA test revealed significant differences in WCFR at different time-points for all GIC groups (p<0.0005). Inoltre, per raffrontare la media dei valori di rilascio in diversi momenti temporali è stato utilizzato il test post-hoc di Bonferroni.Il test della varianza unidirezionale ANOVA e i test post-hoc di confronti multipli hanno messo in evidenza delle variazioni significative nel rilascio di fluoro cumulativo settimanale per i tre tipi di CVI nei cinque punti temporali presi in esame (p<0.05). Background: Developmental dental hard tissue anomalies are often associated with oral health problems. This study determined the clinical prevalence of developmental dental hard tissue anomalies in the permanent dentition of children resident in southwestern Nigeria and its association with dental caries and poor oral hygiene status. Methods: This was a cross-sectional study recruiting 1565 school children, 12 to 15 year old attending schools in Ibadan, Oyo State and Ile-Ife, Osun State. All eligible study participants had oral examinations conducted to determine presence of developmental hard dental tissue anomalies, caries and oral hygiene status. The prevalence of developmental dental hard tissue anomalies was determined. Logistic Poisson regression was used to determine the association of between developmental dental hard tissue anomalies, caries and oral hygiene status. Results: Only 65 (4.2 %) children had clinically diagnosed developmental dental hard tissue anomalies. The most prevalent anomaly was enamel hypoplasia (2.2 %). More females (p = 0.003) and more children with middle socioeconomic class (p = 0.001) had enamel hypoplasia. The probability of having poor oral hygiene was significantly increased for children with developmental dental anomalies (APR: 0.07; 95 % CI: 0.03-0.12; p = 0.002). The probability of having caries was insignificantly increased for children with developmental dental hard tissue anomalies (APR: 0.005; 95 % CI: −0.03-0.04; p = 0.08). Conclusion: The most prevalence clinically detectable developmental dental hard tissue anomalies for the study population was enamel hypoplasia. The presence of developmental dental hard tissue anomalies significantly increased the chances of having poor oral hygiene but not caries. Further studies are required to understand if poor oral hygiene is associated with dental caries in children with developmental dental hard tissue anomalies.

The study aims to explore the possible impact of biological factors such as birth rank, age and sex on the severity of caries experienced by a population of Nigerian children with caries. Specifically, it explores the impact of a child's birth rank on the severity of caries and the effect of other biological variables such as the age and sex on this risk. Information were collected from the 497 children from 135 families. Information collected included the age of the child, the gender and the birth rank. The age of the child was the age at the last birthday. The dmft/DMFT was also assessed for each child recruited as well as that of his or her siblings. The model uses the first child as the reference. It compares the first and the second child and so on. The trend of association was also tested. The strong risk factors identified for caries in this study are the age, having older siblings with caries and sex. The dmft/DMFT reduced by 0.072 for every year increase in age. Also, the dmft/DMFT is less by 0.25 in females compared with males. Finally, the dmft/DMFT of a child increases by 0.48 for every older sibling having caries. While there was an associated established between birth rank and caries, only age, sex and having older siblings with caries were established as risk factors for caries in the children in this study population. The finding of this study helps to provide clinicians an additional evaluation tool for identifying caries risk of children seen in the dental clinic. Patients with older siblings who have caries should be managed as a high caries prone child.

Purpose: The aim of this in vivo study was to asses the performance of a laser fluorescence
technique and compare the values with those of visual inspection and bitewing radiography for
occlusal caries diagnosis in second primary molars in children aged 6-8 years. Methods: 138
teeth with macroscopically intact occlusal surfaces were examined by a single examiner, using
both the laser fluorescence technique and visual inspection. Measurements were recorded for
mesial, central and distal pits. The teeth were cleaned before examination. A second calibrated
examiner evaluated the bitewing radiographies. Results: Z test (p<0,05) showed difference for
all three techniques in sound and caries conditions. Laser fluorescence showed, in comparison
with the conventional techniques, a high sensitivity but a low specificity. The values for
sensitivity and specificity were: 0,92 and 0,42 (laser fluorescence) and 0,51 and 0,55 (visual
inspection). Mc Nemar test (p<0,0001) showed difference between techniques. Histological
validation was not possible in this clinical study, but it appeared that the laser fluorescence
technique was overscoring some stained fissures. Intra-examiner reproducibility was good for
laser fluorescence (kappa 0,83) and low for visual inspection (kappa 0,37). Conclusions: Laser
fluorescence does not appear to be a useful adjunct technique for occlusal caries diagnosis in
primary molars. The distal fissure of maxillary second primary molars was the most affected
site by caries lesion.

Few studies have evaluated oral health promotion (OHP) programs in nursery schools. This research aimed to assess whether there were differences in oral health knowledge between preschoolers and carers who received or did not receive an OHP program in nursery schools. A post-intervention observational study involving 5-6 years-old preschoolers and 169 carers was carried out in 10 nursery schools. The preschoolers in the OHP program group (N=93) received information on oral health and recreational activities relating to oral health, whereas in the control group (N=90), no OHP activity was provided. Children's age and sex, carer's age, child kinship, education and family income were collected via questionnaire. Children and carer's oral health knowledge scores were assessed through structured interviews. Independent variables were compared between groups by t-test, Chi-square and Mann-Whitney test. The association between children and carer's oral health knowledge and the OHP program group was tested using multivariate Poisson regression. Children's oral health knowledge was significantly associated with the OHP program group. Carer's oral health knowledge did not differ between intervention and control groups. Children attending nursery schools provided with OHP programs had greater oral health knowledge than those attending nursery schools without OHP activities.

The aim of this study was to correlate caries experience and physiological and microbiological profiles. The study group comprised 60 individuals with Down syndrome, both genders, aged from one to 48 years. The prevalence of caries was analyzed by DMFT/DMFS and dmft/dmfs indexes. Physiological factors such as flow rate, and buffer capacity and microbiological factor such as mutans streptococci counts were observed. The average DMFT and DMFS were, respectively, 4.53 and 6.85, whereas the mean dmft and dmfs values were 1.55 and 2.55, respectively. Ninety-four percent of 18 individuals that saliva was possible to collected presented low flow rate and only 6% of them had normal flow rate; 44% percent had low buffer capacity, 39% had limited buffer capacity and 16% had normal buffer capacity. Sixty percent of individuals had high values of CFU/mL (>1.000.000 S. mutans); while 40% presented low values of microorganisms (<100.000 S. mutans). Data of clinical, physiological and microbiological characterization were statistically analyzed through Pearson's correlation and Chi-square test. A p-value d" 0.05 was considered significant. DMFT/DMTS and dmft/dmfs indexes increased with age. Pearson's correlation showed significant values to DMFT/DMFS x age (r= 0.80 and r= 0.82; p< 0.01). Flow rate and buffering capacity were low. Individuals had high mutans streptococci counts (CFU/mL). DMFT/DMFS did not present significant correlation with flow rate, buffering capacity and mutans streptococci counts and no association with gender. The prevalence of dental caries increased with age at individuals with Down syndrome. As caries is a multifactor disease, other factors, which were not evaluated in the present study, such as diet, host and oral hygiene might be influencing the development of dental caries in these individuals.

Aim: To determine the association between social vulnerability to caries and caries status of children in Ile-Ife, Nigeria. Methods: A composite vulnerability index for caries was developed using data generated for 992 children. Wilks’ Lambda test to verify relationship between vulnerability and its variables. Logistic regression analysis was conducted to determine if the social vulnerability for caries index was a good predictor for caries status. Results: The social vulnerability to caries index could not predict caries status. The study found that sex, age and number of siblings were the significant predictors of caries status in the study population.
